Abstract

More than 25% of seizures in children are refractory to anti-seizure medications and need other modalities to control. Recently, herbal agents with anticonvulsive effects on seizure models in animals have received significant attention, but studies that show their impact on seizures in children are scarce. In a randomized, double-blind placebo-controlled cross-over design, we tried to demonstrate the efficacy of a mixture of Nigella sativa and Thymus vulgaris extracts (Epistop) on seizures in a group of children with refractory epilepsies. Twenty-two children with refractory epilepsies were randomly assigned into two sequences to receive Epistop or placebo in a cross-over design. Neither Epistop nor placebo had effects on the seizure frequency and duration over the study period. Only four children experienced clinically substantial seizure frequency (more than 50%) reduction after Epistop administration. These results indicate that a mixture of Nigella sativa and Thymus vulgaris extracts does not affect the seizure frequency and duration in children with refractory seizures. However, a minority of highly selected children with refractory seizures might benefit from the effects of a mixture of these agents' extracts.

Hypofractionated radiation therapy in the management of locally advanced NSCLC: a narrative review of the literature on behalf of the Italian Association of Radiation Oncology (AIRO)—Lung Working Group

Abstract

A systematic literature was performed to assess the benefit in terms of effectiveness and feasibility of hypofractionated radiotherapy (HypoRT), with or without chemotherapy (CT), in the treatment of locally advanced non-small cell lung cancer (NSCLC). We have identified all studies, published from 2007 onwards, on patients with locally advanced NSCLC treated with HypoRT with radical intent, with a minimal dose per fraction of 2.4 Gy, with or without concurrent chemotherapy. Twenty-nine studies were identified, for a total of 2614 patients. Patients were divided in the concurrent chemo-radiation therapy group (CT-RT) and radiotherapy alone (RT). In RT group, the delivered dose ranged from 45 to 85.5 Gy, with a dose/fraction from 2.4 to 4 Gy. Actuarial 2-year PFS ranged from 13 to 57.8%, and 1, 2- and 3-year overall survival (OS) ranged from 51.3 to 95%, from 22 to 68.7%, and from 7 to 32%, respectively. Acute Grade ≥ 3 esophagitis occurred in 0–15%, while late esophageal toxicity was 0–16%. Acute pneumonitis occured in 0–44%, whereas late pneumonitis occured in 0–47%, most commonly grade ≤ G3. In CT-RT group, the delivered dose ranged from 52.5 to 75 Gy, with a dose/fraction ranging from 2.4 to 3.5 Gy. Actuarial 2-year PFS ranged from 19 to 57.8%, and OS at 1, 2 and 3 years ranged from 28 to 95%, 38.6 to 68.7%, and 31 to 44%, respectively. Acute Grade 2 and 3 esophagitis occurred in 3–41.7%, while late esophageal toxicity occurred in 0–8.3%. Acute pneumonitis ranged from 0 to 23%, whereas late pneumonitis occured 0–47%. HypoRT seems to be safe in patients with locally advanced NSCLC. The encouraging survival results of several studies analyzed suggest that hypofractionated radiation schemes should be further investigated in the future.

A Systematic Review of the Role of Penile Rehabilitation in Prostate Cancer Patients Receiving Radiotherapy and Androgen Deprivation Therapy

Abstract
Introduction/Background

Treatment-induced erectile dysfunction (ED) is a common side effect of radiotherapy and androgen deprivation therapy (ADT) that impacts on patient quality of life. Penile rehabilitation interventions including pharmacologic and physical therapies aim to reduce the impact of ED. Despite The National Institute for Health and Care Excellence guidelines recommending access to ED services, penile rehabilitation is not widely discussed or implemented. This systematic review aimed to appraise the evidence base for penile rehabilitation and identify evidence-based recommendations for practice.

Methods

A systematic review of the evidence base was undertaken using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. The Scopus and Medline (EBSCO) databases were searched for articles relevant to penile rehabilitation interventions for prostate radiotherapy patients. Study quality was graded using the Oxford Levels of Evidence and the Scottish Intercollegiate Guidelines Network.

