Preparation of truncated tissue factor antineuropilin-1 monoclonal antibody conjugate and identification of its selective thrombosis in tumor blood vessels

In recent decades, selectively inducing tumor vascular thrombosis, followed by necrosis of tumor tissues has been a promising and potential anticancer strategy. In this report, we prepared a kind of vascular targeting drug that consists of anti-neuropilin-1 monoclonal antibody (anti-NRP-1 mAb) and truncated tissue factor (tTF). Anti-NRP-1 mAb could guide tTF to the surface of tumor vascular endothelial cells and lead to subsequent vascular embolization. This vascular targeting drug, which is also one of the antibody drug conjugates, was generated using a coupling method with water-soluble 1-ethyl-3-(3-dimethylaminopropyl) carbodiimide and N-hydroxysulfosuccimide. Afterwards, in-vitro and in-vivo assays were performed to characterize its potential coagulation ability and antitumor activity. In-vitro experiments indicated that tTF-anti-NRP-1 monoclonal antibody (tTF–mAb) retained both the targeting activity of anti-NRP-1 mAb and the procoagulant activity of tTF. Live imaging system was used to assess its biodistribution and tumor-binding capability, which also yielded promising results. Furthermore, in-vivo studies showed that tTF–mAb was capable of significantly inducing tumor vascular thrombosis and inhibiting tumor growth in nude mice bearing subcutaneous xenografts, and histopathologic changes were rarely observed in normal organs. PSMA5 promotes the tumorigenic process of prostate cancer and is related to bortezomib resistance

Proteasome α5 subunit (PSMA5) is related to poor prognosis in various cancers. The first therapeutic proteasome inhibitor, bortezomib, induces apoptosis, suppressing cell growth in many tumor types. However, the effects of PSMA5 and bortezomib in prostate cancer (PCa) are still unknown. In this study, we investigated whether PSMA5 is associated with the tumorigenic progression and the interaction of PSMA5 with bortezomib in PCa. We knocked down PSMA5 with siRNA and studied the changes in cell viability and motility with Cell Counting Kit-8, quantitative PCR, fluorescence-activated cell sorting, scratch, and invasion assays. We also investigated the effect of PSMA5 in PCa cells treated with bortezomib and in those that are resistant to bortezomib. We found that silencing PSMA5 inhibited cell proliferation, induced apoptosis, restricted cell migration and invasion, and demonstrated a coordinated effect with bortezomib. Cells resistant to bortezomib gained sensitivity to bortezomib after PSMA5 was knocked down. Formylated honokiol analogs showed antitumor activity against lung carcinoma

Honokiol, a biphenolic neolignan with inappreciable toxicity isolated from Magnolia officinalis, has been reported to have antiangiogenic and antitumor properties in several tumor cell lines and tumor xenograft models. In our previous study, structural modification by chemical synthesis has been carried out to develop novel honokiol derivatives to improve antitumor activity and clarify the structure–activity relationship. Honokiol analogs, especially 3,5′-diformylated honokiol HK-(CHO)2, have been found to moderately block the newly grown segmental vessels from the dorsal aorta in the transgenic zebrafish-based assay, show antiangiogenic property, and exert medium cytotoxicity against two lung cell lines (Lewis lung carcinoma LL/2 cells and human non-small-cell lung cancer A549 cells). However, the in-vitro and in-vivo antitumor effects of formylated honokiol derivatives against lung carcinoma remained poorly understood. In the study, two formylated honokiol derivatives also showed potent antitumor effects in the Lewis lung carcinoma cells, K-rasG12D-dirived lung adenocarcinoma mice, and a mouse lung tumor xenograft model, with HK-(CHO)2 being most efficacious. The potential mechanism was inhibiting cell proliferation and inducing apoptosis in lung cancer by the regulation of vascular endothelial growth factor A expression. These results further suggested that HK-(CHO)2 might be potential candidates for the treatment of lung carcinoma. Polish validation of the University of Washington “quality of life” questionnaire in patients with cancer of the larynx

Rehabilitation using immediate loading in patients with partial resection of the jaw

ABSTRACT: Ameloblastoma is a rare tumor that affects the maxillomandibular region. Surgical resection is often indicated, and oral rehabilitation becomes a challenge. This study aims to report on the mandibular rehabilitation with implant supported prosthesis using immediate loading with subsequent resection. A patient with a confirmed diagnosis of multicystic ameloblastoma in the left jaw underwent a partial resection of the mandible and a reconstruction with a titanium plate. After 2 years of follow-up with clinical examinations and imaging testing and with no signs of recurrence, the patient underwent the technique of immediate load implants, rehabilitating the mandible with an implant supported fixed prosthesis and the maxilla with a conventional complete denture. During the seven years of follow-up with the patient, there was no sign of recurrence of ameloblastoma. The rehabilitation with oral implants under immediate loading demonstrated to be successful, and the cemented cylinder technique used in this study coupled with passivity showed a favorable prognosis for the longevity of implants.

