Why I Went to ASHA ’ s Advocacy Day and Plan to Go Back

Do you remember those cellular commercials that bombarded us with the catchphrase, "Can you hear me now?" Guess what? Over time, it worked. A number of people switched their phone service to the company … maybe not because of the catchphrase itself, but possibly because of the totality of the message, repeated time and time again. A simple and efficient message changed minds and perspectives. And we can do the same when it comes to advocating for our professions of audiology and speech-language pathology. I'm an audiologist and not a minister, so I don't need to preach to the choir regarding these issues, but please indulge me sharing my first experience participating in ASHA's annual Capitol Hill Advocacy Day. Pretibial Lymphoplasmacytic Plaque in Children: Case Report and Review of the Literature

Abstract: An 8-year-old girl presented with a persistent 5 × 2-cm violaceous doughy plaque on the left lower leg. Histologic examination revealed hyperkeratosis, variable but mild epidermal hyperplasia, and vacuolar interface changes with melanin pigment incontinence confined to the papillary dermis. A diagnosis of pretibial lymphoplasmacytic plaque in children was made. This report outlines the clinical characteristics of this entity and a review of other cases reported in the literature. Acquired Dermal Melanocytosis Associated With Diffuse Systemic Sclerosis in a White Patient

Abstract: We report a case of a 65-year-old man who developed an asymptomatic bluish spot that affected the flank and left lumbar region with the onset 10 years prior. He had a history of diffuse systemic sclerosis with anti-Scl-70–positive antibodies. The appearance of the skin lesion coincided with the onset of his disease. The skin biopsy was consistent with the diagnosis of acquired dermal melanocytosis. The relationship between the appearance of acquired pigmented macules and spots and systemic sclerosis has been known for years, although it is an infrequent finding. Reflectance Confocal Microscopy Can Help the Dermatopathologist in the Diagnosis of Challenging Skin Lesions

Abstract: Despite the successful assignment of Current Procedural Terminology codes, there are barriers to incorporating in vivo reflectance confocal microscopy (RCM) into daily practice. Importantly, the dermatopathologist can play a key role in interpreting RCM images and can use these images to correlate with histopathology. Herein, we describe, using a case series, how RCM can be incorporated into the dermatopothalogist's practice. We also summarize the criteria for RCM diagnosis of common neoplasms. Alveolar Soft-Part Sarcoma of the Tongue

Abstract: Alveolar soft-part sarcoma is a rare neoplasm of unknown histogenesis that accounts for less than 1% of all soft-tissue sarcomas. The tumor is highly vascularized with small vascular spaces separating nests of cells, and from cytogenetic point of view, is characterized by chromosome rearrangement der(17)t(X:17)(p11:q25) that results in the ASPL-TFE3 translocation. It can occur at any age, but it is most common between 15 and 35 years of age. The prognosis is poor, despite the relatively slow growth of the tumor. We present here an atypical case of alveolar soft-part sarcoma in which the age of the patient, the location, and the histopathologic characteristics of the lesion represented a diagnostic challenge. Intraoperative factors associated with free flap failure in the head and neck region: a four-year retrospective study of 216 patients and review of the literature

The aim of this study was to identify intraoperative factors associated with free flap failure. The outcomes of 216 patients (220 flaps) were analyzed retrospectively. A statistical analysis was performed to determine the association of flap failure with the intraoperative factors of prolonged operation time, flap type, vascular pedicle at the recipient site, and use of vasoactive medication. A review of the recent literature was also conducted to identify other intraoperative risk factors. Univariate regression analysis revealed that a prolonged operative time (P=0.013) and the vascular pedicle at the recipient site (P=0.027) were primary risk factors for flap failure. However, only a few patients present with the full triad, and most exhibit a wide spectrum of phenotypic variability. The aim of this study was to phenotypically delineate Pai syndrome and to propose new criteria to facilitate a clinical diagnosis in the future. The study cohort consisted of seven case patients and an additional 60 cases diagnosed with Pai syndrome identified in a literature review. Hintergrund: In den vergangenen Jahren wurden die Indikationen zur Cochleaimplantation ausgeweitet und gleichzeitig schonendere operative Vorgehensweisen entwickelt. Methoden und Patienten: Das hier vorgestellte und von uns entwickelte Verfahren kombiniert eine verkürzte retroauriculäre Hautinzision von 25 mm Länge mit einer Reduzierung der Mastoidbohrung, der konsequenten Nutzung des Rundfensterzugangs und der Bohrung einer temporalen Implantatmulde (sog. Bremer Zugang). In der vorliegenden Arbeit werden erste Erfahrungen und Ergebnisse mit einem minimal-invasiven Vorgehen (MiCI) bei n = 31Patienten im Alter zwischen 9–79 Jahren berichtet. Ergebnisse: Es gelang die Länge des retroauriculären Hautschnitts auf im Mittel 29,7 mm, die Fläche der Mastoidbohrung auf 20mm2 und die Verweildauer um 0,55 Tage zu reduzieren. Es zeigte sich eine signifikante Lernkurve bei der Verkürzung der OP-Zeiten (p < 0.03). Die Frequenz und Art der Komplikationen entsprach der eines konventionellen Vorgehens. Schlußfolgerung: Die minimal-invasive Cochleaimplantation ist eine zuverlässige und sichere Methode zur Implantatversorgung.