Results

This study identified 19 articles on penile rehabilitation in prostate radiotherapy patients, consisting of eight randomized controlled trials, three systematic reviews, and eight case studies. Findings confirmed the value of early use of phosphodiesterase type 5 inhibitors.

Discussion

Despite the range of available physical and pharmaceutical interventions, relevant research focussed solely on the use of phosphodiesterase type 5 inhibitors. Themes from the reviewed articles indicated that timing of intervention was important with early on-going dosage most useful. There was also an identified need for ongoing prophylactic use during ADT. Evidence, in general, was of low quality and based on quantitative data only.

Conclusion

Additional research into the wider range of penile rehabilitation interventions is urgently required to ensure patients have access to those therapies that are most appropriate for them. A paradigm shift toward qualitative research in the field of penile rehabilitation for prostate cancer patients treated with radiotherapy and ADT may be of value in future studies.

Functional characterization of an ER-stress responding Crustin gene in Litopenaeus vannamei

In this study, we cloned a new Crustin gene (LvCruU) from Litopenaeus vannamei. We showed that LvCruU has antibacterial activity, and reducing its expression would increase the cumulative mortality of L. vannamei upon the Vibrio parahemolyticus infection. In addition, we found that promoter activity of LvCruU was enhanced not only by the deformed epidermal autoregulatory factor-1 (Deaf1), but also by activating transcription factor 3 (LvATF3) of shrimp UPR. Real-time RT-PCR showed that LvCruU and LvATF3 both were induced upon UPR activation. And moreover, in Thapsigargin plus dsLvCruU injection test, we showed that down-regulation of LvCruU increased the cumulative mortality of V. parahemolyticus-infected shrimp under ER-stress. Évaluation et impact du respect du référentiel ESMO sur la prise en charge des lésions des tissus mous, étude menée au centre de référence de Lorraine

Résumé
Introduction

Malgré les recommandations régulières émises par l'European Society for Medical Oncology (ESMO), la prise en charge des lésions des tissus mous est encore trop souvent insuffisante, pouvant aggraver le pronostic fonctionnel mais aussi vital du patient.

Méthode

Une étude a été menée au centre de référence de Lorraine. Tous Les patients inscrits dans la base de données NetSarc, au décours d'un passage en RCP, entre le 1er janvier 2010 et le 1er septembre 2016 ont été inclus. Les critères de conformité de prise en charge ont été établis en utilisant le dernier référentiel ESMO 2014, permettant de distinguer deux groupes, « conforme » et « non conforme » aux recommandations. Une première analyse comparative a été faite entre les deux groupes toutes lésions confondues. Puis une seconde uniquement sur les sarcomes.

Résultats

Au total, 445 patients présentant une lésion des tissus mous ont été inclus, 344 cas ont été traités selon les recommandations de l'ESMO, soit un taux de conformité de 77,3 %% (IC95 % : 73,4 %, 81,2 %). La conformité était meilleure pour le centre de référence que pour les structures périphériques (p < 0,001), avec des taux de conformité respectivement de 88,7 % et 51,6 %. Sur les 247 sarcomes, nous retrouvions un taux de résection R0 meilleur en cas de dossiers conformes 55 % contre 18 % (p < 0,001). Les taux de R1 étaient de 34 % contre 56 % et R2 de 11 contre 26 %. La survie n'était pas liée de manière significative à l'observance des recommandations dans l'analyse univariée. Après ajustement sur les facteurs pronostiques potentiels, en analyse multivariée, les résultats étaient similaires.

Conclusion

Le respect des recommandations de l'ESMO, permet d'améliorer la qualité de l'exérèse chirurgicale des sarcomes et évite les gestes chirurgicaux inappropriés.

Introduction

Despite regular recommendations issued by the European Society for Medical Oncology (ESMO), patients faced still too often inadequate care with a direct influence on prognosis.

Methods

A retrospective study was carried out at the Competence center in Lorraine Area. Patients registered in the NetSarc database between 1st, 2010 and September 1st, 2016 were included. Compliance criteria were established using the latest ESMO 2014 referential. Two groups "conforming" and "non conforming" were analyzed. A first analyze about all of the soft tissue lesion and a second only about sarcomas.

