Coronectomy has become an increasingly prescribed surgical treatment for mandibular third molars deemed to pose a risk to the inferior dental nerve. The retention of the roots can have subsequent potential for root retrieval in the future if symptoms are reported. The long term outcome and symptoms leading to coronectomy root retrieval have not been well documented or studied which has understandably led to hesitation by some clinicians offering the procedure. The current series assesses the patients who have undergone root retrieval and their reported indication for removal as well as the histopathological status of the removed roots.

The Shamblin classification is the commonest method to predict complications and outcomes during carotid body tumor (CBT) resection. The aim of this study is to describe the utility of the three-dimensional volumetric reconstruction (3DVR) analysis of the CBT and its relationship with postoperative outcomes.
Preoperative computed tomography angiography (CTA) was obtained to perform 3DVR of the CBT. Imaging data were analyzed and correlated with surgical outcomes: estimated blood loss (EBL), surgical time (ST), and hospital length of stay (HLOS). The Pearson test was used to determine the correlation between volume and postoperative outcomes.
Fifty-seven patients were studied, 13 had Shamblin type I tumors with a mean 3DVR of 7.69 cm3 (standard deviation [SD 8.27]), 21 were type II with a mean 3DVR of 15.57 cm3 (SD 8.40), and 23 were type III with a mean 3DVR of 30.58 cm3 (SD 20.16). EBL mean was 559 milliliters (mL) (standard error [SE] 80.44), the mean ST of resection was 202 min (SD 67), and the mean HLOS was 5.8 days (SD 3.23). The Pearson test showed a correlation of r = 0.69 (p < 0.0001) between 3DVR and EBL, a r = 0.4 (p = 0.05) was obtained between 3DVR and ST, and finally, a r = 0.3 (p = 0.05) between 3DVR and HLOS was obtained.
The preoperative 3DVR determination of CBTs allows to analyze possible predictors of surgical outcomes. We found a positive correlation between the 3DVR volume and EBL. Further research is necessary to validate this method in the evaluation of these rare neoplasms.

Records from 202 patients with clinically suspected allergic sensitizations were extracted from files at two sites applying either the "ISAC-first" workup with IgE-testing by immuno-solid phase allergen chip ISAC112 followed by selected skin prick tests (SPT) or the "SPT-first" starting with SPT followed by the microarray test.
In the ISAC-first procedure significantly less SPTs were performed during allergy diagnosis (median 4 vs. 14). By SPT in 19% of patients in the ISAC-first group and in 34% in the SPT-first group additional respiratory allergens (p = 0.014) were detected not positive in ISAC microarray. By ISAC microarray test 18% additional sensitizations were found in the ISAC-first, and 32% in SPT-first cohort (p = 0.016). For food allergens 13 and 12% additional sensitizations were detected by the microarray not detected by SPT in the two groups (p = 0.800). No additional food allergen was found by SPT in the ISAC-first group, while in 6% of the cases in the SPT-first group detected sensitizations were negative in the microarray.
The ISAC-first approach followed by (fewer) SPTs meets the demands for a patient's tailored diagnostic work-up and therefore can be considered equivalent to the conventional way using the skin prick test as first screening tool, followed by IgE diagnosis.
For the diagnostic verification of clinically suspected allergy, the novel concept "from molecules to clinic" offers a reliable diagnostic workup in shorter time. Due to lower skin test numbers it is especially applicable for young children and seniors, in atopic patients, and whenever skin tests get difficult or unreliable.

Children with bicuspid aortic valve (BAV) are at risk for serious complications including aortic valve stenosis and aortic rupture. Most studies investigating biomarkers predictive of BAV complications are focused on adults.
To investigate whether hemodynamic parameters change over time in children and young adults with BAV by comparing baseline and follow-up four-dimensional (4-D) flow MRI examinations.
We retrospectively included 19 children and young adults with BAV who had serial 4-D flow MRI exams (mean difference in scan dates 1.8±1.0 [range, 0.6–3.4 years]). We compared aortic peak blood flow velocity, three-dimensional (3-D) wall shear stress, aortic root and ascending aortic (AAo) z-scores between baseline and follow-up exams. We generated systolic streamlines for all patients and visually compared their baseline and follow-up exams.
The only significant difference between baseline and follow-up exams occurred in AAo z-scores (3.12±2.62 vs. 3.59±2.76, P<0.05) indicating growth of the AAo out of proportion to somatic growth. There were no significant changes in either peak velocity or 3-D wall shear stress between baseline and follow-up exams. Ascending aortic peak velocity at baseline correlated with annual change in AAo z-score (r=0.58, P=0.009). Visual assessment revealed abnormal blood flow patterns, which were unique to each patient and remained stable between baseline and follow-up exams.
In our pediatric and young adult BAV cohort, hemodynamic markers and systolic blood flow patterns remained stable over short-term follow-up despite significant AAo growth, suggesting minimal acute disease progression. Baseline AAo peak velocity was a predictor of AAo dilation and might help in determining pediatric patients with BAV who are at risk of increased AAo growth.

Recommendations for safe medication injection practices to eliminate the risk of patient-to-patient transmission of blood-borne infections have been in place for many years. The purpose of our study was to evaluate the medication administration practices of Canadian anesthesiologists relative to current safe practice guidelines.
An anonymous 17-question online survey was sent to all members of the Canadian Anesthesiologists' Society (CAS) via the membership email list. Data pertaining to respondent demographics, practice characteristics, and medication preparation and administration practices were collected and analyzed descriptively using frequencies and percentages as well as Fisher's exact tests followed by post hoc multiple comparisons.
Of the 2,656 CAS members, 546 (21%) responded. The practice of reusing needles (2%) and/or syringes (7%) between patients is reported by only a minority of practitioners; however, sharing a medication vial between more than one patient using new needles and syringes is still widely practiced with 83% doing this sometimes or routinely. The main reasons for sharing medications include the desire to reduce medication waste and the associated costs.

To describe critical care medicine residents' training, expertise, and skills regarding organ and tissue donation processes and procedures.
We undertook a qualitative multicentre study and employed a purposive sample of program directors, physicians, nurses, residents, and organ donation leaders from all nine academic intensive care unit (ICU) training centres (five adult, four pediatric) in Ontario (n = 71). Interviews, conducted by telephone between December 2015 and March 2016, were audio-recorded and transcribed verbatim. Data collection and analysis were performed using an iterative process and continued until saturation was achieved.
Five main themes were identified: 1) gaps in residents' knowledge for both neurologic determination of death (NDD) and circulatory determination of death (DCD) cases; 2) commitment to the provision of organ and tissue donation training; 3) limited experiential learning (NDD and DCD); 4) challenges related to the provision of training on organ donation and need for a standardized curriculum; and 5) communication with family members. Overall, this study showed system-level gaps in training resulting from the lack of a standardized provincial curriculum on organ donation.
Qualitative data corroborated that residents need more exposure to clinical cases, especially regarding DCD donors. A standardized education curriculum would be beneficial for all residents within the ICU. Developing a better shared understanding of the donation process will improve team communication and performance, translate into a better end-of-life experience for families, and potentially result in increased donation rates.
