General versus spinal anesthesia for the elderly hip fractured patient

Purpose of review There is an urge to improve care for patients with hip fracture. Pulse pressure and perioperative stroke

Purpose of review Central pulse pressure (PP), a marker of vascular stiffness, is a novel indicator of risk for perioperative morbidity including ischemic stroke. Appreciation for the mechanism by which vascular stiffness leads to organ dysfunction along with understanding its clinical detection may lead to improved patient management. Recent findings Vascular stiffness is associated with increased mortality and neurologic, cardiac, and renal injury in nonsurgical and surgical patients. Left ventricular hypertrophy and diastolic dysfunction along with microcirculatory changes in the low vascular resistance, high blood flow, cerebral and renal vasculature are seen in patients with vascular stiffness. Pulse wave velocity and the augmentation index have higher sensitivity for detecting of vascular stiffness than peripheral PP as the hemodynamic consequences of vascular stiffness are secondary to alterations in the central vasculature. Vascular stiffness alters cerebral autoregulation, resulting in a high likelihood of having a lower limit of autoregulation more than 65 mmHg during surgery. Vascular stiffness may predispose to cerebral hypoperfusion, increasing vulnerability to ischemic stroke, postoperative delirium, and acute kidney injury. Summary Vascular stiffness leads to alterations in cerebral, cardiac, and renal hemodynamics increasing the risk of perioperative ischemic stroke and neurologic, cardiac, and renal dysfunction. High-flow nasal cannula oxygen therapy in patients undergoing thoracic surgery: current evidence and practice

Purpose of review: Patients undergoing thoracic surgery are at high risk for pulmonary and extra pulmonary complications, and may develop impairment of gas exchange during surgery and in the postoperative period. This review focuses on the potential benefits of high-flow nasal cannula (HFNC) oxygen therapy in those patients. Recent findings: HFNC oxygen therapy can be used pre, intra and postoperatively. However, evidence for the use of HFNC oxygen therapy is still limited. Most trials investigated the effects of HFNC oxygen therapy in the postoperative period only, with promising beneficial effects. Preoperative HFNC oxygen therapy might be an alternative to conventional techniques, and allows continuous oxygenation during the apneic time of laryngoscopy. In certain patients, thoracic surgery might be performed in awake and nonintubated patients who are breathing spontaneously. Under these conditions, HFNC oxygen therapy might be considered for respiratory support by experienced anesthesiologists. In the postoperative period, HFNC oxygen therapy can prevent escalation of respiratory management and has the potential to reduce the length of hospital stay. Throughout the perioperative period, close monitoring of patients receiving HFNC oxygen therapy is key, and intubation criteria to avoid delayed intubation should be defined a priori to prevent harm. Summary: HFNC oxygen therapy is a promising tool in the perioperative care of thoracic surgical patients, when properly set, performed by experienced staff and closely monitored. The Development of a Self-Efficacy Measurement Tool For Counseling in Speech-Language Pathology.

Conclusion: The adapted Counselor Activity Self-Efficacy Scales for speech-language pathologists is psychometrically sound; factor analysis, reliability, and validity were in line with reported values for the original survey tool. Potential uses for the survey tool within the field of speech-language pathology are discussed, along with implications for graduate education and clinical supervision related to counseling skills. Data‐driven adult asthma phenotypes based on clinical characteristics are associated with asthma outcomes twenty years later

Abstract

Background

Research based on cluster analyses led to the identification of particular phenotypes confirming phenotypic heterogeneity of asthma. The long‐term clinical course of asthma phenotypes defined by clustering analysis remains unknown, although it is a key aspect to underpin their clinical relevance. We aimed to estimate risk of poor asthma events between asthma clusters identified 20 years earlier.

Methods

The study relied on two cohorts of adults with asthma with 20‐year follow‐up, ECRHS (European Community Respiratory Health Survey) and EGEA (Epidemiological study on Genetics and Environment of Asthma). Regression models were used to compare asthma characteristics (current asthma, asthma exacerbations, asthma control, quality of life and FEV1) at follow‐up and the course of FEV1 between seven cluster‐based asthma phenotypes identified 20 years earlier.

Results

The analysis included 1325 adults with ever asthma. For each asthma characteristic assessed at follow‐up, the risk for adverse outcomes differed significantly between the seven asthma clusters identified at baseline. As compared with the mildest asthma phenotype, ORs (95%CI) for asthma exacerbations varied from 0.9 (0.4 to 2.0) to 4.0 (2.0 to 7.8) and the regression estimates (95%CI) for FEV1% predicted varied from 0.6 (‐3.5 to 4.6) to ‐9.9 (‐14.2 to ‐5.5) between clusters. Change of FEV1 over time did not differ significantly across clusters.

