We examined the unique role of textbase memory and situation model building ability in first (L1) and second (L2) language reading comprehension. Participants were 76 monolingual and 102 bilingual children in 4th grade. A pathfinder network approach was used to assess textbase memory and situation model building ability, on top of other well-known cognitive and linguistic predictors of reading comprehension (working memory, nonverbal reasoning, decoding, vocabulary, and grammar). The syntactic and semantic features of two-year-olds' verb vocabularies: a comparison of typically developing children and late talkers.

We examined syntactic (transitive, intransitive, ditransitive) and semantic (manner, result) features of two-year-olds' verb vocabularies, contrasting late talkers and typically developing children to look for early differences in verb vocabulary. We conducted a retrospective analysis of parent-reported expressive vocabulary from the Language Development Survey (N = 564, N(LT) = 62) (Rescorla, 1989). Verbs were coded for the presence or absence of each syntactic and semantic feature. Binomial mixed-effects regressions revealed the effect of feature on children's knowledge and whether feature interacted with group classification. Vocal production and novel word learning in the first year.

This study focuses on the relationship between vocal production and new word learning. Thirty monolingual Italian-learning infants were recorded at about 11 months, to establish the extent of their consonant production. In parallel, the infants were trained on novel word-object pairs, two consisting of early learned consonants (ELC), two consisting of late learned consonants (LLC). Word learning was assessed through Preferential Looking. The results suggest that vocal production supports word learning: Only children with higher, consistent consonant production attended more to the trained ELC images. Vocal Fold Polyps: Literature Review

Introduction Vocal fold polyps are one of the most frequent benign laryngeal lesions, impacting the quality of life of those affected by them, primarily the vocal production. Despite being a well-established therapy in conjunction with surgery, speech therapy alone may also be effective in treating these lesions. As such, otolaryngologists and speech therapists need updated bibliographic knowledge on the issue. Objective To describe the literature findings on vocal fold polyps that discuss prevalence, etiology, histology, physiopathology, vocal characteristics or treatment. Data Synthesis The present study is a review article based on a bibliographic search using platforms, databases and search engines, with no restrictions on means of publication, methodological quality or language. All the articles on vocal fold polyps pertaining to the object of study published in the past 15 years were included. Among the characteristics investigated, the most discussed were prevalence of men, smoking as an etiological cofactor, the possibility of histological differentiation from vocal nodules, the relationship with cover minor structural alterations, and the indication and effectiveness of different treatment options. Conclusion Despite the discrepancies found in the present literature review on vocal fold polyps, there has been a notable scientific progress in the otolaryngologic techniques and in the effectiveness of speech therapy as initial treatment, with direct and indirect techniques, corroborating the need for scientific investigation of the issue.

Daily outpatient intravenous antibiotic therapy for the management of pediatric periorbital cellulitis, a retrospective case series of sixty‐six patients

Abstract

Objective

To evaluate whether outpatient treatment of periorbital cellulitis with daily administration of intravenous antibiotics and physician evaluation is an effective and safe alternative to admission.

Design

A retrospective chart review study of pediatric patients treated on an outpatient basis for periorbital cellulitis at a tertiary children's hospital between 2013‐2015 was performed. Children were assessed day by a pediatrician to monitor for resolution of symptoms or complications.

Setting

The Montreal Children's hospital, a tertiary care center.

Participants

Children diagnosed with an uncomplicated periorbital cellulitis secondary to an acute sinusitis or upper respiratory tract infection.

Main outcome measures

The number of days of intravenous antibiotics, complications or need for subsequent admission. Complications were defined as formation of an abscess or phlegmon confirmed on computerized tomography scan, worsening or recurrent persistent cellulitis, failure to improve on intravenous antibiotics, and intracranial complications.

Results

Sixty‐six children with a diagnosis of uncomplicated periorbital cellulitis secondary to sinusitis who received intravenous antibiotics via medical day hospital and who fit the inclusion criteria were identified. The mean duration of intravenous antibiotic therapy was 4.1 days. All children received ceftriaxone, with one patient also receiving cefuroxime. Two of 66 patients developed complications; one patient required admission for failure to improve/subperiosteal phlegmon and later underwent functional endoscopic sinus surgery, and one patient developed an eyelid abscess that did not require admission. No patients developed severe neurological or visual deficits.

Conclusions

Outpatient intravenous therapy with daily reassessment by a physician may be a safe alternative to admission in select cases of periorbital cellulitis without systemic signs of illness.