Results

In total, 445 patients were eligible, 344 cases were treated according to the ESMO guidelines, giving a 77.3% conformity rate (95% CI: 73.4%, 81.2%). Compliance was better for the competence center than district hospitals (P < 0.001), with compliance rates of 88.7%, and 51.6%, respectively. Among the 247 sarcomas, we found a R0 resection rate better according to the ESMO guideline, 55% against 18% (P < 0.001). R1 rates were 34% vs. 56% and R2 11 vs. 26% disease free survival was not related significantly to the observance of recommendations in the univariate analysis. After adjustment on potential DFS prognostic factors, in the multivariate analysis, the results were similar.

Functional characterization of an ER-stress responding Crustin gene in Litopenaeus vannamei

In this study, we cloned a new Crustin gene (LvCruU) from Litopenaeus vannamei. We showed that LvCruU has antibacterial activity, and reducing its expression would increase the cumulative mortality of L. vannamei upon the Vibrio parahemolyticus infection. In addition, we found that promoter activity of LvCruU was enhanced not only by the deformed epidermal autoregulatory factor-1 (Deaf1), but also by activating transcription factor 3 (LvATF3) of shrimp UPR. Real-time RT-PCR showed that LvCruU and LvATF3 both were induced upon UPR activation. And moreover, in Thapsigargin plus dsLvCruU injection test, we showed that down-regulation of LvCruU increased the cumulative mortality of V. parahemolyticus-infected shrimp under ER-stress. Résumé
Introduction

Malgré les recommandations régulières émises par l'European Society for Medical Oncology (ESMO), la prise en charge des lésions des tissus mous est encore trop souvent insuffisante, pouvant aggraver le pronostic fonctionnel mais aussi vital du patient.

Méthode

Une étude a été menée au centre de référence de Lorraine. Tous Les patients inscrits dans la base de données NetSarc, au décours d'un passage en RCP, entre le 1er janvier 2010 et le 1er septembre 2016 ont été inclus. Les critères de conformité de prise en charge ont été établis en utilisant le dernier référentiel ESMO 2014, permettant de distinguer deux groupes, « conforme » et « non conforme » aux recommandations. Une première analyse comparative a été faite entre les deux groupes toutes lésions confondues. Puis une seconde uniquement sur les sarcomes.

Résultats

Au total, 445 patients présentant une lésion des tissus mous ont été inclus, 344 cas ont été traités selon les recommandations de l'ESMO, soit un taux de conformité de 77,3 %% (IC95 % : 73,4 %, 81,2 %). La conformité était meilleure pour le centre de référence que pour les structures périphériques (p < 0,001), avec des taux de conformité respectivement de 88,7 % et 51,6 %. Sur les 247 sarcomes, nous retrouvions un taux de résection R0 meilleur en cas de dossiers conformes 55 % contre 18 % (p < 0,001). Les taux de R1 étaient de 34 % contre 56 % et R2 de 11 contre 26 %. La survie n'était pas liée de manière significative à l'observance des recommandations dans l'analyse univariée. Après ajustement sur les facteurs pronostiques potentiels, en analyse multivariée, les résultats étaient similaires.

Conclusion

Le respect des recommandations de l'ESMO, permet d'améliorer la qualité de l'exérèse chirurgicale des sarcomes et évite les gestes chirurgicaux inappropriés.

Summary
Introduction

Despite regular recommendations issued by the European Society for Medical Oncology (ESMO), patients faced still too often inadequate care with a direct influence on prognosis.

Methods

A retrospective study was carried out at the Competence center in Lorraine Area. Patients registered in the NetSarc database between 1st, 2010 and September 1st, 2016 were included. Compliance criteria were established using the latest ESMO 2014 referential. Two groups "conforming" and "non conforming" were analyzed. A first analyze about all of the soft tissue lesion and a second only about sarcomas.

