Evaluation of the effect of teriparatide therapy on mandibular fracture healing in rats with medication-related osteonecrosis of the jaw

Abstract

Objective

The aim of this study was to evaluate the effect of teriparatide therapy on mandibular fracture healing in rats with medication-related osteonecrosis of the jaw (MRONJ).

Materials and methods

To induce MRONJ, a total of 120 rats received intravenous zoledronate 0.06 mg/kg once a week for 6 weeks and their right mandibular first molar was extracted. Eighty of 94 rats with MRONJ were randomly selected and underwent unilateral mandibular osteotomy to replicate a fracture. After surgery, the rats were randomly assigned to T (teriparatide-treated) and C (control) groups. Group T (n = 40) received subcutaneous injection of 2 μg/kg/day teriparatide and group C (n = 40) received the same volume of normal saline until sacrifice. Four and 8 weeks after surgery, 20 rats in each group were sacrificed. Fracture healing was scored using a histological grading system (1 to 10).

Results

In group C, at 4 weeks and 8 weeks post-fracture, fibrous and cartilaginous tissues and scant bone formation at the fracture site and lacunae without osteocyte in adjacent mandibular bone were seen. In group T, substantial amounts of new trabecular bone rimmed by osteoblasts and some areas of remodeled mature bone were seen. After 8 weeks, extensive replacement of trabecular bone with mature bone occurred. Except between C4 and C8 groups, the healing score was significantly different between all subgroups.

Conclusion

Teriparatide therapy successfully improved mandibular fracture healing in rats with MRONJ. However, this study was limited by the use of an animal model whose anatomy, physiology, and drug metabolism might be different from humans.

Clinical relevance

The present study showed that teriparatide therapy may be used adjunctive to surgery in the treatment of mandibular fractures in MRONJ patients.

Abstract

Background

Metabolomics is the study of metabolome which describes the full repertoire of small molecules and the analysis of salivary metabolomics may help in identifying tumor specific biomarkers for early diagnosis and prediction of tumor progression. The aim of the study was to evaluate the clinical utility of salivary metabolites in oral leukoplakia and oral squamous cell carcinoma.

Methods

Salivary metabolomic profile of patients diagnosed with oral leukoplakia (n=21) and oral squamous cell carcinoma (n=22) were compared with apparently normal controls (n=18) using QTOF‐ liquid chromatography mass spectrometry. MassHunter profile software and Metlin database was used for metabolite identification. ANOVA analysis to identify the regulation of metabolites between the three groups; t‐test (p<0.05) to signify the changes between two groups and chi‐square test (p<0.05) to indicate the presence or absence of metabolites in the study participants of the three groups was performed.

Results

Significant upregulation of 1‐methylhistidine, inositol 1,3,4‐ triphosphate, d‐glycerate‐2‐phosphate, 4‐nitoquinolone‐1‐oxide, 2‐oxoarginine, norcocaine nitroxide, sphinganine‐1‐phosphate and pseudouridine in oral leukoplakia and OSCC was noted. Downregulated compounds in the diseased groups included L‐homocysteic acid, ubiquinone, neuraminic acid and estradiol valerate.

Conclusion

A range of salivary metabolites were significantly altered in oral leukoplakia and oral squamous cell carcinoma. Further, it is necessary to evaluate the clinical utility of the individual metabolites in preventing malignant transformation of oral leukoplakia and to improve prognosis of oral squamous cell carcinoma.

A step‐up approach starting from a 1‐2 food elimination diet and increasing the number of foods based on a personalised dietary approach is recommended if the goal is to achieve remission using the least number of endoscopies and with increased acceptability to the patient.

