The Role of Palliative Care in Oral Cavity Carcinoma

Abstract

Purpose of Review

The purpose of this study is to review the current and future role of palliative care in relation to oral cavity carcinoma (OCC).

Recent Findings

Palliative care for patients with OCC has traditionally been reserved for patients with advanced disease, recurrent disease, or poor performance status who are unable to undergo surgical or adjuvant treatments. Patients with early- or mid-stage disease were not typically offered palliative services. However, patients at all stages face quality of life issues including disruption of speech, eating, pain, mood, self-image, social interactions, work, and daily activities. This can directly affect performance status and has been shown to increase length of hospitalizations and decrease overall survival. Recent evidence suggests that involving palliative care earlier may decrease rates of depression, decrease cost, improve communication, and increase overall survival outcomes. Furthermore, new applications and innovations in radiation therapy, immunotherapy, and chemotherapy have broadened the palliative therapies available to patients with incurable disease.

Summary

Given the impact that OCC has on quality of life during all stages of disease, a paradigm shift is occurring to involve palliative care at earlier stages. Recent evidence has shown that upstream palliative care increases quality of life and ultimately overall survival.

Zusammenfassung

Medikamenteninduzierte Osteonekrosen der Kiefer stellen ein relativ neues, multifaktoriell ausgelöstes Krankheitsbild dar. Erkrankungen mit erhöhter Knochenresorption werden heute breit mit Antiresorptiva therapiert. Aufgrund des aktiven Knochenstoffwechsels, insbesondere im Unterkiefer, kann es bei gestörter Barrierefunktion in der Mundhöhle sowie kompliziert durch systemische Begleiterkrankungen in Abhängigkeit von Dauer, Intensität und Applikation der antiresorptiven Therapie zu Osteonekrosen kommen. Wegen des besonderen Einflusses dieser Veränderungen auf die Lebensqualität und wegen des komplexen Therapiemanagements kommt der Früherkennung bzw. der Ausdehnungsbestimmung durch die Bildgebung eine besondere Bedeutung zu. Die Orthopantomographie der Kiefer stellt eine gute Basisuntersuchung, vor allem auch für die Detektion dentogener Ursachen als mögliche Differenzialdiagnose, dar. Die dreidimensionalen bildgebenden Verfahren wie die Computertomographie (CT) und insbesondere die Magnetresonanztomographie (MRT) können aber ossäre Veränderungen wesentlich sensitiver detektieren und die in höheren Erkrankungsstadien manifesten komplexen Pathologien erfassen.

Immediate implant placement following 1-year treatment with oral versus intravenous bisphosphonates: a histomorphometric canine study on peri-implant bone

Abstract

Objectives

Bisphosphonates (BPs) are anti-resorptive medications with inhibitory effects on osteoclasts which decrease bone turnover. The present study aimed to assess the early effects of BPs on peri-implant bone.

Materials and methods

Twelve female mongrel dogs were assigned to one control and two experimental groups as follows: 12 months of oral alendronate (ALN, 3.5 mg/kg/week) or intravenous (IV) pamidronate (PAM, 1 mg/kg/week) for experimental groups. Following 8 weeks after implant insertion, the specimens were sacrificed, histological evaluation was performed, and C-terminal telopeptide of collagen I (CTx) analysis was performed on serums.

Results

PAM demonstrated the greatest marginal bone level (MBL), 2.64 ± 0.48, followed by ALN and control, 2.5 ± 0.4 and 0.66 ± 0.5 respectively. The least bone-implant contact was observed in PAM group (%39 ± 0.03) while it was significantly greater in the control group (p < 0.05). PAM and ALN demonstrated < 10 CTx levels versus > 300 in controls.

Conclusion

It is assumed that long-term oral or IV BP therapy decreases marginal bone resorption and osseointegration. These changes were more accentuated in IV BP administration. CTx does not seem to be a precise predictor for bisphosphonate-related osteonecrosis of the jaw.

Clinical relevance

BPs impair peri-implant bone remodeling and this phenomena may be effective for reducing resorption at esthetic zone implants; however, decrease in osseointegration may be a concern for implant treatment in patients receiving BPs.

