Anatomy of the pterygopalatine fossa: an innovative metrical assessment based on 3D segmentation on head CT-scan

Abstract

Purpose

The pterygopalatine fossa is an important anatomical structure for several surgical and anaesthesiologic procedures; yet, very few data are available about its size. This study aims at providing a metrical assessment of pterygopalatine fossa through an innovative 3D segmentation procedure on head CT-scans.

Methods

CT-scans from 100 patients (50 males and 50 females) aged between 18 and 85 years were chosen for the study. Right and left pterygopalatine fossae were segmented through ITK-SNAP open source software. Height and volume were calculated on the acquired 3D models. In addition, anterior–posterior nasal spine distance, upper facial height (nasion–prosthion) and biorbital breadth (ectoconchion–ectoconchion) were measured as well. Statistically significant differences of height and volume according to sex and side were assessed through two-way ANOVA test: sexually dimorphic measurements were further assessed through one-way ANCOVA test using the three cranial measurements as covariates (p < 0.05).

Results

On average pterygopalatine fossa height was 24.1 ± 3.5 mm in males, and 22.8 ± 3.4 mm in females, whereas volume was 0.930 ± 0.181 cm3 in males and 0.817 ± 0.157 cm3 in females, with statistically significant differences according to sex (p < 0.05), but not to side (p > 0.05); interaction was negligible for both the measurements. ANCOVA test verified that sexual dimorphism of both measurements is independent from general cranial size (p < 0.05).

Conclusions

The present study highlighted the sexual dimorphism of pterygopalatine fossa: results may improve the knowledge of this anatomical structure difficult to explore, but crucial in several fields of clinics and surgery.

Hintergrund Eine Umfrage aus dem Jahr 2009 zeigte, dass lediglich etwa 24 % aller deutschen HNO-Kliniken die Sialendoskopie anboten. Durch die Wiederholung der Untersuchung im Jahr 2016 sollte die Entwicklung der Sialendoskopie an deutschen HNO-Kliniken reevaluiert werden. Material und Methoden Erneut wurde ein selbstentwickelter Fragebogen mit elf Fragen an alle deutschen HNO-Kliniken verschickt. Der gleiche Fragebogen wurde bereits im Jahr 2009 verwendet. Die aktuellen Umfrageergebnisse wurden mit den Ergebnissen von damals verglichen und ausgewertet. Ergebnisse Die Zahl der sialendoskopierenden HNO-Kliniken konnte sich mehr als verdoppeln. Die Anzahl der Interventionen verdreifachte sich. Die Gründe dafür, dass eine Klinik keine Sialendoskopien durchführte, waren verschieden. Ein Hauptargument war ein Mangel an Patienten. Kaum Veränderungen ergaben sich bei ambulanten und stationären Therapien und der Eingriffsdauer. Die Sonographie wird unverändert von allen Kliniken präoperativ durchgeführt. Schlussfolgerung Auffallend ist, dass bei zunehmender Anzahl an Kliniken, die die Sialendoskopie anbieten und steigenden Fallzahlen, ein Teil der Kliniken einen Patientenmangel angibt. Dies könnte für die Etablierung von spezialisierten Zentren sprechen. Inzwischen werden neben der obstruktiven Sialadenitis weitere Krankheitsbilder durch die Sialendoskopie therapiert. Die Sonographie bleibt die präoperative Diagnostik der Wahl. Characterization of normal-appearing white matter in multiple sclerosis using quantitative susceptibility mapping in conjunction with diffusion tensor imaging

Abstract

Purpose

Quantitative susceptibility mapping (QSM) is influenced by iron as well as myelin, which makes interpretation of pathologic changes challenging. Concurrent acquisition of MR sequences that are sensitive to axonal/myelin integrity, such as diffusion tensor imaging (DTI), may provide context for interpreting quantitative susceptibility (QS) signal. The purpose of our study was to investigate alterations in normal-appearing white matter (NAWM) in multiple sclerosis (MS) using QSM in conjunction with DTI.