When muscle bulk is needed under the skin paddle, it may be important to know the exact location of the inferior edge of the trapezius muscle (TM) in order to design the skin paddle accordingly. Due to the anatomic variations seen especially in the origin of trapezius muscle, there are no reliable surface landmarks to define the inferior edge of TM. Radiomics in Oncological PET/CT: a Methodological Overview

Abstract

Radiomics is a medical imaging analysis approach based on computer-vision. Metabolic radiomics in particular analyses the spatial distribution patterns of molecular metabolism on PET images. Measuring intratumoral heterogeneity via image is one of the main targets of radiomics research, and it aims to build a image-based model for better patient management. The workflow of radiomics using texture analysis follows these steps: 1) imaging (image acquisition and reconstruction); 2) preprocessing (segmentation & quantization); 3) quantification (texture matrix design & texture feature extraction); and 4) analysis (statistics and/or machine learning). The parameters or conditions at each of these steps are effect on the results. In statistical testing or modeling, problems such as multiple comparisons, dependence on other variables, and high dimensionality of small sample size data should be considered. Standardization of methodology and harmonization of image quality are one of the most important challenges with radiomics methodology. Even though there are current issues in radiomics methodology, it is expected that radiomics will be clinically useful in personalized medicine for oncology.

Clinical Personal Connectomics Using Hybrid PET/MRI

Abstract

Brain connectivity can now be studied with topological analysis using persistent homology. It overcame the arbitrariness of thresholding to make binary graphs for comparison between disease and normal control groups. Resting-state fMRI can yield personal interregional brain connectivity based on perfusion signal on MRI on individual subject bases and FDG PET produces the topography of glucose metabolism. Assuming metabolism perfusion coupling and disregarding the slight difference of representing time of metabolism (before image acquisition) and representing time of perfusion (during image acquisition), topography of brain metabolism on FDG PET and topologically analyzed brain connectivity on resting-state fMRI might be related to yield personal connectomics of individual subjects and even individual patients. The work of association of FDG PET/resting-state fMRI is yet to be warranted; however, the statistics behind the group comparison of connectivity on FDG PET or resting-state MRI was already developed. Before going further into the connectomics construction using directed weighted brain graphs of FDG PET or resting-state fMRI, I detailed in this review the plausibility of using hybrid PET/MRI to enable the interpretation of personal connectomics which can lead to the clinical use of brain connectivity in the near future.

A specific synbiotic-containing amino acid-based formula in dietary management of cow’s milk allergy: a randomized controlled trial

Abstract

Background

Here we report follow-up data from a double-blind, randomized, controlled multicenter trial, which investigated fecal microbiota changes with a new amino acid-based formula (AAF) including synbiotics in infants with non-immunoglobulin E (IgE)-mediated cow's milk allergy (CMA).

Methods

Subjects were randomized to receive test product (AAF including fructo-oligosaccharides and Bifidobacterium breve M-16V) or control product (AAF) for 8 weeks, after which infants could continue study product until 26 weeks. Fecal percentages of bifidobacteria and Eubacterium rectale/Clostridium coccoides group (ER/CC) were assessed at 0, 8, 12, and 26 weeks. Additional endpoints included stool markers of gut immune status, clinical symptoms, and safety assessments including adverse events and medication use.

Results

The trial included 35 test subjects, 36 controls, and 51 in the healthy reference group. Study product was continued by 86% and 92% of test and control subjects between week 8–12, and by 71% and 80%, respectively until week 26. At week 26 median percentages of bifidobacteria were significantly higher in test than control [47.0% vs. 11.8% (p < 0.001)], whereas percentages of ER/CC were significantly lower [(13.7% vs. 23.6% (p = 0.003)]. Safety parameters were similar between groups. Interestingly use of dermatological medication and reported ear infections were lower in test versus control, p = 0.019 and 0.011, respectively. Baseline clinical symptoms and stool markers were mild (but persistent) and low, respectively. Symptoms reduced towards lowest score in both groups.

Conclusion

Beneficial effects of this AAF including specific synbiotics on microbiota composition were observed over 26 weeks, and shown suitable for dietary management of infants with non-IgE-mediated CMA.