Results

In total, 445 patients were eligible, 344 cases were treated according to the ESMO guidelines, giving a 77.3% conformity rate (95% CI: 73.4%, 81.2%). Compliance was better for the competence center than district hospitals (P < 0.001), with compliance rates of 88.7%, and 51.6%, respectively. Among the 247 sarcomas, we found a R0 resection rate better according to the ESMO guideline, 55% against 18% (P < 0.001). R1 rates were 34% vs. 56% and R2 11 vs. 26% disease free survival was not related significantly to the observance of recommendations in the univariate analysis. After adjustment on potential DFS prognostic factors, in the multivariate analysis, the results were similar.

Conclusion

Compliance with the ESMO guidelines, through appropriate management, improves the quality of surgical excision for sarcomas and avoids non-corresponding surgical gestures.

The Use of Video Glasses Improved Learning of Tonsillectomy and Adenoidectomy Surgery: A Randomized Controlled Trial

Objective

One of the most common challenges in surgical education for trainees is gaining practical experience through observing procedures in the operating room. Due to the nature of some procedures, a narrow surgical view severely limits the learning experience. Video glasses are new devices that offer the potential to project the primary surgeon's exact view to learners in real-time, allowing for an enhanced operative learning experience.

Study Design

Single center randomized prospective trial.

Setting

Tertiary care pediatric hospital.

Participants

Using block randomization, medical students and surgical residents observed either a tonsillectomy or adenoidectomy, either directly at table-side or by real-time video feed from the surgeon's video glasses projected to a screen in the operating room, in random order. Participants then completed a survey comparing aspects of their learning experience viewing the procedure through the video feed in comparison to direct observation.

Main Outcome

Measures: Evaluating the hypothesis that video glasses provided an improved overall learning experience and a realistic simulation of the open surgical procedures tested.

Results

23 trainees participated in the study. Survey results demonstrated that the overall learning experience with the use of video glasses was significantly improved when compared to direct visualization (average Visual Analog scale (VAS) score 82/100 vs. 64/100, p=0.021).Video glasses were shown to be superior when comparing the view of the surgical field (83/100 vs. 54/100 on VAS, p<0.001) and the ability to identify anatomical structures (79/100 vs. 56/100 on VAS, p=0.001). The ease of following surgical steps with video glasses was also shown to be better than by direct visualization (81/100 vs. 69/100 on VAS, p=0.039). All participants stated that video glasses closely simulated the learning environment of the real-life open procedure.

Conclusion

This study showed that the use of video glasses was beneficial for surgical education and a realistic tool for learners at varying levels of training. Video glasses may significantly improve the learning experience for procedures with a narrow field of view.

The purpose of this study was to describe and predict hearing aid uptake and retention in Korean pediatric patients with unilateral hearing loss (UHL) in a secondary referral hospital.

Methods

This was a retrospective study using clinical data collected at the time of UHL diagnosis. The study included data collected from 2009 to 2016. Serial audiograms were extracted from clinical charts, and follow-up status and rehabilitation decisions were analyzed.

Results

Of 102 children and adolescents (9.5±5.1 years, 64 male), 52.9% followed a check-up schedule, and 31 (30.4%) obtained a hearing aid. Hearing threshold and speech discrimination scores were predictive parameters of hearing aid uptake. Among those who used a hearing aid, 17 (56.7%) subjects used it successfully based on significant predictive parameters of channel number.

Conclusion

Hearing aid retention in pediatric patients seems less predictable than in adults with UHL. No good predictable parameter for hearing aid retention was identified except channel number for pediatric UHL cases. Regular monitoring of hearing and selection of a multi-channel hearing aid are crucial to minimize the potential negative effects of UHL.