Conclusion

Our findings show that the long‐term risk for poor asthma outcomes differed between comprehensive adult asthma phenotypes identified 20‐years earlier, and suggest a strong tracking of asthma activity and impaired lung function over time.

Abstract

Body packing is the internal concealment of illicit substances within the alimentary tract with the intention of smuggling them across borders and evading law enforcement. Medical imaging plays an important role in the management of suspected body packers through the identification of drug packets, as well as recognising medical complications. This literature review aims to determine whether abdominal radiography (AXR) or computed tomography [CT] is the most appropriate imaging modality for the detection of illegal substances in suspected body packers, taking into consideration the sensitivity, specificity and radiation dose. In addition, whether CT could completely replace the use of radiography given the radiation dose of CT can be decreased to levels that can be comparable to x-ray. AXR is a widely available and relatively low-dose modality commonly performed as the first line of imaging for suspected body packers but is very unreliable due to its highly variable sensitivity of 40–90% and high false positive and false negative rates. CT has a reported sensitivity and specificity of close to 100% but is not used as an initial imaging modality due to its higher radiation dose. CT is not only superior in sensitivity and specificity, but radiologists are also able to accurately identify the number of packets and their locations within the body while delivering radiation doses similar or even below the average dose of an AXR. The use of a low-dose CT protocol with reduced mAs is now preferred as the initial screening tool for suspected body packers over AXR.

Abstract

The high cost of commercial components and parts for post-mortem computed tomography angiography (PMCTA) has resulted in the development of a very low cost PMCTA-kit. It contains ubiquitous parts from hardware stores, and rapid prototyping derived models for 3D-printing. Design specifications have been tested and improved after finite-element modeling. A case study performed in using the final PMCTA kit parts for a PMCTA shows perfect vascular contrast without relevant leaks.

This literature review aims to determine whether abdominal radiography (AXR) or computed tomography [CT] is the most appropriate imaging modality for the detection of illegal substances in suspected body packers, taking into consideration the sensitivity, specificity and radiation dose. In addition, whether CT could completely replace the use of radiography given the radiation dose of CT can be decreased to levels that can be comparable to x-ray. AXR is a widely available and relatively low-dose modality commonly performed as the first line of imaging for suspected body packers but is very unreliable due to its highly variable sensitivity of 40–90% and high false positive and false negative rates. CT has a reported sensitivity and specificity of close to 100% but is not used as an initial imaging modality due to its higher radiation dose. CT is not only superior in sensitivity and specificity, but radiologists are also able to accurately identify the number of packets and their locations within the body while delivering radiation doses similar or even below the average dose of an AXR. The use of a low-dose CT protocol with reduced mAs is now preferred as the initial screening tool for suspected body packers over AXR.

The high cost of commercial components and parts for post-mortem computed tomography angiography (PMCTA) has resulted in the development of a very low cost PMCTA-kit. It contains ubiquitous parts from hardware stores, and rapid prototyping derived models for 3D-printing. Design specifications have been tested and improved after finite-element modeling. A case study performed in using the final PMCTA kit parts for a PMCTA shows perfect vascular contrast without relevant leaks.

Parent-Implemented Communication Treatment for Infants and Toddlers With Hearing Loss: A Randomized Pilot Trial.

Conclusions: This study provides modest preliminary support for the short-term effects of a parent-implemented communication treatment for children with hearing loss. Parents learned communication support strategies that subsequently impacted child prelinguistic skills. Although these results appear promising, the sample size is very small. Peri-operative endothelial dysfunction in patients undergoing minor abdominal surgery: An observational study