Children with EoE on elimination diets require frequent monitoring of growth and nutrition, as well as screening for symptoms of EoE, allergy, and mindfulness regarding psychosocial impact of chronic disease on the family and child. Food avoidance strategies in eosinophilic oesophagitis

Abstract

EoE is a chronic disease associated with significant morbidity that can result in permanent fibrosis and stricture formation. Given the complexity a multidisciplinary approach is recommended to manage these patients. In the majority of children with EoE not responsive to proton pump inhibitors (PPI), inflammation is driven by sensitivity to foods and treatment with an elimination diet can be effective.

Abstract

Objective

To prospectively evaluate Patient Reported Outcome Measures (PROMs) and treatment outcomes of mandibular two‐implant retained overdentures (IOD) in an edentulous geriatric cohort with history of deficient Complete Dentures (CD).

Materials and Methods

103 patients with deficient CD received new optimal CD. After a period of 3 months, 80 of the patients voluntarily received IOD. Outcomes collected at pre‐, post‐CD/post‐IOD treatment annually up to 5 years, included: A) Clinical outcomes: denture‐quality (Woelfel's index), complications, and maintenance events, B) PROMs: patient complaints (maxillary, mandibular functional complaint scores, generic aesthetic complaint scores, frequency, and intensity of complaints) and patient satisfaction, C) Maintenance events: technical complications and adverse events.

Results

5 years data were collected from 67 patients (mean age at initiation =71.3 years, mean observation=5.9 years) with cumulative implant survival‐rate=98.72%. Multiple comparisons for 8 time‐points showed significant improvements in denture‐quality and PROMs after new CD and IOD delivery. Further significant improvements were only after IOD delivery in: mandibular denture‐quality, mandibular and generic functional complaints, frequency, intensity of complaints, and overall patient satisfaction score. Thereafter, denture‐quality and PROMs remained stable. Maintenance events clustered on 1st year and within a minority of patients. Technical complications and overall maintenance events significantly correlated with overall patient satisfaction score at 1st year.

Conclusions

Mandibular IOD was a beneficial treatment option for seniors with history of deficient CD, improving denture‐quality, patient satisfaction, and reducing patient complaints up to 5 years. Maintenance events clustered on 1st year, showing no significant impact on long‐term patient satisfaction and other PROMs.

Stepwise approach towards adoption of allergen immunotherapy for allergic rhinitis and asthma patients in daily practice in Belgium: a BelSACI-Abeforcal-EUFOREA statement

Abstract

Allergic rhinitis (AR) affects 23–30% of the European population with equal prevalence reported in Belgium. Despite guidelines on the correct use of effective treatment, up to 40% of AR patients remain uncontrolled. Allergen immunotherapy (AIT) has been shown to improve the level of control up to 84% of patients being controlled by AIT. Recently, new guidelines for AIT have been published, supporting the clinical evidence for effectiveness of various subcutaneous and sublingual products for AIT in patients who are allergic to airborne allergens. AIT in AR patients not only reduces nasal and/or ocular symptoms but also induces tolerance and has preventive potential. Adoption of AIT into daily clinical practice in Belgium and other European countries is hampered primarily by reimbursement issues of each of the single products but also by several patient- and physician-related factors. Patients need to be better informed about the effectiveness of AIT and the different routes of administration of AIT. Physicians dealing with AR patients should inform patients on tolerance-inducing effects of AIT and are in the need of a harmonized and practical guide that supports them in selecting eligible patients for AIT, in choosing evidence-based AIT products and in following treatment protocols with proven efficacy. Therefore, a stepwise and holistic approach is needed for better adoption of AIT in the real-life setting in Belgium.

