European Surveillance System on Contact Allergies (ESSCA): contact allergies in relation to body sites in patients with allergic contact dermatitis
Background
Analyses of the European Surveillance System on Contact Allergies (ESSCA) database have focused primarily on prevalence of contact allergies to the European baseline series; overall and in subgroups of patients. However, affected body sites have hitherto not been addressed.
Objective
To determine prevalence of contact allergies for distinct body sites in patients with allergic contact dermatitis (ACD).
Methods
Analysis of data collected by ESSCA (www.essca‐dc.org) in consecutively patch tested patients, 2009‐2014, in 8 European countries. Cases were selected based on the presence of minimally one positive patch test to the baseline series, and a final diagnosis of ACD attributed to only one single body site.
Results
N=6,255 cases were analyzed. The head and hand were the most common single sites ACD was attributed to. Differences between countries were seen for several body sites. Nickel, fragrance mix I, cobalt and methylchloroisothiazolinone/methylisothiazolinone were the most frequent allergens reported for various body sites.
Conclusion
Distinct allergen patterns per body site were observed. However, contact allergies were probably not always relevant for the dermatitis that patients presented themselves with. Adding possibility to link positive patch test reactions to relevance, along with affected body sites should be a useful addition to data capturing systems.

However, since the introduction of national standards on day case surgery in the UK in 2011, patients at the Royal Gwent Hospital, Newport, have been treated as day cases, and have been pleased with the results. We made a systematic search of published papers listed in PubMed, Web of Knowledge, Scopus, and Cochrane Central up to March 2018, and identified a total of 437 records. Eight studies met the inclusion criteria, in which seven significant risk factors for early failure were selected and used to build a conceptual simulation model. Selected risk factors were: "male sex", "smoking", "quality of bone", "short implants", "wide implants", "adjacent teeth", and "periodontitis". Lack of Sphenoid Pneumatization Does Not Affect Endoscopic Endonasal Pediatric Skull Base Surgery Outcomes

Currently, due to the rarity of pathology, there are limited data surrounding outcomes of pediatric skull base surgery. Traditionally, surgeons have proceeded with caution when electing endonasal endoscopic transsellar/transplanum approaches to the skull base in pediatric patients due to poor sphenoid pneumatization. In this study, we review outcomes of endoscopic pediatric skull base surgery based on sphenoid pneumatization patterns.
Study Design
Retrospective chart review.
Methods
A review of all cases of pediatric (age < 18 years) craniopharyngioma managed via an endoscopic endonasal approach at a tertiary academic medical center.
Results
A total of 27 patients were included in the analysis. The median age was 8 years. Nineteen (70%) patients were male. Presellar, sellar/postsellar, and conchal sphenoid pneumatizations were found in 6, 11, and 10 patients, respectively. There was no significant association between sphenoid pneumatization pattern and extent of resection (gross vs. subtotal, P = .414), postoperative cerebrospinal fluid (CSF) leak (P = .450), intraoperative estimated blood loss (P = .098), total operative time (P = .540), and length of stay (P = .336). On multivariate analysis, after accounting for age, sex, preoperative cranial nerve involvement, and cavernous sinus invasion, there remained no significant association between sphenoid pneumatization pattern and extent of resection (P = .999) and postoperative CSF leak (P = .959).
Conclusions
Sphenoid pneumatization pattern does not appear to affect outcomes in endoscopic skull base surgery in the pediatric population. Importantly, lack of sphenoid pneumatization does not impede gross total resection or increase complications. Thorough knowledge of the anatomy during the endoscopic approach is critical to optimize outcomes.

New Cancer Therapies: Implications for the Perioperative Period
Abstract
Purpose of Review
Cancer is on the rise. Standing on verge of exciting discoveries, research is being translated into therapies that are being widely administered to patients. Providing a hope for cure, where none existed before. This new body of knowledge has come from a better understanding of cancer genetics, molecular and sub molecular behavior, and understanding of cancer-generated cellular environments. These have led to development of immunotherapy and its many sub-genres, improvement and introduction of new radiation technologies, and decreasing toxicities of existing chemotherapies.
Recent Findings
The purpose of this review is to have a summary look at this huge landscape of cancer therapy. Specially looking at toxicities that an anesthesiologist should be familiar with while providing perioperative care for these patients, complications like tumor lysis syndrome, cytokine release syndromes, Kounis syndrome, myocarditis, encephalopathies, and pituitary failure need to be kept in mind.
Summary
One should be knowledgeable about these therapies and approach these patients with a high index of suspicion. Anesthesiologists will need to refine preoperative assessment with appropriate testing and intraoperative and postoperative management in collaboration with oncologists, while involving the expertise of internists, cardiologist, and endocrinologists in helping assess and manage these patients in the perioperative period.