Methods

Twenty relapsing–remitting MS patients and 20 age-matched healthy controls (HC) were recruited for this prospective study. QS, radial diffusivity (RD), fractional anisotropy (FA), and R2* maps within the whole brain as well as individual tracts were generated for comparison between NAWM and HC white matter (HCWM).

Results

MS lesions demonstrated significant differences in QS, FA, RD, and R2* compared to HCWM (p < 0.03). These metrics did not show a significant difference between whole-brain NAWM and HCWM. Among NAWM tracts, the cingulate gyri demonstrated significantly decreased QS compared to HCWM (p = 0.004). The forceps major showed significant differences in FA and RD without corresponding changes in QS (p < 0.01).

Conclusion

We found discordant changes in QSM and DTI metrics within the cingulate gyri and forceps major. This may potentially reflect the influence of paramagnetic substrates such as iron, which could be decreased along these NAWM tracts. Our results point to the potential role of QSM as a unique biomarker, although additional validation studies are needed.

Management of abnormal deep inferior epigastric vessels in microsurgical breast reconstruction: a report of 3 complex cases

Abstract

The deep inferior epigastric artery perforator flap is the gold standard in microsurgical breast reconstruction. It is now increasingly offered in patients with previous abdominal scars and in such cases, computed tomography angiography (CTA) is routinely implemented. However, previous injury of the deep inferior epigastric vessels may be difficult to detect with CTA. We report three cases of successful bilateral breast reconstruction, in which a unilateral DIEA was found to be abnormal. In the first two cases, the unilateral DIE vessels were found to be damaged and the flaps salvaged based on the deep superior epigastric artery. In the third patient, previous DIEA injury resulted in vessel recanalization, which was only evident following initial anastomotic failure. In this case, the damaged DIEA segment was subsequently removed and the flap successfully revascularized. All three patients had an uneventful post-operative recovery and all flaps survived with no complications. Based on our experience, we believe that awareness of the potential pitfalls and careful planning for lifeboats are essential to avoid flap loss in such circumstances.

Simultaneous Loss of Both Atypical Protein Kinase C Genes in the Intestinal Epithelium Drives Serrated Intestinal Cancer by Impairing Immunosurveillance

Serrated adenocarcinomas are an aggressive and treatment-resistant form of colorectal cancer. Nakanishi et  al. show that expression of atypical PKCs is reduced in human serrated tumors and that their loss results in spontaneous serrated tumorigenesis with stromal activation and immunosuppression in mice. NMDAR encephalitis presenting as akinesia in a patient with Parkinson disease

We describe the case of a woman with Parkinson disease who developed an N-methyl-d-aspartate receptor antibody-mediated encephalitis. As a novelty, the encephalitis presentation mimicked a worsening of the pre-existing extrapyramidal syndrome, manifesting mainly as severe bradykinesia and, eventually, akinesia. Management of abnormal deep inferior epigastric vessels in microsurgical breast reconstruction: a report of 3 complex cases

Abstract

The deep inferior epigastric artery perforator flap is the gold standard in microsurgical breast reconstruction. It is now increasingly offered in patients with previous abdominal scars and in such cases, computed tomography angiography (CTA) is routinely implemented. However, previous injury of the deep inferior epigastric vessels may be difficult to detect with CTA. We report three cases of successful bilateral breast reconstruction, in which a unilateral DIEA was found to be abnormal. In the first two cases, the unilateral DIE vessels were found to be damaged and the flaps salvaged based on the deep superior epigastric artery. In the third patient, previous DIEA injury resulted in vessel recanalization, which was only evident following initial anastomotic failure. In this case, the damaged DIEA segment was subsequently removed and the flap successfully revascularized. All three patients had an uneventful post-operative recovery and all flaps survived with no complications. Based on our experience, we believe that awareness of the potential pitfalls and careful planning for lifeboats are essential to avoid flap loss in such circumstances.