OIPAV: an Integrated Software System for Ophthalmic Image Processing, Analysis, and Visualization

Abstract

Ophthalmic medical images, such as optical coherence tomography (OCT) images and color photo of fundus, provide valuable information for clinical diagnosis and treatment of ophthalmic diseases. In this paper, we introduce a software system specially oriented to ophthalmic images processing, analysis, and visualization (OIPAV) to assist users. OIPAV is a cross-platform system built on a set of powerful and widely used toolkit libraries. Based on the plugin mechanism, the system has an extensible framework. It provides rich functionalities including data I/O, image processing, interaction, ophthalmic diseases detection, data analysis, and visualization. By using OIPAV, users can easily access to the ophthalmic image data manufactured from different imaging devices, facilitate workflows of processing ophthalmic images, and improve quantitative evaluations. With a satisfying function scalability and expandability, the software is applicable for both ophthalmic researchers and clinicians.

Continuous Vocal Fry Simulated in Laboratory Subjects: A Preliminary Report on Voice Production and Listener Ratings.

Conclusions: Thirty minutes of simulated, continuous vocal fry worsened some voice measures when compared with a habitual voice quality. Samples of continuous vocal fry were rated as significantly less employable, less natural, and requiring greater listener concentration as compared with samples of habitual voice quality. Low rates of recanalization for wide-necked aneurysms treated with stenting after balloon-assisted coiling: combination of techniques delivers stable and improved results during follow-up

Abstract

Purpose

In the treatment of wide-necked aneurysms, stenting may be required after balloon-assisted coiling (BAC) for reconstructing the lumen/flow of the parent artery in cases of coil herniation. The potential benefits and complications of both techniques remain unclear when used together. Our aim was to assess the safety and long-term angiographic outcomes of stenting after BAC.

Methods

Retrospective review of 87 unruptured wide necked aneurysms (in 80 patients) treated with BAC and requiring stent placement at the final stage of the procedure due to coil prolapse. Aneurysm characteristics, technical issues, clinical records, and MR results were assessed. Post-treatment and follow-up angiograms were evaluated by two independent reviewers and categorized as "stable," "further thrombosis," or "recanalization."

Results

The mean maximum diameter was 9.1 ± 4.7 mm and aspect ratio 1.20 ± 0.47. Complete and near-complete initial occlusion were observed in 53 (60.9%) and 28 (32.2%) of all treated aneurysms, respectively. Angiographic follow-up (80 aneurysms; mean, 34.68 ± 25.26 months) revealed stability and further thrombosis in 92.4% (74/80). Recanalization was observed in six cases (7.5%, 6/83), four of which were retreated. No significantly different outcomes were found in terms of aneurysm size (p = 0.641). Transient and reversible procedure-related complications occurred in eight (9.2%), minor permanent deficits in 2.3%, and major deficits, including death, in four cases (4.6%).

Conclusion

Our results suggest that treatment of wide-necked aneuryms with stent placement after BAC yields low recanalization rates and enhanced angiographic outcomes in long-term follow-up.

Intraoperative factors associated with free flap failure in the head and neck region: a four-year retrospective study of 216 patients and review of the literature

The aim of this study was to identify intraoperative factors associated with free flap failure. The outcomes of 216 patients (220 flaps) were analyzed retrospectively. A statistical analysis was performed to determine the association of flap failure with the intraoperative factors of prolonged operation time, flap type, vascular pedicle at the recipient site, and use of vasoactive medication. A review of the recent literature was also conducted to identify other intraoperative risk factors. Univariate regression analysis revealed that a prolonged operative time (P=0.013) and the vascular pedicle at the recipient site (P=0.027) were primary risk factors for flap failure.

Pai syndrome was originally described as the association of a midline cleft lip, midline facial polyps, and lipoma of the central nervous system. However, only a few patients present with the full triad, and most exhibit a wide spectrum of phenotypic variability. The aim of this study was to phenotypically delineate Pai syndrome and to propose new criteria to facilitate a clinical diagnosis in the future. The study cohort consisted of seven case patients and an additional 60 cases diagnosed with Pai syndrome identified in a literature review.

Timing of the reticular lamina and basilar membrane vibration in living gerbil cochleae

Auditory sensory outer hair cells are thought to amplify sound-induced basilar membrane vibration through a feedback mechanism to enhance hearing sensitivity. For optimal amplification, the outer hair cell-generated force must act on the basilar membrane at an appropriate time at every cycle. However, the temporal relationship between the outer hair cell-driven reticular lamina vibration and the basilar membrane vibration remains unclear. Consensus approach for the management of severe combined immune deficiency caused by adenosine deaminase deficiency

Inherited defects in adenosine deaminase (ADA) cause a subtype of severe combined immunodeficiency (SCID), known as ADA-SCID. Most affected infants can be diagnosed while still asymptomatic by a SCID newborn screening test, allowing early initiation of therapy. We reviewed the evidence currently available and propose a consensus management strategy. In addition to the treatment of the immune deficiency of ADA-SCID, patients should be followed for specific non-infectious respiratory, neurological and biochemical complications associated with ADA deficiency.