RESUMEN: El paciente fisurado labiopalatino muestra una falta de fusión de los procesos palatinos presentando una comunicación de la cavidad buconasal, anomalías de forma y/o la agenesia de los dientes cercanos a la fisura y un deficiente crecimiento sagital y transversal maxilar, esto es más complejo en la fisura bilateral, por ser el más severo de los subtipos de fisura. Se dificultan las funciones básicas como comer, hablar y relacionarse con los demás. Su tratamiento requiere del trabajo de un equipo transdisciplinario, coordinado y organizado, conformado por todos aquellos profesionales que colaboren en proporcionar un resultado integral. El objetivo de este artículo es exponer un caso clínico de rehabilitación protésica en conjunto con un tratamiento de ortodoncia en un paciente que presenta labio leporino bilateral y fisura palatina, además de una premaxila móvil, severamente extruida.


ABSTRACT: The cleft lip and palate patient is mainly characterised by the presence of an oronasal communication, malformation or agenesis of the teeth close to the cleft, and deficient sagittal and transverse growth of the maxilla. These patients require various treatments involving a multidisciplinary team, which may include a maxillofacial surgeon, an orthodontist, a speech therapist, a paediatrician, plastic surgeon, a general dentist, a prosthodontist, an ENT specialist, a psychologist, and all those professionals who can help provide functional, aesthetic and psychological improvement. Membrana de fibrina versus injerto conectivo como tratamiento de recesióngingival

RESUMEN: Este caso clínico de boca dividida a 6 meses tiene como objetivo comparar los resultados obtenidos con una membrana de fibrina rica en plaquetas y leucocitos (L-PRF) y un injerto de tejido conectivo (ITC) en el tratamiento de recesiones gingivales clase1 de Miller en un paciente con biotipo gingival grueso. El resultado muestra que el uso de una membrana de L-PRF provee un parcial cubrimiento de la recesión y un menor malestar subjetivo del paciente cuando se compara con el ITC. Sin embargo, el ITC proporciona un mayor porcentaje de cubrimiento radicular y un mejor resultado estético que la membrana de L-PRF al cabo de 6 meses.


ABSTRACT: The aim of this split-mouth clinical study, covering a 6 month period, is to compare the results obtained from Leucocyte and Platelet Rich Fibrin Membrane (L-PRF) and Connective Tissue Grafting (CTG) in the treatment of class 1 Miller gingival recessions in a patient with thick gingival biotype. The results show that the use of L-PRF membrane provides partial coverage of the recession, as well as lower subjective patient discomfort when compared with CTG. However, CTG provides a higher percentage of root coverage and better aesthetic results than the L-PRF membrane after a period of 6 months.

Biomateriales en levantamiento de seno maxilar para implantes dentales

RESUMEN: Objetivos El objetivo principal es describir las técnicas y condiciones en que se usan los diferentes biomateriales para los procedimientos de aumento del seno maxilar en implantes oseointegrados inmediatos o diferidos. Método Se realizó una búsqueda electrónica de textos completos desde el año 2010 hasta el 15 de agosto del 2014, y que respondieran la pregunta de investigación, utilizando diferentes buscadores y una manual en las revistas científicas de periodoncia. Se determinó el nivel de evidencia, calidad de reporte, sesgos de la literatura analizada y aspectos éticos. Resultados Se seleccionaron 17 estudios, 4 revisiones sistemáticas y 13 ensayos clínicos aleatorizados. Diez estudios comparaban los diferentes biomateriales disponibles entre sí, y no encontraron diferencias estadísticamente significativas en la nueva formación ósea. Un estudio examinaba la opción de no utilizar un biomaterial en el aumento de senos maxilares. Seis estudios indicaban que el uso de biomateriales adicionales no era significativo. Tres estudios concluyeron que no existían diferencias significativas entre uno o 2 tiempos quirúrgicos para realizar los procedimientos. Tres estudios comparaban la técnica quirúrgica de ventana lateral con la técnica indirecta, sin encontrar diferencias estadísticamente significativas, pero sí se asocia a un menor número de complicaciones a la técnica indirecta, especialmente al estar acompañada de la instalación de implantes cortos. Conclusiones No se describen diferencias entre los diferentes biomateriales utilizados en procedimientos de aumento de senos maxilares. El uso de biomateriales adicionales pareciera no ofrecer beneficios medibles al tratamiento. La técnica quirúrgica indirecta puede ser asociada a un menor número de complicaciones. Son necesarios más estudios de ensayos clínicos aleatorizados, con tamaños de muestra mayores, variables controladas y seguimientos a largo plazo para establecer conclusiones.


