Revisiting the dose constraints for head and neck OARs in the current era of IMRT

Publication date: November 2018

Source: Oral Oncology, Volume 86

Author(s): N.Patrik Brodin, Wolfgang A. Tomé

Abstract

Head and neck cancer poses a particular challenge in radiation therapy, whilst being an effective treatment modality it requires very high doses of radiation to provide effective therapy. This is further complicated by the fact that the head and neck region contains a large number of radiosensitive tissues, often resulting in patients experiencing debilitating normal tissue complications. In the era of intensity-modulated radiation therapy (IMRT) treatments can be delivered using non-uniform dose distributions selectively aimed at reducing the dose to critical organs-at-risk while still adequately covering the tumor target. Dose-volume constraints for the different risk organs play a vital role in one's ability to devise the best IMRT treatment plan for a head and neck cancer patient. To this end, it is pivotal to have access to the latest and most relevant dose constraints available and as such the goal of this review is to provide a summary of suggested dose-volume constraints for head and neck cancer RT that have been published after the QUANTEC reports were made available in early 2010.

Evaluation of Aberrant Metabolism Related Proteins in Oral Submucous Fibrosis: A Pilot Study

Publication date: Available online 8 September 2018

Source: Journal of Oral Biosciences

Author(s): Vertika Rai, Surajit Bose, Rashmi Mukherjee, Arpita Sarbajna, Chandan Chakraborty

Abstract
Objectives

The mortality rate within five years of diagnosis is very high (> 60%) for patients with oral cancer (OC), primarily due to late stage detection. Oral submucous fibrosis (OSF) is a heterogeneous oral lesion that has a high probability of developing into OC. One of the hallmark characteristics of malignant cells is alteration of metabolic pathways as the disease progresses towards carcinogenesis. This study aims to examine the altered metabolism-related proteins using immunohistochemistry (IHC).

Methods

The expression levels of glucose transporter 1 (GLUT1), hexokinase 2 (HK2), and fatty acid synthase (FASN) were analyzed in normal oral mucosa (NOM) (n = 30) and oral sub mucous fibrosis (OSF) (n = 30) using IHC. The degree of intensity and distribution of immunostaining was semi-quantitatively assessed.

Results

The expression levels of GLUT-1, HK 2, and FASN were higher in the OSF group (p < 0.001) than in NOM control group.

Conclusions

These results indicate evidence of increased levels and alteration in the expression of metabolism-related proteins. Overexpression of these proteins may play an oncogenic role in the proliferative processes and disease progression of OSF. These results serve as a fundamental base for employing these proteins in future clinical applications.

Anti-IL5 treatments in severe asthma by blood eosinophil thresholds: indirect treatment comparison

Publication date: Available online 8 September 2018

Source: Journal of Allergy and Clinical Immunology

Author(s): William Busse, Geoffrey Chupp, Hiroyuki Nagase, Frank C. Albers, Scott Doyle, Qin Shen, Daniel J. Bratton, Necdet B. Gunsoy

Abstract
Background

Three anti-interleukin (IL)5 pathway-directed therapies are approved for use in severe eosinophilic asthma (SEA); however, no head-to-head comparison data are available.

Objective

To compare the efficacy of licensed doses of mepolizumab, benralizumab and reslizumab in patients with SEA, according to baseline blood eosinophil count.

Methods

This indirect treatment comparison (ITC) used data from a Cochrane review and independent searches. Eligible studies were randomized, controlled trials in patients aged ≥12 years with SEA. Endpoints included annualized rate of clinically significant exacerbations and change from baseline in Asthma Control Questionnaire (ACQ) score and forced expiratory volume in 1 second. An ITC was performed in patients with ACQ ≥1.5 and stratified by baseline blood eosinophil counts.