Abstract

Structural equation modeling using full information maximum likelihood is the predominant method to empirically assess complex theories involving multiple latent variables in addiction research. Although full information estimators have many desirable properties including consistency, a major limitation in structural equation models is that they often sustain significant bias when implemented in small to moderate size studies (e.g., fewer than 100 or 200). Recent literature has developed a limited information estimator designed to address this limitation—implemented through a bias-corrected factor score path analysis approach—that has been shown to produce unbiased and efficient estimates in small to moderate sample settings. Despite its theoretical and empirical merits, literature has suggested that the method is underused because of three primary reasons—the methods are unfamiliar to applied researchers, there is a lack of practical and accessible guidance and software available for applied researchers, and comparisons against full information methods that are grounded in discipline-specific examples are lacking. In this study, I delineate this method through a step-by-step example analysis of a sequential mediation involving internet addiction. I provide example R code using the lavaan package and data based on a hypothetical study of addiction. I examine the differences between the full and limited information estimators within the example data and subsequently probe the extent to which these differences are indicative of a consistent divergence between the estimators using a simulation study. The results suggest that the limited information estimator outperforms the conventional full information maximum likelihood estimator in small to moderate sample sizes in terms of bias, efficiency, and power.

Aims

Compared to the general U.S. population, military veterans and those living in rural areas disproportionately smoke cigarettes at higher rates, leading to increased health consequences. In the current study, prevalence and severity of cigarette smoking in Iraq and Afghanistan era veterans was assessed across rural and urban areas and comorbid mental health disorders.

Method

Iraq/Afghanistan era veterans who participated in the Post-Deployment Mental Health study from 2005 to 2017 (N = 3229) were cross-sectionally assessed for the probability of being a current cigarette smoker based on locality status and psychiatric comorbidity. Multivariate logistic and linear regressions, adjusted for demographic characteristics, were used to model the odds of being a current smoker and the severity of nicotine dependence, respectively.

Results

Veterans residing in rural regions, veterans with psychiatric comorbidities, and the interaction of locality and psychiatric disorders were significantly associated with smoking rates. Those veterans living in extremely rural areas and, independently, those living with psychiatric comorbidities were also more severely dependent on nicotine compared to urban veterans and veterans without psychiatric conditions.

Conclusions

Rural veterans and veterans with psychiatric comorbidities are at increased risk of smoking and are more severely dependent on nicotine than urban veterans. These findings underscore the need to reduce barriers for treatment both for smoking cessation and mental healthcare for veterans residing in the most rural areas.

A practical approach to limit the radiation dose from building materials applied in dwellings, in compliance with the Euratom Basic Safety Standards

Abstract

Individuals receive a significant part of their radiation exposure indoors. We anticipate that this exposure is likely to increase in the near future, due to a growing use in the building industry of recycled materials and materials previously regarded as waste. Such materials often contain elevated levels of natural radionuclides. Directive 2013/59/Euratom ('Basic Safety Standards', BSS) pays comprehensive attention to indoor exposure from natural radionuclides, but proper implementation of all corresponding BSS regulations is not straightforward, especially when regarding the regulation of building materials containing so-called Annex XIII materials. In this paper, we discuss the most relevant deficiencies in the BSS and present a practical approach to cope with these. Our most important observation is that adequate methods for assessing the annual dose due to gamma radiation from building materials are not provided by the BSS. This is in particular difficult because compliance of single building materials has to be tested, but the corresponding BSS reference level refers to gamma radiation emitted by all building materials present in a room. Based on a simple model of three layers of building materials, we present a set of operational conditions for building materials, either used for construction purposes ('bulk layers') or for the finishing of walls, floors and ceilings ('superficial layers'). Any customary combination of building materials meeting these conditions will stay below the BSS reference level for gamma radiation. This statement holds for the middle of a reference room, but is not always the case close to the walls, especially when low density materials with a relatively high content of natural radionuclides are present at the inner side of the room. This can be avoided by applying more strict conditions for those kind of materials than presented in this paper. We further focus on the indoor exposure to thoron progeny. Building materials that pass the test for gamma radiation can still be a significant source for indoor air concentrations of thoron progeny. When the average annual thoron inhalation dose were to be restricted to 1 mSv a−1 − a level comparable to the BSS reference level for gamma radiation − the activity concentration of Ra-224 in (especially porous) building materials used for wall finishing purposes should be limited to a value of typically 50 Bq kg−1. Even if our suggested approach of the BSS regulations is fully implemented, it still allows for a significant increase in the average radiation exposure in dwellings due to external radiation and thoron progeny. However, the situation will be worse if a less strict interpretation of the BSS regulations will be applied.

