Morphological evaluation of the calcaneofibular ligament in different ankle positions using a three-dimensional MRI sequence

Abstract

Purpose

Evaluating images of the lateral ligament of the ankle is not easy, and evaluation of the calcaneofibular ligament (CFL) in particular is difficult. We prospectively conducted morphological measurements of the CFL in different ankle positions and obtain basic data for use in functional assessment of the CFL, diagnosis of CFL injury, and determination of treatment effects.

Methods

The subjects were ten healthy volunteers (ten ankles) with a mean age of 27.8 years and no history of ankle disease. Imaging was done using a 3-T magnetic resonance imaging (MRI) machine and fast imaging employing steady-state acquisition cycled phases (FIESTA-C), a three-dimensional (3D) sequence, with the ankle in a neutral position, maximum dorsiflexion, and maximum plantar flexion. 3D images of the CFL, peroneal muscle tendons, fibula, and calcaneus were prepared at a workstation, and morphological measurements of the CFL were made.

Results

In all positions, the CFL showed a gently curving course with the peroneal muscle tendons as a fulcrum. The tortuosity angle was significantly smaller in plantar flexion (30.0° ± 7.4°) than in the neutral position (41.7° ± 8.3°).

Conclusions

3D MRI sequences showed that, in all positions, the CFL curved due to the influence of the peroneal muscle tendons. With maximum plantar flexion, the CFL tortuosity angle was small, which was thought to have been due to the tension in the CFL.

The radioanatomical assessment of the Körner’s septum

Abstract

Purpose of the study

Körner's septum (KS) is a developmental remnant formed at the junction of mastoid and temporal squama, representing the persistence of the petrosquamosal suture. During mastoid surgery, it could be taken as a false medial wall of the antrum so that the deeper cells might not be explored. The aim of the study was to assess a Körner's septum prevalence and to analyze its topography.

Methods

The study was performed on 80 sets of cone-beam computed tomography (CBCT) images of temporal bone (41 male, 39 female, 160 temporal bones). Körner's septum was identified and its thickness was measured on axial sections at three points: at the level of superior semicircular canal (SCC), at the level of head of malleus (HM) and at the level of tympanic sinus (TS).

Results

KS was encountered at least in one point of measurements in 50 out of 80 sets of CBCT images (62.5%). The average thickness at the level of SCC was 0.87 ± 0.34 mm, at the level of HM was 0.99 ± 0.37 mm and at the level of TS was 0.52 ± 0.17 mm.

Conclusions

Körner's septum is a common structure in the temporal bone–air cell complex. It is more often encountered in men. In half of the patients, it occurs bilaterally. However, in most of the cases it is incomplete with anterior and superior portions being the most constant.

A unique case of an accessory sartorius muscle

Abstract

The sartorius muscle (SM) is a long strap muscle originating from the anterior superior iliac spine and inserting onto the medial surface of the proximal tibia. It crosses the anterior compartment of the thigh obliquely and descends towards the medial aspect of the knee. We found an accessory sartorius muscle (ASM) from the inguinal ligament and an original SM bifurcated into medial and lateral heads. The ASM merged with the medial head of the SM and inserted on the medial aspect of the tibia as the pes anserinus. The lateral head of the SM continued inferiorly and inserted on the medial aspect of the patella. We report a unique variation in the morphology of the SM, and discuss its functional and clinical implications.

Predictive factors for complications associated with penetrated fish bones outside the upper gastrointestinal tract

Abstract

Purpose

To investigate predictive risk factors for complications associated with migrating fish bones in the surrounding tissue of upper gastrointestinal tract.

Methods

A retrospective analysis over 12 years was conducted of 45 cases of buried fish bones in the surrounding tissue of upper gastrointestinal tract with complications. Meanwhile, a control group, including 39 cases of prolonged buried fish bones in the surrounding tissue of upper gastrointestinal tract without complications, was set. Patient clinical data were collected and analyzed to predict the risk factors for complications.

Results

The results of Chi-square test and univariate analysis both showed a significant difference in length of fish bone (> 2 cm), a history of concurrent medical illness (diabetes mellitus and renal hypofunction), symptoms (medium or heavy pain and dysphagia), and duration of significant symptoms (> 7 days) between the complication group and non-complication group. Multivariate analysis further identified length (> 2 cm), diabetes mellitus, medium or heavy pain, dysphagia, and duration of significant symptoms (> 7 days) as independent risk factors for complications.

Conclusions

The consequences of fish bones migrating outside the upper gastrointestinal tract are various in different people. Awareness should be raised when encountering a patient ingesting a long fish bone, having a history of diabetes mellitus, presenting with significant discomforts, or these discomforts lasting for a long time. This study will help practitioners counsel their patients on the risks and `benefits of surgery versus observation of this condition.

Conclusions The faster RTs indicated that AV input did not enhance detection in any group, but the slower RTs indicated that AV speech and dynamic V speech (mouthing) significantly minimized attentional lapses and thus did influence performance. Conclusions The results suggest that, for hearing aid users, poor speech understanding in noise and sensitivity to both static and slow-moving ripples may reflect deficits in the same underlying auditory processing mechanism. Significant factor loadings involving ripple stimuli with low-frequency content may suggest an impaired ability to use temporal fine structure information in the stimulus waveform. This description introduces a drilling sleeve, which was printed according to the individual tooth sectioning situation preoperatively, using the diagnostic cone-beam CT data. 3D printed individual drill sleeve for depth controlled sections in third molar surgery

During surgical third molar removals and coronectomy procedures, tooth sectioning is an important and, in some cases, an inferior alveolar nerve endangering step. This description introduces a drilling sleeve, which was printed according to the individual tooth sectioning situation preoperatively, using the diagnostic cone-beam CT data. The sleeve functioned in our case not only as a mark on the drill, it was a reliable physical limiter, determinant of the required depth during tooth sectioning.

Developing a valid method for assessing mandibular injury severity may permit standardized comparisons of treatment-specific outcomes between and among various mandibular fracture studies. Thrombosis of the internal jugular vein in the ENT-department — Prevalence, causes and therapy: A retrospective analysis

Less than 5% of deep vein thrombosis is due to thrombosis of the internal jugular vein. Genetic, malignant or inflammatory underlying diseases as well as insertion of venous catheters can be responsible for this pathology. Due to its rare occurrence, it is difficult to find systematic research about thrombosis of the internal jugular vein.

Sinonasal inverted papilloma associated with small cell neuroendocrine carcinoma: A case report and literature review of rare malignancies associated with inverted papilloma

We report a rare case of sinonasal inverted papilloma (IP) associated with small cell neuroendocrine carcinoma (SNEC). To our knowledge, this is the first report to describe SNEC found during the treatment of sinonasal IP. Surgery and five cycles of cisplatin plus etoposide with concurrent intensity modulated radiation therapy were performed. Neither local recurrence nor distant metastasis was noted during 6 years of post-diagnostic follow-up. The prognosis of SNEC is very poor. Treatment planning for sinonasal IP should consider a possible association with this rare but aggressive malignancy, whose treatment is completely different from that of squamous cell carcinoma, a malignancy which is commonly associated with IP.
