Alkaptonuria—an atypical case: multi-modality imaging review

Abstract

Alkaptonuria is a rare inborn metabolic disorder due to a mutation in the homogentisic acid oxidase enzyme (HGO) gene on chromosome 3q. As HGO is deficient in alkaptonuria patients, there is an accumulation of homogentisic acid in the blood and urine. Homogentisic acid gets deposited in the soft tissues, tendons, cartilages, large joints and intervertebral discs. Ochronosis usually affects the dorsolumbar spine and typically spares the cervical spine and sacroiliac joints. However, in this case of isolated ochronosis, we report co-existent extensive cervical spine degenerative changes and cervical vertebral fusion, which has not been described in the literature so far.

Three-dimensional force analysis of surgical manipulations at the long process of the incus

Abstract

Purpose

Surgical manipulation with application of inappropriate force may damage middle ear structures leading to hearing loss. This work analyzes the forces applied in simulated otosurgical exercises in a laboratory set-up by measuring the spatial components of applied forces with objective assessment criteria. With these criteria, the individual force characteristics applied by the surgeon can be quantified and an objective feedback can be given about their surgical maneuvers.

Methods

A natural size model of the human incus was mounted on a load cell to measure the spatial forces in all three directions during different manipulation tasks performed under the microscope by ten surgeons from our department having different levels of experience in otosurgery. The motions of the incus model and the instrument tip were recorded simultaneously with a video camera.

Results

Independent of surgical experience, a three-dimensional force pattern could be detected with components transverse to the desired force directions. The measured forces applied by trainees showed larger variations in magnitude, in spatial distribution and in temporal course than those applied by experienced surgeons. A better repeatability of identical tasks, constancy of force patterns and low peak force values could be seen in the group of experienced surgeons.

Conclusions

The laboratory system presented in this study using simultaneous video and 3-D force registration allows the objective assessment of surgical manipulations, e.g., at the long process of the incus. Training with video and force feedback provides information about surgical techniques and skill development of surgeons and has the potential to shorten the learning curve and to diminish intra-operative risks to patients.

Perioperative Stress, Inflammation, and Cancer Progression: Opportunities for Intervention in Breast and Colorectal Cancer Surgery Utilizing Beta-Adrenergic Blockade and COX-2 Inhibition

Abstract

Purpose of review

This review explores the blockade of cyclooxygenase (COX)-2/prostaglandin and catecholamine/beta(β)-adrenergic signaling as opportunities to enhance the curative potential of surgical excision of breast and colorectal cancer malignancies.

Recent findings

Phase-II randomized placebo-controlled biomarker clinical trials, employing perioperative administration of the COX-2 inhibitor etodolac and the β-adrenergic antagonist propranolol, have shown improved biomarkers associated with both breast and colorectal cancer progression. Tumor epithelial-to-mesenchymal transition (EMT) status and tumor transcription activity levels of GATA-1, GATA-2, EGR3, and STAT-3 were reduced, while anti-cancer immune parameters within the tumor microenvironment improved perioperatively. Increased tumor-associated B cells and NK cells and reduced tumor-associated monocytes were noted. Reduced systemic pro-inflammatory markers IL-6 and CRP were indicated, starting before surgery.

Summary

Improved biomarkers of tumor-associated (i) anti-metastatic transcriptional activity, (ii) anti-metastatic immune function, and (iii) reduced systemic inflammatory indices were observed following the combined administration of etodolac and propranolol. These findings support future investigation with larger clinical trials to test long-term cancer outcomes.

MicroRNA let-7c Improves LPS-Induced Outcomes of Endometritis by Suppressing NF- κB Signaling

In this study, we confirmed that let-7c was significantly reduced in LPS-induced mouse endometritis model, and overexpression of let-7c was able to effectively reduce uterine tissue damage caused by lipopolysaccharide (LPS), and then, a LPS-induced bovine endometrial epithelial cell (BEND) line was used to mimic the inflammatory modelin vitro. Our data showed that overexpression of let-7c significantly reduced the expression of pro-inflammatory cytokines in BEND cells induced by LPS. Meanwhile, immunofluorescence and western blotting results showed that let-7c significantly inhibited LPS-induced phosphorylation of NF- κB, thereby inhibiting downstream pro-inflammatory cytokine expression. Fibular groove morphology and measurements on MRI: correlation with fibularis tendon abnormalities

Abstract

Purpose

Fibular (peroneal) groove morphology may influence fibularis tendon pathology, including tendinosis, tears, and luxation. The study goal was to evaluate the inter-reader agreement of morphologic characterization and measures of the fibular groove at two different levels on MRI and correlation with fibularis tendon pathology.