Surgical Management Of Mandibular Subcondylar Fractures Under Local Anaesthesia - A Proposed Protocol

The optimal treatment for adult mandibular condylar fractures (MCFs) has largely shifted in favour of open reduction and rigid internal fixation (ORIF). However a sizeable portion of these injuries are still being treated closed based on old literature, convenience, "acceptable" results, and lesser associated morbidity.Most MCFs with multiple associated facial trauma are appropriately treated under general anaesthesia(GA). Complications associated with these challenges have been well documented in the literature. We read with great interest the recently published cohort study by Hettiaratchy et al1 comparing penetrating extremity injury examination results to operative findings. The increasing rate of crime involving a knife or sharp object2 is of concern to all trauma surgeons, and a great pressure to develop safe and effective clinical pathways for penetrating trauma. We agree with the authors that more pragmatic methods may be appropriate to treat these injuries, but question the author's assertion that their data has shown selective non-operative management to be a safe alternative to surgical exploration.

A Comparison of Patient Reported Outcome Measures in Patients who received both DIEP Flap and PAP Flap Breast Reconstructions

Breast reconstruction should be routinely offered to patients undergoing mastectomy surgery for breast cancer.1 Post-operative recovery-time, the patient's occupation, hobbies and lifestyle must be considered when determining the most appropriate reconstruction technique for individual patients. We appreciate the comments by Dickson and colleagues in their letter to the editor on our published article.1 We agree with them that non-operative management is a pragmatic approach for penetrating trauma and our paper is the first documented attempt to apply this approach to a UK trauma population. It is inevitable and appropriate that conservative management of trauma is scrutinized as it is counter to most current plastic surgical doctrine, though conservative management of other types of trauma is commonplace.

Letter to the Editor regarding – Selective non-operative management for penetrating extremity trauma: A paradigm shift in management

We read with great interest the recently published cohort study by Hettiaratchy et al1 comparing penetrating extremity injury examination results to operative findings. The increasing rate of crime involving a knife or sharp object2 is of concern to all trauma surgeons, and a great pressure to develop safe and effective clinical pathways for penetrating trauma. We agree with the authors that more pragmatic methods may be appropriate to treat these injuries, but question the author's assertion that their data has shown selective non-operative management to be a safe alternative to surgical exploration.

A Comparison of Patient Reported Outcome Measures in Patients who received both DIEP Flap and PAP Flap Breast Reconstructions

Breast reconstruction should be routinely offered to patients undergoing mastectomy surgery for breast cancer.1 Post-operative recovery-time, the patient's occupation, hobbies and lifestyle must be considered when determining the most appropriate reconstruction technique for individual patients. Generally, patients prefer autologous breast reconstruction over implant-based methods for cosmetic and psychological reasons.2 The Deep Inferior Epigastric Perforator (DIEP) flap is considered the gold-standard technique in free-flap autologous breast reconstruction however the Profunda Artery Perforator (PAP) flap is increasing in popularity amongst selected patients.

Osmidrosis surgery is still recognized as one of the best ways to treat body odor,1 especially the liposuction assisted curettage due to the relatively small incision that does not leave any scars, which, nevertheless, may still cause many complications. One of the most common complications is the erosion of the skin flap, where the necrosis of the flap incision edge is about 4%.2 Therefore, physicians who perform such surgeries often observe varying degrees of manifestations, some of which are relatively common superficial erosions (Figure 1) and some of which are proportionately deep erosions.

Clinical trial testing fecal microbiota transplant for recurrent diarrheal disease begins

(NIH/National Institute of Allergy and Infectious Diseases) A research consortium recently began enrolling patients in a clinical trial examining whether fecal microbiota transplantation (FMT) by enema--putting stool from a healthy donor in the colon of a recipient--is safe and can prevent recurrent Clostridium difficile-associated disease (CDAD), a potentially life-threatening diarrheal illness. Investigators aim to enroll 162 volunteer participants 18 years or older who have had two or more episodes of CDAD within the previous six months. Flap Reconstruction after Groin and Medial Thigh Sarcoma Resection Reduces the Risk of Lower-Extremity Lymphedema

Extensive resection is an effective treatment for sarcomas in the groin and medial thigh region; however, various complications may develop postoperatively.1,2 Infection, lymphorrhea, and lymphedema frequently develop in the groin/medial thigh because of the important lymphatic pathways from the lower extremities. Prolonged lymphorrhea and lymphatic cysts are causes of chronic lower-extremity lymphedema.1 Symptoms become further exacerbated because of repeated cellulitis and movement limitations.