Abstract

The sliding window correlation (SWC) analysis is a straightforward and common approach for evaluating dynamic functional connectivity. Despite the fact that sliding window analyses have been long used, there are still considerable technical issues associated with the approach. A great effort has recently been dedicated to investigate the window setting effects on dynamic connectivity estimation. In this direction, tapered windows have been proposed to alleviate the effect of sudden changes associated with the edges of rectangular windows. Nevertheless, the majority of the windows exploited to estimate brain connectivity tend to suppress dynamic correlations, especially those with faster variations over time. Here, we introduced a window named modulated rectangular (mRect) to address the suppressing effect associated with the conventional windows. We provided a frequency domain analysis using simulated time series to investigate how sliding window analysis (using the regular window functions, e.g. rectangular and tapered windows) may lead to unwanted spectral modulations, and then we showed how this issue can be alleviated through the mRect window. Moreover, we created simulated dynamic network data with altering states over time using simulated fMRI time series, to examine the performance of different windows in tracking network states. We quantified the state identification rate of different window functions through the Jaccard index, and observed superior performance of the mRect window compared to the conventional window functions. Overall, the proposed window function provides an approach that improves SWC estimations, and thus the subsequent inferences and interpretations based on the connectivity network analyses.

Importance of root uptake of 14CO2 on 14C transfer to plants impacted by below-ground 14CH4 release

Abstract

14C-labelled methane (14CH4) released from deep underground radioactive waste disposal facilities can be a below-ground source of 14CO2 owing to microbial oxidation of 14CH4 to 14CO2 in soils. Environmental 14C models assume that the transfer of 14CO2 from soil to plant occurs via foliar uptake of 14CO2. Nevertheless, the importance of 14CO2 root uptake is not well understood. In the present study, below-ground transport and oxidation of 14CH4 were modeled and incorporated into an existing land-surface 14CO2 model (SOLVEG-II) to assess the relative importance of root uptake and foliar uptake on 14CO2 transfer from soil to plants. Performance of the model in calculating the below-ground dynamics of 14CH4 was validated by simulating a field experiment of 13CH4 (as a substitute for 14CH4) injection into subsoil in a wheat field in the UK. The proposed model simulation was then applied to 14C transfer in a hypothetical ecosystem impacted by continuous 14CH4 input from the water table (bottom of 1-m thick soil), which simulated continuous release of 14CH4 from a deep underground radioactive waste disposal facility. The contrast between the results obtained from the model calculation that assumed different distributions of roots (rooting depths of 11 cm, or 97 cm) and methane oxidation (characterized by e-folding depths of 5 cm, 20 cm, or 80 cm) in the soil provided insight into the relative importance of root uptake and foliar uptake pathways. In the shallowly rooted ecosystem with rooting depth of 11 cm, foliar uptake of 14CO2 was significant, accounting for 80% of the 14C accumulation (as organic 14C) in the plant (leaf compartment). By contrast, in a deeply rooted ecosystem (rooting depth of 97 cm), where the root penetrated to depths close to the water-table, more than half (63%) the 14C accumulated in the plant was transferred via the root uptake pathway. We found that 14CO2 root uptake (thus 14C accumulation in the plant) in this ecosystem depended on the distribution of methane oxidation in the soil; all 14C accumulated in the plant was transferred by the root uptake pathway when methane oxidation occurred at considerable depths (e-folding depths of 20 cm, or 80 cm) in the soil. The high level of 14CO2 root uptake was ascribed to the oxidation of added 14CH4 (i.e., production of 14CO2) in the deep part of the soil and the subsequent high level of root uptake of the deep soil-water containing 14CO2. These results indicate that 14CO2 root uptake contributes significantly to 14CO2 transfer to plants if 14CH4 oxidation occurs at great depths and roots penetrate deeply into the soil. It is recommended that current environmental 14C models must be refined to consider the importance of the root uptake pathway to ensure that dose estimates of 14CH4 release from deep underground waste disposal facilities are accurate.

Zoledronate Treatment Duration Is Linked to Bisphosphonate‐Related ONJ Prevalence in Rice Rats with Generalized Periodontitis

Abstract

Objectives

To determine the extent that zoledronate (ZOL) dose and duration is associated with bisphosphonate‐related osteonecrosis of the jaw (BRONJ) prevalence in rice rats with generalized periodontitis (PD); characterize structural and tissue‐level features of BRONJ‐like lesions in this model; and examine the specific anti‐resorptive role of ZOL in BRONJ.