Materials and methods

47 ankle MRIs in patients without lateral ankle pain were reviewed by two musculoskeletal radiologists. Fibular groove morphology and various measurements were assessed at both the level of the tibial plafond and 1 cm proximal to the tip of the lateral malleolus. Fibularis tendon pathology and other variants were also recorded. Intraclass correlation (ICC) and kappa statistic (κ) were applied to assess inter-observer agreement. Receiver operating characteristic (ROC) and area under the curve (AUC) analysis were performed to determine correlation between fibular groove morphometry and fibularis (peroneus) brevis tendon tears.

Results

Between readers, there was fair-to-excellent agreement (ICC = 0.61–0.95) for performed fibular groove measurements and moderate-to-very good agreement for identification and description of fibular groove and fibularis tendon morphology and pathology and normal variants in this region (κ = 0.46-1), with the exception of fibular groove morphology at 1 cm proximal to the lateral malleolar tip (κ = 0.34). Individually, no measurement or description of pathology could discriminate between patients with or without fibularis brevis tendon tears except fibularis brevis tendinosis (AUC = 0.87 for reader 1).

Conclusion

There is overall moderate-to-excellent inter-reader agreement for various measurements and descriptors of fibular groove and fibularis tendon morphometry and pathology, including novel measurements introduced in this study.

Paralytic Lagophthalmos: Comprehensive Approach to Management

Abstract

Purpose of Review

Facial paralysis can lead to lagophthalmos, loss of corneal protection, and devastating ocular consequences like corneal ulcers or even blindness. The purpose of this review is to discuss the importance of comprehensive and early eye care.

Recent Findings

Treatment of paralytic lagophthalmos should be individualized to each patient's situation; depending on the severity of the problem and the prognosis for recovery, treatment modalities are broad and include lubrication, selective chemodenervation, filler injection, fat augmentation, and surgical procedures like canthopexy and tarsorrhaphy.

Summary

This paper serves to highlight the importance of treating paralytic lagophthalmos and provides an overview of the optimal management algorithm of the compromised periocular complex to help improve function and quality of life. Prospective studies and long-term comparisons of surgical techniques for repair of paralytic lagophthalmos will help improve our understanding of this complex problem and perhaps introduce novel treatment strategies.

Stimm- und Kehlkopfveränderungen im Alter (Presbyphonie und Presbylarynx)

Etwa 20 % der über 60-Jährigen weisen eine Altersstimmstörung (Presbyphonie) auf, die oftmals von Presbyakusis (Altersschwerhörigkeit), Presbyphagie (Schluckstörungen im Alter), Presbyvertigo (Altersschwindel) und/oder Presbyopie (Alterssichtigkeit) begleitet wird.Presbyphonie betrifft die Singstimme häufiger, früher und deutlicher als die Sprechstimme. Aber nicht nur Qualität und Belastbarkeit von Sing- und Sprechstimme werden beeinträchtigt, auch der Atemantrieb wird durch die eingeschränkte Lungenfunktion schwächer. Durch das Absenken des Kehlkopfes verändern sich bei Senioren zudem die Resonanzverhältnisse im Ansatzrohr, und das Aspirationsrisiko steigt.Die stimmlichen Einbußen beruhen auf morphologischen Altersveränderungen des Kehlkopfes (Prebylarynx), die sowohl das ektolaryngeale Kehlkopfskelett als auch die endolaryngealen Weichteile (insbesondere die Stimmlippen) betreffen.Der Vokalismuskel atrophiert und wird hypoton, was zu einer spindelförmig-ovalären Glottisschlussinsuffizienz mit Luftverlust bei Phonation führt. Die Schleimhäute trocknen aus im Sinne einer Laryngitis sicca.Bei der Stimmrehabilitation im Alter müssen allgemeine und speziell auf die Stimme bezogene Behandlungsoptionen unterschieden werden: Zu den allgemeinen Therapieansätzen gehört die Mitbehandlung internistischer und neurologischer Grunderkrankungen (z. B. Reflux, neurodegenerative oder zerebrovaskuläre Erkrankungen) – auch Nebenwirkungen von Medikamenten müssen beachtet werden. Ein gesunder Lebensstil ohne Noxen begünstigt den Funktionserhalt von Phonations- und Atemorganen. Stimmtherapeutisch kommen sowohl Tonisierungsübungen zur Verbesserung des phonatorischen Glottisschlusses als auch detonisierende Entspannungsübungen zum Abbau supraglottischer hyperfunktioneller Pressmechanismen in Betracht. In Einzelfällen mit großem Glottisspalt bei Phonation kann eine phonochirurgische Stimmlippenaugmentation den Glottisschluss verbessern und somit die Stimme kräftigen. Stimmhygienische Maßnahmen (z. B. regelmäßiges Befeuchten der Atemwege) ergänzen das Stimmtraining.Singen im Alter hat sowohl psychosozial (Steigerung des Wohlbefindens und der sozialen Teilhabe) als auch immunologisch (Anstieg des IgA im Speichel) positive Effekte.