BACKGROUND Myocardial injury after noncardiac surgery is common and associated with major adverse cardiac events. Surgery induces acute endothelial dysfunction, which might be central in the pathophysiology of myocardial injury; however, the relationship between surgical stress and endothelial function remains incompletely understood. OBJECTIVES This study aimed to assess the acute peri-operative changes in endothelial function after minor elective abdominal surgery. DESIGN A prospective, observational, single-centre study. SETTING A university hospital from February 2016 to January 2017. PATIENTS Sixty patients undergoing elective minor abdominal surgery. MAIN OUTCOME MEASURES The change in endothelial function, expressed as the reactive hyperaemia index (RHI), was assessed by non-invasive digital pulse tonometry. RHI, biomarkers of nitric oxide bioavailability and oxidative stress were assessed prior to and 4 h after surgery. RESULTS RHI decreased significantly from 1.93 [95% confidence interval (95% CI 1.78 to 2.09)] before surgery to 1.76 (95% CI 1.64 to 1.90), P = 0.03, after surgery. The nitric oxide production, L-arginine/asymmetric dimethylarginine, decreased significantly from a ratio of 213.39 (95% CI 188.76 to 241.2) to a ratio of 193.3 (95% CI 171.82 to 217.54), P = 0.03. Abstract

Background

We sought to investigate the prognostic value of volumetric positron emission tomography (PET) parameters in patients with human papillomavirus (HPV)‐related oropharyngeal squamous cell carcinoma (OPSCC) and a ≤10 pack‐year smoking history treated with chemoradiation.

Methods

A total of 142 patients were included. Maximum standardized uptake value, metabolic tumor volume, and total lesion glycolysis (TLG) of the primary tumor, involved regional lymph nodes, and total lesion were calculated. Cox proportional hazard modeling was used to evaluate associations of clinical and PET parameters with locoregional failure‐free survival (LRFFS), distant metastasis‐free survival (DMFS), and overall survival (OS).

Results

On univariate analysis, volumetric PET parameters were significantly associated with all endpoints, and 8th edition American Joint Committee on Cancer/Union Internationale Contre le Cancer staging was significantly associated with DMFS and OS. On multivariate analysis, total lesion TLG was significantly associated with LRFFS, while staging was most significantly prognostic for DMFS and OS.

Conclusion

Volumetric PET parameters are uniquely prognostic of LRFFS in low‐risk HPV‐related OPSCC and may be useful for directing de‐intensification strategies.

Rare anatomic variation of the hepatic arterial blood supply: case report and literature review

Abstract

Purpose

Our aim is to present a rare case of anatomic variation of the arterial blood supply to the liver because preoperative knowledge of hepatic vascular variations is mandatory in hepatic surgery and liver transplantation.

Methods

We present a case of unusual arterial blood supply to the liver, a right hepatic artery coming from the splenic artery, associated to a classical common hepatic artery and a left hepatic artery from the left gastric artery. Preoperative diagnosis was made using CT-scan and 3D reconstruction.

Results

The right hepatic artery was found behind the portal vein and its diameter showed its importance in the vascularisation of the liver. To our knowledge this type of variation has only twice been described before. The accuracy of the 3D reconstruction allowed us to adopt the best surgical strategy to avoid lesions of the two accessory arteries which proved important sources of blood supply.

Conclusions

Precise preoperative evaluation of liver blood supply has great importance on surgical, transplantation strategy and outcome and rare anatomic variations have to be known to avoid lesions of potentially important arteries. New techniques of 3D reconstruction can ease the preoperative recognition of such difficult anatomic variations.

Motor endplate‐expressing cartilage‐muscle implants for reconstruction of a denervated hemilarynx

Objective

Tissue engineering of the larynx requires a complex, multiple tissue layer design. Additionally, spontaneous reinnervation of the larynx after recurrent laryngeal nerve (RLN) injury is often disorganized, resulting in subpar function. This study investigates use of tissue‐engineered cartilage and motor endplate‐expressing (MEE) tissue‐engineered skeletal muscle implants for laryngeal reconstruction and the promotion of organized reinnervation after RLN injury.

Methods

F344 rat primary muscle progenitor cells (MPCs) were isolated. Three‐dimensional muscle constructs were created by encapsulating MPCs in type I oligomeric collagen under passive tension. Constructs were then cultured in differentiation medium (MPC control constructs) or induced to form motor endplates (MEE constructs) with neurotrophic agents. Three‐dimensional cartilage constructs were created with adipose stem cells differentiated in chondrocyte medium. The muscle and cartilage constructs were implanted into surgically created myochondral defects in the F344 rat larynx with injured or intact (control) RLN. At 1‐, 3‐, and 6‐month timepoints, videolaryngoscopy, electromyography (EMG), histology, and immunohistochemistry were used to assess outcomes.

Results

At all timepoints, cartilage‐muscle implants were well integrated into host tissue. Functionally, there was increased vocal fold adduction and EMG activity in nerve‐injured rats treated with the MEE constructs when compared to those treated with the MPC control constructs. Motor endplate‐expressing constructs had increased myofiber cross‐sectional area compared to MPC control constructs.