Materials and Methods

Rice rats (n=228) consumed high‐sucrose‐casein diet to enhance generalized PD. Groups of rats received 0, 8, 20, 50 or 125μg/kg IV ZOL/4wks encompassing osteoporosis and oncology ZOL doses. Rats from each dose group (n=9‐16) were necropsied after 12, 18, 24, and 30wks of treatment. BRONJ‐like lesion prevalence and tissue‐level features were assessed grossly, histopathologically, and by MicroCT. ZOL bone turnover effects were assessed by femoral pQCT, serum bone‐turnover marker ELISAs, and osteoclast immunolabeling.

Results

Prevalence of BRONJ‐like lesions was significantly associated with 1) ZOL treatment duration, but plateaued at the lowest oncologic dose, and 2) there was a similar dose‐related plateau in the systemic anti‐resorptive effect of ZOL. ZOL and BRONJ‐like lesions also altered the structural and tissue‐level features of the jaw.

Conclusion

The relationship between BRONJ‐like lesion prevalence and ZOL dose and duration varies depending on the co‐ or pre‐existing oral risk factor. At clinically relevant doses of ZOL, BRONJ‐like lesions are associated with anti‐resorptive activity.

Zoledronate Treatment Duration Is Linked to Bisphosphonate‐Related ONJ Prevalence in Rice Rats with Generalized Periodontitis

Abstract

Objectives

To determine the extent that zoledronate (ZOL) dose and duration is associated with bisphosphonate‐related osteonecrosis of the jaw (BRONJ) prevalence in rice rats with generalized periodontitis (PD); characterize structural and tissue‐level features of BRONJ‐like lesions in this model; and examine the specific anti‐resorptive role of ZOL in BRONJ.

Materials and Methods

Rice rats (n=228) consumed high‐sucrose‐casein diet to enhance generalized PD. Groups of rats received 0, 8, 20, 50 or 125μg/kg IV ZOL/4wks encompassing osteoporosis and oncology ZOL doses. Rats from each dose group (n=9‐16) were necropsied after 12, 18, 24, and 30wks of treatment. BRONJ‐like lesion prevalence and tissue‐level features were assessed grossly, histopathologically, and by MicroCT. ZOL bone turnover effects were assessed by femoral pQCT, serum bone‐turnover marker ELISAs, and osteoclast immunolabeling.

Results

Prevalence of BRONJ‐like lesions was significantly associated with 1) ZOL treatment duration, but plateaued at the lowest oncologic dose, and 2) there was a similar dose‐related plateau in the systemic anti‐resorptive effect of ZOL. ZOL and BRONJ‐like lesions also altered the structural and tissue‐level features of the jaw.

Conclusion

The relationship between BRONJ‐like lesion prevalence and ZOL dose and duration varies depending on the co‐ or pre‐existing oral risk factor. At clinically relevant doses of ZOL, BRONJ‐like lesions are associated with anti‐resorptive activity.

Effect of periodontal therapy on disease activity in patients of rheumatoid arthritis with chronic periodontitis

Abstract
Background

Evidence have been proposed a positive association between severity of Periodontitis and Rheumatoid arthritis (RA) activity, individuals with advanced RA are more likely to develop periodontal problems compared to their non-RA counterparts, and vice versa. Studies have been suggested that RA manifest as a result of an inflammatory imbalance and autoimmunity. In this perspective, treatment modalities that lead to inhibition of proinflammatory mediators, may prove beneficial for reducing the severity of RA. This study examined the effects of non surgical periodontal therapy (NSPT) on disease activity of RA.

Methods

Diagnosed patients of active rheumatoid arthritis with chronic periodontitis were recruited in this study and divided in to treatment and controls groups, both groups were similar in all demographics assessed. Treatment group (n = 20) and controls group (n = 20) underwent assessment for periodontal clinical parameters (plaque index, gingival index, probing pocket depth, clinical attachment level), Rheumatologic clinical (simplified disease activity index) and biochemical parameters(C-reactive protein, Rheumatoid factor, Anti-cyclic citrullinated protein) at baseline and 8 weeks. Serum levels of biochemical parameters were measured by enzyme-linked immunosorbent assay (ELISA).