Arrhythmia in thiamine responsive megaloblastic anemia syndrome.
We present cardiac features of five patients with TRMAS. Five patients had macrocytic anemia, diabetes mellitus, and sensorineural deafness. Two siblings had also optic atrophy. SLC19A2 gene mutation was shown in all patients. Two patients developed supraventricular tachycardia during an episode of diabetic ketoacidosis. Five patients had absent P waves on baseline electrocardiography, and one patient had additional low QRS voltage. None of the patients had structural heart disease. Hintergrund In Deutschland leiden etwa 15 Millionen Menschen an einer Schwerhörigkeit (SH), doch nur 16 % tragen Hörgeräte. Diese Unterversorgung kann für die Betroffenen zu schwerwiegenden Folgen (soziale Isolation, Depression, Förderung einer Demenz etc.) führen. Ein frühzeitiges und flächendeckendes Hörscreening ab dem 50. Lebensjahr kann dies verbessern. In dieser Studie wurde der Mini-Audio-Test (MAT), ein sechs Fragen zum subjektiven Hörverlust (HV) umfassender Test mit dreistufiger Antwortskala, an einem Normalkollektiv überprüft (Sensitivität (Se), Spezifität (Sp), positiver prädiktiver Wert (PPW)). Methode Es wurden 943 Patienten (älter als 50 Jahre) ohne bekannte Ohrenerkrankungen mittels MAT befragt, zum Vergleich die Hörschwellen tonaudiometrisch ermittelt und die Se, Sp und der PPW zur Detektion eines relevanten HV durch den MAT für die Altersklassen < 60 Jahre (AG1) und ≥ 60 Jahre (AG2) mittels binomialer Proportionen ermittelt. Ergebnisse Die Se lag für AG1 bei 0,66, die Sp bei 0,61, der PW bei 0,60, für die AG2 lag die Se bei 0,47, die Sp bei 0,80, der PPW bei 0,89. Schlussfolgerung Entsprechend der vorliegenden Ergebnisse wird der Einsatz des MAT als Screeningmethode für Nicht-HNO-Ärzte zur Detektion von schwerhörenden Patienten ab dem 50. Lebensjahr vorgeschlagen. Hierdurch ließen sich auch schwerwiegende Begleiterkrankungen wie Minderung der kognitiven Leistungsfähigkeit, Sturzrisiko, Depression und Demenz positiv beeinflussen.

Arrhythmia in thiamine responsive megaloblastic anemia syndrome.
We present cardiac features of five patients with TRMAS. Five patients had macrocytic anemia, diabetes mellitus, and sensorineural deafness. Two siblings had also optic atrophy. SLC19A2 gene mutation was shown in all patients. Two patients developed supraventricular tachycardia during an episode of diabetic ketoacidosis. Five patients had absent P waves on baseline electrocardiography, and one patient had additional low QRS voltage. None of the patients had structural heart disease. The North-Western Italian experience with anti IL-5 therapy amd comparison with regulatory trials
Abstract
Background
The severe forms of asthma represent a major burden, because of severity of symptoms, costs and impact on everyday life. Recently, Mepolizumab (MEP) was approved and marketed for the treatment of hypereosinophilic severe asthma. This anti-IL-5 monoclonal antibody reduced exacerbation rates and oral corticosteroid (OCS) use in well selected patients. The aim of this study was to evaluate the characteristics of patients receiving MEP in a real-life setting. Thus, we describe a retrospective analysis of patients treated with MEP in six centres in North Western Italy, including those who participated in the main regulatory trials.
Methods
The baseline data, before prescription, from six North Western Italy severe asthma clinics, between June 1st 2017 and December 31st 2017, were evaluated. The collected real-life data were then compared with those of SIRUS, MENSA, DREAM and MUSCA trials.
Results
Sixty-five patients were included (45% female; mean age 56 years; age range 19–84). Main observed differences with regulatory trials could be observed in eosinophils blood count at baseline, where the mean of our real-life patients (653 cells/μL) was overall higher than the one of all trials (240 cells/μL, 296 cells/μL, 253 cells/μL; p < 0.0001). The incidence of polyposis was also significantly higher in our sample (72% vs. 24%, 49%, 10%, 19%; p < 0.0001). The daily average dose of OCS was lower in our real-life patients (9 mg), if compared with SIRIUS (13.7 mg), MENSA (13.2) and MUSCA (13), and similar to the data published in DREAM (10.8).
Conclusions