In der hier vorgelegten Darstellung der Entwicklung der Schilddrüsenchirurgie hin zu einer sicheren Chirurgie, die auf dem aktuellen chirurgischen und medizinhistorischen Forschungsstand und der verfügbaren einschlägigen Literatur basiert, wird die Entwicklungskontinuität hin zu den minimalinvasiven Operationsverfahren, die den dank der historischen Entwicklung erreichten hohen Sicherheitsstandards mindestens genügen müssen, erstmals im medizingeschichtlichen Kontext interpretiert. Das zunehmende Verständnis der Erkrankungen der Schilddrüse und der perioperativen Folgen und Komplikationen, die zu neuen medizinischen und therapeutischen Erkenntnissen führten, wird somit erstmals auch in diesem Kontext interpretiert.Zur Verbesserung des Patientenkomforts im Hinblick auf die Operationswunde und spätere Narbenbildung werden seit Beginn des 21. Jahrhunderts immer wieder neue minimalinvasive Operationsmethoden beschrieben, die sich durch die Zugangsmodalitäten und -wege unterscheiden. Zukünftige Weiterentwicklungen der operativen Zugangswege und Methoden müssen dem heutigen hohen Stand der chirurgischen und nuklearmedizinischen Therapie und ihren Qualitäts- und Sicherheitsstandards mindestens genügen.

Stimm- und Kehlkopfveränderungen im Alter (Presbyphonie und Presbylarynx)

Etwa 20 % der über 60-Jährigen weisen eine Altersstimmstörung (Presbyphonie) auf, die oftmals von Presbyakusis (Altersschwerhörigkeit), Presbyphagie (Schluckstörungen im Alter), Presbyvertigo (Altersschwindel) und/oder Presbyopie (Alterssichtigkeit) begleitet wird.Presbyphonie betrifft die Singstimme häufiger, früher und deutlicher als die Sprechstimme. Aber nicht nur Qualität und Belastbarkeit von Sing- und Sprechstimme werden beeinträchtigt, auch der Atemantrieb wird durch die eingeschränkte Lungenfunktion schwächer. Durch das Absenken des Kehlkopfes verändern sich bei Senioren zudem die Resonanzverhältnisse im Ansatzrohr, und das Aspirationsrisiko steigt.Die stimmlichen Einbußen beruhen auf morphologischen Altersveränderungen des Kehlkopfes (Prebylarynx), die sowohl das ektolaryngeale Kehlkopfskelett als auch die endolaryngealen Weichteile (insbesondere die Stimmlippen) betreffen.Der Vokalismuskel atrophiert und wird hypoton, was zu einer spindelförmig-ovalären Glottisschlussinsuffizienz mit Luftverlust bei Phonation führt. Die Schleimhäute trocknen aus im Sinne einer Laryngitis sicca.Bei der Stimmrehabilitation im Alter müssen allgemeine und speziell auf die Stimme bezogene Behandlungsoptionen unterschieden werden: Zu den allgemeinen Therapieansätzen gehört die Mitbehandlung internistischer und neurologischer Grunderkrankungen (z. B. Reflux, neurodegenerative oder zerebrovaskuläre Erkrankungen) – auch Nebenwirkungen von Medikamenten müssen beachtet werden. Ein gesunder Lebensstil ohne Noxen begünstigt den Funktionserhalt von Phonations- und Atemorganen. Stimmtherapeutisch kommen sowohl Tonisierungsübungen zur Verbesserung des phonatorischen Glottisschlusses als auch detonisierende Entspannungsübungen zum Abbau supraglottischer hyperfunktioneller Pressmechanismen in Betracht. In Einzelfällen mit großem Glottisspalt bei Phonation kann eine phonochirurgische Stimmlippenaugmentation den Glottisschluss verbessern und somit die Stimme kräftigen. Stimmhygienische Maßnahmen (z. B. regelmäßiges Befeuchten der Atemwege) ergänzen das Stimmtraining.Singen im Alter hat sowohl psychosozial (Steigerung des Wohlbefindens und der sozialen Teilhabe) als auch immunologisch (Anstieg des IgA im Speichel) positive Effekte.