Incidence and cost of perioperative red blood cell transfusion for elective spine fusion in a high-volume center for spine surgery

Abstract

Background

Spine fusion is a surgical procedure characterized by a significant perioperative bleeding, which often requires red blood cell (RBC) transfusion.

Methods

The incidence and the cost of RBC transfusion were evaluated in all patients undergoing elective surgery for spine fusion in our Institution, a high-volume center for spine surgery, over a period of 3 years. The analysis specifically addressed the RBC transfusion need in all the different spine fusion procedures (atlanto-axial, cervical, dorsal, lumbar, revisions) with the different surgical approaches (anterior, posterior).

Results

During the 3 years of observation, a total of 1.882 elective spine fusions were performed. More than half of the procedures (n = 964) were posterior lumbar fusions. Overall, 5% of the patients (n = 103) required RBC transfusion. The cervical fusions were the procedures with the lowest percentage of RBC need (0–5%), while the dorsal and the lumbar ones, with the anterior approach, represented the procedures with the highest rate of transfusion (29% and 25% respectively). More than 60 % of the RBC units were employed in the instance of posterior lumbar fusion, while a variable 1–10% of the units was used in each of the other procedures. The overall transfusion cost was of 46.000 euros, with a distribution of costs that paralleled the amount of units transfused for each procedure.

Conclusions

Several surgical and patient factors may contribute to the perioperative blood loss. An accurate patient blood management, may efficiently decrease transfusion requirements and ultimately healthcare costs.

Comparison of upper airway patency in patients with mild obstructive sleep apnea during dexmedetomidine or propofol sedation: a prospective, randomized, controlled trial

Abstract

Background

In addition to propofol, dexmedetomidine is a suitable alternative for intraoperative sedation in procedures requiring regional anesthesia. To date, however, little is known about the influences of each drug on upper airway patency. Accordingly, the authors investigated differences between dexmedetomidine and propofol sedation in the occurrence of upper airway obstruction and requirements for airway intervention in patients with mild obstructive sleep apnea.

Methods

Patients with an apnea/hypopnea index of 5–14/h according to Watch-PAT 200 analysis were enrolled in this study. Spinal anesthesia was routinely performed for surgery. Intraoperative sedation was initiated using either dexmedetomidine or propofol infusion at a level of modified observer's assessment of alertness/sedation scale 3. The primary outcome was the proportion of patients exhibiting signs of upper airway obstruction. A sign of upper airway obstruction was defined as no detection of end-tidal carbon dioxide for at least 10 s despite respiratory efforts.

Results

A total of 50 patients were included in the final analysis (dexmedetomidine [n = 26]; propofol [n = 24]). During the intraoperative sedation period, there was a significantly lower proportion of patients exhibiting signs of upper airway obstruction in the dexmedetomidine group than in the propofol group (11.5% vs. 41.7%, P = 0.035). An artificial airway was inserted in 1 patients (3.8%) and 5 patient (20.8%) in the dexmedetomidine and propofol groups, respectively (P = 0.093).

Conclusion

Dexmedetomidine sedation was associated with a lower incidence of upper airway obstruction than propofol sedation in patients with mild obstructive sleep apnea.

Ultrasound-guided phrenic nerve block for intraoperative persistent hiccups: a case report

Abstract

Background

Continuous hiccups during surgery not only affect the surgical procedure, they can also cause adverse effects for the patient. Apart from active investigation of the cause of the hiccups, their timely termination is also necessary.

Case presentation

We reported a case of a 70-year-old woman with continuous intraoperative hiccups that appeared during vaginal hysterectomy under low continuous epidural anesthesia. After the ineffectiveness CO2 repeated inhalation and intravenous administration of chlorpromazine and methoxychlorpromide, we performed unilateral phrenic nerve block under ultrasound guidance. Hiccups were terminated without any related complications.

Conclusions

During intraoperative continuous hiccups, ultrasound guided phrenic nerve block may be a suitable treatment option when physical methods and drug therapy are not effective. However, given the absence of a vital risk related to hiccups, this block should imply the complete absence of any respiratory contraindication and a prolonged postoperative respiratory monitoring.