A 28-year-old man with Fraser syndrome presented with cryptophthalmos, microphthalmia, lacrimal system dysgenesis, and chronic sinusitis.
Objective: The patients' clinical condition and surgical treatment are described. A literature review was conducted, and articles relevant to the case are presented.
Methods: Case report.
Results: To our knowledge, this is the first published case report of endonasal management of dacryocystoceles in a Fraser syndrome patient. The patient was treated via endoscopic endonasal marsupialization and drainage.
Conclusion: Fraser syndrome patients may initially present to many different specialties as the spectrum of clinical manifestations is broad. Physicians treating these patients should take a collaborative approach to surgical and medical management.

Transarterial embolization of peripheral high-flow arteriovenous malformation with ethylene vinyl alcohol copolymer (Onyx ® ): single-center 10-year experience

Abstract

Purpose

High-flow arteriovenous malformations (AVMs) are complex vascular lesions for which transcatheter embolization is considered as first-choice treatment nowadays. Multiple embolizing agents have been described, and among them, Onyx® seems to be promising; this is a liquid embolic agent, originally applied in neurointerventional radiology and recently adopted also in peripheral embolizations. The aim of this study is to report on a 10-year experience of transarterial embolization of peripheral high-flow AVM with Onyx® in terms of technical and clinical outcomes.

Materials and methods

Retrospective analysis was conducted on patients affected by high-flow AVM and treated electively by transarterial embolization with Onyx®. Data collection included: preinterventional clinical radiological evaluations, procedural data and post-procedural clinical radiological assessment. Technical and clinical success was evaluated; follow-up was conducted 30 days after the last treatment session and yearly in case of success.

Results

Sixteen patients have been included, totally 38 embolizing procedures. Additional embolizing agents were required in 5 patients. Technical success was obtained in 11 patients; at 30-day follow-up, 15 patients showed improvements in symptoms, even those with incomplete embolization; however, after almost 1 year from treatment accomplishment, 7 patients showed relapse of symptoms and presented radiological signs of AVM recurrence. No clinically relevant complications occurred.

Conclusions

In this series, in accordance with previous but limited published data, Onyx® appeared safe and technically effective to embolize high-flow peripheral AVM with transarterial approach. Clinical radiological follow-up is mandatory because new feeder recruitment has to be expected; patients should be informed of the concrete possibility of multiple treatment sessions.

On the scaling behavior of water diffusion in human brain white matter

Development of therapies for neurological disorders depends on our ability to non-invasively diagnose and monitor the progression of underlying pathologies at the cellular level. Physics and physiology limit the resolution of human MRI to be orders of magnitude coarser than cell dimensions. Here we identify and quantify the MRI signal coming from within micrometer-thin axons in human white matter tracts in vivo, by utilizing the sensitivity of diffusion MRI to Brownian motion of water molecules restricted by cell walls. We study a specific power-law scaling of the diffusion MRI signal with the diffusion weighting, predicted for water confined to narrow axons, and quantify axonal water fraction and orientation dispersion.

MDCT in acute ischaemic left colitis: a pictorial essay

Abstract

The pathogenesis of acute ischaemic colitis depends on two different forms of vascular colonic insult: occlusive injury and non-occlusive injury. Clinically, ischaemic colitis may be classified as two major forms: mild (non-gangrenous) and acute fulminant (gangrenous). The classic presentation is abdominal pain, diarrhoea and/or rectal bleeding, but it is not specific and highly variable and so the diagnosis usually depends on clinical suspicion and is supported by serologic and colonoscopic findings. Imaging methods have their role in diagnosing IC. While plain radiography and ultrasound can orient the diagnosis, CT allows to define the morphofunctional alterations discriminating the non-occlusive forms from the occlusive forms and in most cases to estimate the timing of ischaemic damage. Purpose of the review is to define the role of CT in the early identification of pathological findings and in the definition of evolution of colonic ischaemic lesions, in order to plan the correct therapeutic approach, suggesting the decision of medical or surgical treatment.