Correlations between cervical spinal cord magnetic resonance diffusion tensor and diffusion kurtosis imaging metrics and motor performance in patients with chronic ischemic brain lesions of the corticospinal tract
Abstract
Purpose
To investigate modifications of Magnetic Resonance Diffusion Tensor Imaging (DTI) and Diffusion Kurtosis Imaging (DKI) metrics in lateral white matter (WM) bundles of the cervical spinal cord in patients with previous stroke in the vascular territory of the middle cerebral artery (MCA).
Methods
Twenty consecutive patients with a previous ischemic stroke of the MCA territory and a varying degree of upper motor impairment were enrolled. DKI was centered at the C3C4 and C5C6 intervertebral level.
Results
The fractional anisotropy (FA) values in C3C4 and C5C6 were found to be significantly lower in the lateral WM bundles contralateral to the ischemic lesion and thus, in the WM bundle including the affected corticospinal tract (CST) (p = 0.005 and p = 0.008, respectively), as well as mean kurtosis (MK) and axonal water fraction (AWF) values (p = 0.004 and p = 0.04. respectively). FA values correlated significantly with the Global Motor Index (GMI) both for C3C4 (ρ = 0.61, p = 0.004) and C5C6 (ρ = 0.69, p = 0.002). At C3C4, AWF correlated significantly with GMI (ρ = 0.54, p = 0.03). No correlations were found between lateral WM bundle volumes and GMI.
Conclusion
A reduction of anisotropy and microstructural complexity in the affected lateral WM bundle of the cervical spinal cord was observed in patients with previous ischemic stroke involving the CST. The correlations between these metrics and motor performance were statistically significant.

Long‐term outcomes in chronic hypersensitivity pneumonitis
Abstract
Introduction
The objective of this study was to analyze mortality, possible predictors of long‐term survival and health‐related quality of life of a large chronic hypersensitivity pneumonitis (CHP) patient sample.
Methods
Longitudinal study in patients diagnosed with CHP during 2004‐2013, followed for at least one year. Patients remaining alive and consenting to participate had a follow‐up visit during 2015, including a complete pulmonary function study and the EuroQol‐5D and Beck Depression and Anxiety Inventories.
Results
Out of the 160 patients finally included, 87 remained alive. Seventy‐three had died or underwent lung transplantation at the time of the study with a median survival of 7.0 (4.4‐14.5) years. A Cox proportional risk model showed that factors associated with lower survival were: increased age, a low percentage of lymphocytes in bronchoalveolar lavage (BAL), a decreased transfer factor of the lung for carbon monoxide (DLCO), presence of honeycomb in the high resolution chest scan (HRCT) and the usual interstitial pneumonia (UIP) histologic pattern. Conclusion
CHP is a severe disease with a bad mid‐term prognosis. Lymphocyte values in BAL and DLCO values at baseline, presence of honeycomb in HRCT and UIP histologic pattern were found to be predictors of survival. Early accurate diagnosis of the disease is fundamental for prompt initiation of antigen avoidance.