Sweet syndrome: a rare feature of ANCA-associated vasculitis or unusual consequence of azathioprine-induced treatment

Abstract

Background

Sweet syndrome is a rare skin condition characterised by fever, neutrophilia, and tender erythematous skin lesions and has been reported to occur in association with anti-neutrophil cytoplasmic antibodies (ANCA) as well as complicate treatment with azathioprine therapy. Azathioprine, a relatively safe immunosuppressive, is frequently used to maintain disease remission in the treatment of ANCA-associated vasculitis. The occurrence of Sweet syndrome in a patient with ANCA-positive vasculitis and following treatment with azathioprine prompted us to present this clinical case and share this unusually rare occurrence. In doing so, we also wish to discuss current understanding of the disease and plausible associations.

Case presentation

Herein, we discuss the case of a 54-year old white male, who presented with features of ANCA vasculitis with haemoptysis, arthralgia, abnormal kidney function with active urine sediment and a positive p-ANCA titre. Standard immunosuppressive treatment with corticosteroids and intravenous rituximab resulted in disease remission. Due to significant steroid side effects, his steroid treatment was gradually tapered and switched to azathioprine over a 6-month period. Two weeks following initiation of azathioprine, he developed a painful maculo-papular erythematous skin rash and fever. A skin biopsy confirmed classical features consistent with Sweet syndrome. Withdrawal of azathioprine and treatment with oral corticosteroids and colchicine therapy resulted in complete resolution of the rash, although he continued to have high titres of MPO positive ANCA.

Conclusion

Sweet syndrome is a rare adverse reaction to azathioprine but has also been reported to occur in association with ANCA vasculitis. The temporal association with azathioprine in our case and the relatively rapid resolution of the skin vasculitis upon its withdrawal suggested a primarily drug-induced reaction rather than an associated feature of ANCA vasculitis.

Component resolved analysis of ash pollen allergy in Bavaria

Abstract

Background

Sensitization to ash pollen is underestimated in various regions. The prevalence in Germany is about 10%. However, allergy to ash pollen is widely overlooked by allergists, since the pollination period of ash and birch in central Europe closely overlap and rhinoconjunctival symptoms during April/May are often assigned to birch pollen. Component resolved analysis of the different ash allergens is not routinely available. Therefore, we would like to question the usefulness of component resolved diagnostic via olive components, as ash and olive are both part of the Oleaceae family.

Methods

113 patients with nasal provocation and skin prick test to ash were retrospectively compared regarding their specific immunoglobulin E antibody profiles with response to native ash extract, rOle e 1, nOle e 7 and rOle e 9.

Results

In nasal provocation testing 58% of 113 patients sensitized to ash were allergic, 42% were only sensitized without showing symptoms. Skin prick testing and serology against native ash extract detected most patients sensitized to ash pollen, whereas rOle e 1 was less sensitive. However, the value of measurements of skin prick test, serology to native ash extract and rOle e 1 did not allow a differentiation between an allergy and clinically silent sensitization. Specific antibodies to nOle e 7 and rOle e 9 were only seen in individual patients and were all positive for native ash extract and rOle e 1.

Conclusion

Skin prick testing and serology to native extract of ash pollen are the most reliable tools to diagnose a sensitization to ash pollen for patients living in Germany. Component resolved diagnostic to the major allergen rOle e 1 as representative of the Oleaceae family is possible but was less sensitive. Diagnostic of nOle e 7 and rOle e 9 did not show any additional benefit. Regarding differentiation between allergy and clinically silent sensitization to ash pollen, provocation is the leading diagnostic tool. Concluding, in routine clinical practice the standard methods—skin prick test, serology to native ash extract and provocation testing—remain crucial in the diagnosis and differentiation of ash sensitization and allergy.

It is characterized by hemolytic anemia, vaso-occlusive crises, relentless end-organ injury, and premature death. Currently, red blood cell transfusion and hydroxyurea are the major disease-modifying therapies available for SCD. Hematopoetic stem cell transplant is curative, but barriers to treatment are substantial and include a lack of suitable donors, immunologic transplant rejection, long-term adverse effects, prognostic uncertainty, and poor end-organ function, which is especially problematic for older patients.