Results

The statistically significant (p < 0.001) reduction observed in mean values of PI, GI, PPD, CAL, SDAI in treatment group at 8weeks after NSPT as compare to control group. However serum level of ACCPA, CRP and RF did not show statistically significant (p > 0.05) changes from baseline to reassessment (8 weeks) in both groups.

Conclusions

The improvement in RA disease activity may occurs after non surgical periodontal therapy.

Objective

To investigate the MDCT features of primary adrenal lymphoma (PAL) and correlate the results with clinicopathological findings.

Materials and Methods

Twenty eight patients (17 men, 11 women; median age, 61.5years) with pathologically confirmed PAL were included in this study. MDCT features of the lesions were retrospectively reviewed and correlated with clinicopathological findings.

Results

All of the 28 cases were non-Hodgkin's lymphoma (NHL), in which 25 cases were diffuse large B-cell lymphoma and 3 cases T-cell lymphoma. 21 (75%) patients had bilateral tumors with maximum diameter of 3.9-17.7 cm (median, 9.18 cm), and 7 (25%) patients had unilateral tumors with maximum diameter of 4.3-14.9 cm (median, 8.6 cm). The sign of infiltrating adjacent organs and encasing adjacent vessels were appeared in 10 and 15 patients respectively, in which 9 patients had both signs. 20 (71.4%) patients had the appearance of adeniform, and 3 (10.7%) patients showed infiltrating and embedding the adrenal gland. Among all the patients, 27 (96.4%) and 26 (92.9%) patients had mild enhancement on arterial and parenchymal phase, respectively; and 12 (42.8%) were homogenous, 14 (50%) slightly inhomogeneous and 2 (7.1%) heterogeneous on both phase. In addition, adrenal insufficiency was found in 33.3% (7/21) of the patients with bilateral lesions, and none in unilateral lesions.

Conclusion

Large adrenal soft tissue mass with maintenance of adreniform shape, encasing vessels, homogenous or slightly inhomogeneous density with no calcification, and mild enhancement on both arterial and parenchymal phase could be seen as the main MDCT features of PAL. In addition, infiltrating and embedding the adrenal gland may be specific imaging characteristics of PAL.

Allergen immunotherapy (AIT) treatment for allergic rhinitis and asthma is used by 2.6 million Americans annually. Clinical and sterility testing studies identify no risk of contamination or infection from extracts prepared using recommended aseptic techniques, but regulatory concerns persist. Social media can be used to investigate rare adverse effects not captured by traditional studies.

Objective

To investigate large social media databases for suggestion of AIT skin and soft tissue infection (SSTI) risk and compare this risk to a comparator procedure with a sterile pharmaceutical.

Methods

We analyzed USA-restricted data from over 10 common text-based social media platforms including Facebook, Twitter, and Reddit between 2012-2016. We employed natural language processing (NLP) to identify posts related to AIT and, separately, Influenza vaccination (comparator procedure). NLP was followed by manual review to identify posts suggesting a possible SSTI associated with either AIT or Influenza vaccination. SSTI frequencies with 95% confidence intervals (CI) were compared.

Results

We identified 25,126 AIT posts, which were matched by social media platform to 25,126 Influenza vaccination-related posts. NLP identified 4,088 (16.3%) AIT posts that required manual review, with 6 posts (0.02%, 95%CI 0.005% to 0.043%) indicative of possible AIT-related SSTI. NLP identified 2,689 (10.7%) Influenza posts that required manual review, with 7 posts (0.03%, 95%CI 0.007% to 0.048%) indicative of possible Influenza vaccination-related SSTI.

Conclusion