Long‐term outcomes in chronic hypersensitivity pneumonitis
Abstract
Introduction
The objective of this study was to analyze mortality, possible predictors of long‐term survival and health‐related quality of life of a large chronic hypersensitivity pneumonitis (CHP) patient sample.
Methods
Longitudinal study in patients diagnosed with CHP during 2004‐2013, followed for at least one year. Patients remaining alive and consenting to participate had a follow‐up visit during 2015, including a complete pulmonary function study and the EuroQol‐5D and Beck Depression and Anxiety Inventories.
Results
Out of the 160 patients finally included, 87 remained alive. Seventy‐three had died or underwent lung transplantation at the time of the study with a median survival of 7.0 (4.4‐14.5) years. A Cox proportional risk model showed that factors associated with lower survival were: increased age, a low percentage of lymphocytes in bronchoalveolar lavage (BAL), a decreased transfer factor of the lung for carbon monoxide (DLCO), presence of honeycomb in the high resolution chest scan (HRCT) and the usual interstitial pneumonia (UIP) histologic pattern. Marjoram Extract Down-regulates the Expression of Pasteurella multocida Adhesion, Colonization and Toxin genes: a Potential Mechanism for its Antimicrobial activity
This study provides an in vitro research framework to investigate the antibacterial activities of 5 herbal (marjoram, garlic, onion, cinnamon and black seed) extracts against 16 multidrug-resistant and virulent P. multocida serogroup A isolates recovered from dead and clinically diseased rabbits. Abstract
Objective
Literature review comparing post-tonsillectomy hemorrhage in pediatric and adult patients with and without suturing tonsil pillars to investigate whether suturing tonsil pillars reduces the risk of post-tonsillectomy hemorrhage.
Review
methods; Online journal databases were searched using the key phrases "post tonsillectomy hemorrhage", "post tonsillectomy bleed", and "tonsil pillar suture". 10 published studies were found regarding tonsil pillar suturing, four directly related to postoperative bleeding and five focusing on postoperative pain reduction. There was one study that evaluated both pain and bleeding. The pain reduction studies were comprised of 225 patients while the postoperative bleeding studies included 3987 patients.
Conclusions
Suturing tonsil pillars after tonsillectomy may be beneficial after cold tonsillectomy.
Implications for practice; Post-operative bleeding is one of the most common complications that can result in increased patient distress and hospitalization. In this article, we provide literature review of tonsil pillar suturing and post-tonsillectomy hemorrhage. Our study suggests suturing the tonsil pillars immediately post-tonsillectomy may reduce the risk of severe post-operative bleeding requiring return to the operating room for certain patients.

Abstract
Objective
E-cigarette use has increased significantly over the past decade. Among the growing population of e-cigarette users, several pattern-based subgroups have emerged, including daily and non-daily users. Daily users are at greater risk for experiencing negative health consequences from use. Yet, little is understood about underlying e-cigarette processes that may be related to daily use relative to non-daily use.
Method
The present study sought to investigate differences in three clinically-relevant e-cigarette processes, including perceived risks (including both absolute and comparable risks to combustible cigarettes), benefits, and innovation of e-cigarettes between daily and non-daily users. The study included 564 adult current e-cigarette users (51.1% female, Mage = 35.1 years, SD = 10.2).
Results
Results indicated a significant difference in levels of perception for all three dependent variables, such that daily users reported a higher level of perceived risks, benefits, and innovation for e-cigarettes.
Conclusions
Overall, the current study provides empirical evidence that perceived risks, benefits and innovations may be more strongly endorsed by more daily e-cigarette uses. Importantly, this study adds to evolving, but thus far limited, knowledge of e-cigarettes by highlighting how frequency of e-cigarette use relates to beliefs about e-cigarette use.

Tobacco smoking and opioid use disorders: A meta-analysis

Abstract
Background
Tobacco smoking has been shown a major risk factor for opioid use disorders in several observational studies, but the results are inconsistent. We conducted a systematic review and meta-analysis of observational studies to investigate the association between smoking behaviors and opioid use disorders.
Methods
We performed a systematic literature search of Medline, Embase, Scopus, ISI Web of Science, and Google Scholar through October 2017, using MeSH terms related to smoking, opioid, and opiate. The reference list of retrieved article was also examined. We calculated pooled odds ratio (ORs) with 95% confidence intervals (CIs) using fixed and random-effects models. Subgroup analyses was performed using participants and studies characteristics to assess the sources of heterogeneity.
Results
We identified 11 eligible observational studies (7 cohort and 4 population based cross-sectional studies) with 199,411 participants. The pooled OR of opioid use disorders was 3.18 (95% CI: 2.37–4.25) for current smoking compared with nonsmoking. Pooled OR for prevalent cases of opiate use disorders were 13.78 (95% CI: 6.81–27.89), pooled OR for prevalent cases of opioid use disorders were 3.15 (95% CI: 2.62–3.80) and pooled OR for incident cases of opiate use disorders were 1.86 (95% CI: 1.39–2.49). Opioid use disorders was positively associated with earlier age of onset tobacco smoking (pooled OR = 1.66; 95% CI: 1.33–2.07).
Conclusion
The results of present meta-analysis confirm that tobacco smoking is associated with opioid use disorders development. If the association between smoking and risk of opioid use disorders is causal or gateway public health efforts to reduce smoking could have a substantial effect on the worldwide burden of opioid use disorders.