Results

Eleven studies were included. All treatments significantly reduced the rate of clinically significant exacerbations and improved asthma control versus placebo in all blood eosinophil subgroups. Mepolizumab reduced clinically significant exacerbations by 34%–45% versus benralizumab across subgroups (rate ratio[RR] [95%CI]: ≥400 cells/μL: 0.55[0.35,0.87]; ≥300 cells/μL: 0.61[0.37,0.99]; ≥150 cells/μL: 0.66[0.49,0.89]; all p<0.05) and by 45% versus reslizumab in the ≥400 cells/μL subgroup (RR[95%CI]: 0.55[0.36,0.85], p=0.007). Asthma control was significantly improved with mepolizumab versus benralizumab (all subgroups: p<0.05), and versus reslizumab in the ≥400 cells/μL subgroup (p=0.004). Benralizumab significantly improved lung function versus reslizumab in the ≥400 cells/μL subgroup (p=0.025).

Conclusions

This ITC of the licensed doses suggests that, in patients with similar blood eosinophil counts, mepolizumab was associated with significantly greater improvements in clinically significant exacerbations and asthma control compared with reslizumab or benralizumab.

Angular resolution enhancement technique for diffusion-weighted imaging (DWI) using predicted diffusion gradient directions

Publication date: December 2018

Source: NeuroImage, Volume 183

Author(s): Mun Bae Lee, Yeon Hyang Kim, Geon-Ho Jahng, Oh-In Kwon

Abstract

Anisotropic diffusion MRI techniques using single-shell or multi-shell acquisitions have been proposed as a means to overcome some limitations imposed by diffusion tensor imaging (DTI), especially in complex models of fibre orientation distribution in voxels. A long acquisition time for the angular resolution of diffusion MRI is a major obstacle to practical clinical implementations. In this paper, we propose a novel method to improve angular resolution of diffusion MRI acquisition using given diffusion gradient (DG) directions. First, we define a local diffusion pattern map of diffusion MR signals on a single shell in given DG directions. Using the local diffusion pattern map, we design a prediction scheme to determine the best DG direction to be synthesized within a nearest neighborhood DG directions group. Second, the local diffusion pattern map and the spherical distance on the shell are combined to determine a synthesized diffusion signal in the new DG direction. Using the synthesized and measured diffusion signals on a single sphere, we estimate a spin orientation distribution function (SDF) with human brain data. Although the proposed method is applied to SDF, a basic idea is to increase the angular resolution using the measured diffusion signals in various DG directions. The method can be applicable to different acquired multi-shell data or diffusion spectroscopic imaging (DSI) data. We validate the proposed method by comparing the recovered SDFs using the angular resolution enhanced diffusion signals with the recovered SDF using the measured diffusion data. The developed method provides an enhanced SDF resolution and improved multiple fiber structure by incorporating synthesized signals. The proposed method was also applied neurite orientation dispersion and density imaging (NODDI) using multi-shell acquisitions.

Corpus callosum microstructure is associated with motor function in preschool children

Publication date: December 2018

Source: NeuroImage, Volume 183

Author(s): Melody N. Grohs, Jess E. Reynolds, Deborah Dewey, Catherine Lebel

Abstract

The preschool period is a time of significant physical and behavioral growth, including the improvement of gross and fine motor skills. Although motor development has been comprehensively mapped from infancy to adulthood, the neural correlates associated with motor advancements during early childhood remain unclear. The current study used diffusion tensor imaging (DTI) to delineate key motor pathways and characterize their relationships with motor performance in 80 typically developing preschool children, aged 3–6 years. The Movement Assessment Battery for Children-2nd edition (MABC-II) was used to assess motor functioning. Partial correlations between DTI parameters and motor performance, controlling for sex and age, revealed a positive correlation between motor performance and fractional anisotropy of corpus callosum motor fibers, as well as negative correlations of motor performance with mean and radial diffusivity. These results appear to be driven by females, as correlations were significant in girls but not boys when analyzed separately. Mean corticospinal tract (CST) diffusion parameters were not significantly related to motor performance, but relationships were observed at regionally specific locations along the bilateral CST. These findings suggest preschool-aged children with better motor performance show more mature white matter patterns within motor pathways, and that the structural variation in these pathways may partially account for the natural variability in motor performance.