Conclusion

Although our laboratory previously demonstrated that muscle and cartilage constructs could be used separately for hemilaryngeal reconstruction, this study suggests combining them with the modification of MEEs rather than MPCs, resulting in improved muscle recovery after recurrent laryngeal nerve injury.

Real‐Time Light‐Guided Vocal Fold Injection: Ex Vivo Feasibility Study in a Canine Model

Objective

The transcricothyroid (CT) membrane approach is a good option for office‐based vocal fold injection (VFI). However, because the needle tip is invisible during injection using the CT approach, precise localization requires a high level of experience, and mastering this approach involves a steep learning curve. To overcome current limitations, we conceptualized a novel technique: real‐time light‐guided VFI (RL‐VFI), which enables simultaneous VFI under direct visualization of the lighted needle tip. Herein, we aimed to verify the feasibility of RL‐VFI in cadaveric canine model, simulating the setting of office‐based VFI, as well as to explore its clinical usefulness.

Study Design

Animal study.

Methods

A customized prototype device was developed. It consisted of three parts: light source, controller, and injector. Light source comprised laser diodes of two wavelengths (635 nanometers [nm], red; 532 nm, green). Four types of injector were developed using 40‐mm needles of 23‐ and 25‐gauge and optic fibers of 50 and 100 μm. ex vivo canine larynx was prepared for the experiment. Flexible laryngoscopy system was used to examine canine vocal folds.

Results

Various routes from three insertion points (3 mm, 10 mm, and 17 mm from the midline) were validated using the device. Regardless of the injection routes, the location of the needle tip was accurately indicated by light. RL‐VFI was feasible under light guidance without difficulties. Moreover, precise and simultaneous re‐injection could be performed at the intended point using the device.

Conclusion

We introduced RL‐VFI using our customized prototype device in an ex vivo canine larynx, simulating the setting of office‐based VFI. Clinical application of RL‐VFI will improve safety and precision of CT approach, as well as expand its applications in laryngology.

Multicenter prospective study on the use and outcome of rehabilitation after total laryngectomy in Germany

Abstract

Background

The purpose of this study was to assess the use and outcome of rehabilitation after total laryngectomy in Germany.

Methods

We enrolled patients who were scheduled for total laryngectomy, approached them again after surgery, after 3 months and 1 year. Patients completed questionnaires and were interviewed.

Results

Of 309 participants, 14% had not received any rehabilitation. Reasons for nonuse were primarily ongoing treatment and poor health. Users of rehabilitation had 4 times the odds of attaining any ability to speak compared to nonusers (odds ratio 3.8, P = .02). The main aim of rehabilitation from the perspective of the users was speech rehabilitation, mentioned by 71% before starting rehabilitation. This was also what most users (27%) found the most helpful part of rehabilitation.

Conclusions

Patients are interested in attending rehabilitation. They especially want to improve their speech capacity, which indeed is better among users of rehabilitation than in those without.

This was also what most users (27%) found the most helpful part of rehabilitation.

Conclusions

Patients are interested in attending rehabilitation. They especially want to improve their speech capacity, which indeed is better among users of rehabilitation than in those without.

3D printing anatomical models of head bones

Abstract

Purpose

In many medical schools, the study of Anatomy is becoming increasingly theoretical owing to the difficulty of having human body parts available, rather than offering the students the possibility of a more realistic and practical approach. We developed a project where we use a 3D printer to produce models of the human skull bones, with high quality and quantity to satisfy the needs for Anatomy classes and to be available for request to study at home.

Methods

We selected regular and well-shaped bones of the head upon which we based the 3D models. These bones were scanned using a 64-channel Computed Tomography (high-resolution volumetric acquisition) and the resulting images were then processed with a segmentation software to isolate and reconstruct the structures of interest. The final digital three-dimensional objects were converted into a printable file that the 3D printer could read. We used two filament extrusion type 3D printers, the Prusa i3 and the Zortrax M200.

Results

We have printed successfully several models of the skull bones, such as the temporal, occipital, and sphenoid. All the models have obtained good anatomical detail, thus demonstrating the practicality of this technology. Key aspects of the CT image post-processing are discussed. The production process is cost-effective and technically accessible.

Conclusions

These results confirm the potential of 3D printing to create more complex models (e.g. regional, vascular, nervous system structures) that would allow a similar experience compared with a dissection.