V1 activity during feedforward and early feedback processing is necessary for both conscious and unconscious motion perception

Abstract

The study of blindsight has revealed a seminal dissociation between conscious vision and visually guided behavior: some patients who are blind due to V1 lesions seem to be able to employ unconscious visual information in their behavior. The standard assumption is that these findings generalize to the neurologically healthy. We tested whether unconscious processing of motion is possible without the contribution of V1 in neurologically healthy participants by disturbing activity in V1 using transcranial magnetic stimulation (TMS). Unconscious processing was measured with redundant target effect (RTE), a phenomenon where participants respond faster to two stimuli than to one stimulus, when the task is just to respond as fast as possible when one stimulus or two simultaneous stimuli are presented. We measured the RTE caused by a motion stimulus. V1 activity was interfered with different stimulus onset asynchronies (SOA) to test whether TMS delivered in a specific time window suppresses conscious perception (participant reports seeing only one of the two stimuli) but does not affect unconscious processing (RTE). We observed that at each SOA, when TMS suppressed conscious perception of the stimulus, the RTE was also eliminated. However, when visibility of the redundant target was suppressed with a visual mask, we found unconscious processing of motion. This suggests that unconscious processing of motion depends on V1 in neurologically healthy humans. We conclude that the neural mechanisms that enable motion processing in blindsight are modulated by neuroplastic changes in connectivity between subcortical areas and the visual cortex after the V1 lesion. Neurologically healthy observers cannot process motion unconsciously without functioning of V1.

Transarterial embolization of peripheral high-flow arteriovenous malformation with ethylene vinyl alcohol copolymer (Onyx ® ): single-center 10-year experience

Abstract

Purpose

High-flow arteriovenous malformations (AVMs) are complex vascular lesions for which transcatheter embolization is considered as first-choice treatment nowadays. Multiple embolizing agents have been described, and among them, Onyx® seems to be promising; this is a liquid embolic agent, originally applied in neurointerventional radiology and recently adopted also in peripheral embolizations. The aim of this study is to report on a 10-year experience of transarterial embolization of peripheral high-flow AVM with Onyx® in terms of technical and clinical outcomes.

Materials and methods

Retrospective analysis was conducted on patients affected by high-flow AVM and treated electively by transarterial embolization with Onyx®. Data collection included: preinterventional clinical radiological evaluations, procedural data and post-procedural clinical radiological assessment. Technical and clinical success was evaluated; follow-up was conducted 30 days after the last treatment session and yearly in case of success.

Results

Sixteen patients have been included, totally 38 embolizing procedures. Additional embolizing agents were required in 5 patients. Technical success was obtained in 11 patients; at 30-day follow-up, 15 patients showed improvements in symptoms, even those with incomplete embolization; however, after almost 1 year from treatment accomplishment, 7 patients showed relapse of symptoms and presented radiological signs of AVM recurrence. No clinically relevant complications occurred.

Conclusions

In this series, in accordance with previous but limited published data, Onyx® appeared safe and technically effective to embolize high-flow peripheral AVM with transarterial approach. Clinical radiological follow-up is mandatory because new feeder recruitment has to be expected; patients should be informed of the concrete possibility of multiple treatment sessions.

18 F-FDG PET/CT detects Metastatic Renal Cell Carcinoma Masquerading as Primary Breast Malignancy

Abstract

We present the case of a 36-year-old woman who underwent 18F-FDG PET/CT with suspicion of a primary breast malignancy. However, PET/CT detected an occult renal cell carcinoma with metastases to the thyroid, breast, lungs and lymph nodes. Thyroid and breast metastases are atypical metastatic sites of renal cell carcinoma. Breast metastases from extra mammary tissue are extremely rare, more so from renal cell carcinoma. Histopathologic confirmation of the breast lesions is imperative to avoid unnecessary mastectomy and imaging can help in raising the suspicion of metastatic involvement versus primary breast malignancy.