ABSTRACT: Objectives The main objective is to describe the techniques and conditions in which the various biomaterials are used for sinus augmentation procedures for immediate or delayed osseointegrated implants. Method An electronic search was conducted looking for papers published from 2010 to 15 August 2014 to answer the research question. Several search engines were used as well as a manual on scientific journals of periodontics. The level of evidence, quality reporting, biases of the analysed literature, and ethical aspects were determined. Results A total of 17 studies were selected, of which 4 were systematic reviews, and 13 were randomised trials. Ten different trials compared the available biomaterials with each other, and found no statistically significant differences in new bone formation. One study examined the option of not using biomaterials in the procedures. Six studies indicated that the use of additional biomaterials was not significant. Three studies concluded that there were no significant differences between 1 and 2 surgical times. Three studies compared the surgical technique of lateral window with the indirect technique, with no statistically significant differences, but the indirect technique was associated with fewer complications, especially when accompanied by the installation of short implants. Conclusions No differences between different biomaterials used in augmentation procedures for maxillary sinuses were described. The use of additional biomaterials does not seem to offer measurable benefits to the treatment. The indirect surgical technique may be associated with fewer complications. Further studies of randomised clinical trials, with larger sample sizes, controlled variables, and long term monitoring are needed to draw stronger conclusions.

The treatment of gummy smile: integrative review of literature

ABSTRACT: The use of periodontal surgery to treat the condition of gummy smile is an essentially aesthetic approach and plays an important role in raising the self-esteem of patients. Effective treatment requires identification of the best technique for the correction, and the long-term predictability of the procedure. Objective: The aim of this study was to conduct an integrative review of scientific evidence relating to the periodontal surgery techniques used to treat gummy smile. Method and Materials: According to the inclusion and exclusion criteria A total of five (05) articles from the PubMed, Scopus and Web of Science databases were analyzed. The majority of cases found featured women, and the average age was 26 years. In most cases, the predominant surgical technique was based on the use of gingivectomy with osteotomy, with a follow-up period of around 6 months. Conclusion: This review revealed a serious lack of controlled and randomized clinical studies into the use of periodontal surgery to treat gummy smiles and highlighted a need for longitudinal clinical studies, with greater evidence, about the best type of periodontal surgery for this purpose.

Láser Er:YAG en el tratamiento de la periimplantitis: revisión de la literatura

RESUMEN: La presente revisión bibliográfica describe la efectividad del uso del láser Er:YAG en los últimos 10 años, con el objetivo de esclarecer cuáles han sido sus efectos clínicos, sus efectos microbiológicos, sus efectos físicos y térmicos en la superficie del implante, su efecto en la biocompatibilidad y el tiempo de trabajo requerido. Se efectuó la búsqueda en la base de datos PubMed, seleccionando investigaciones publicadas entre los años 2005 y 2015. Se seleccionaron 14 investigaciones in vitro, un estudio en animales, un reporte de casos, 6 series de casos, 2 estudios clínicos controlados, 4 estudios clínicos controlados aleatorizados y 5 revisiones bibliográficas. La literatura disponible señala que el uso del láser Er:YAG en el tratamiento de la periimplantitis produce una mejoría clínica, expresada en una disminución del sangrado al sondaje, una disminución en la profundidad de sondaje y una ganancia de inserción que se limita a los primeros 6 meses luego del tratamiento. En relación con la descontaminación de la superficie del implante, la mayoría de los estudios reporta una disminución en el número de bacterias adheridas. Para no causar daños físicos y térmicos es necesario utilizar el láser Er:YAG en distintos niveles de energía, de frecuencia y de tiempo de exposición según el tipo de superficie del implante afectado. Respecto a la biocompatibilidad de la superfice existen aún resultados contradictorios. Las investigaciones coinciden en que el tratamiento con láser Er:YAG supone un menor tiempo de trabajo en comparación con terapias convencionales.


ABSTRACT: A literature review is presented on the effectiveness of the use of the erbium-doped yttrium aluminium garnet (Er:YAG) laser over the past 10 years, with the objective of clarifying its clinical, microbiological, physical, and thermal effects on the implant surface, as well as its effect on the biocompatibility and work time required. A search was conducted in PubMed database, selecting studies published between 2005 and 2015. A total of 14 in vitro studies were selected, including one animal study, one case report, six case series, two controlled clinical trials, four randomised controlled trials, and five literature reviews. The literature shows that the use of Er:YAG laser in the treatment of peri-implantitis leads to a clinical improvement, expressed as a decrease in bleeding on probing, a reduction in probing depth, and a clinical attachment gain, that is limited to the first six months after treatment. As regards decontamination of the implant surface, most studies report a decrease in the number of attached bacteria. To avoid physical and thermal damage, the Er:YAG laser must be used at different energy levels, frequency, and exposure times, according to the type of surface of the implant affected. There are still conflicting results as regards the biocompatibility of the surface. Studies agree that treatment with Er:YAG laser leads to less operating time compared to conventional therapies.

Eficacia de la Azitromicina asociada al raspado y alisado radicular en periodontitis crónica

RESUMEN: Objetivo: evaluar la eficacia de la azitromicina asociada al RAR (asociada al raspado y alisado radicular) en pacientes con periodontitis crónica. Material y método: El presente ensayo clínico paralelo aleatorizado, controlado y triple ciego, se desarrolló en la Clínica Estomatológica de la Universidad Católica Los Ángeles de Chimbote (Trujillo, Perú), entre marzo y diciembre de 2015. Participaron 30 pacientes adultos, distribuidos aleatoriamente en 2 grupos, designados con las letras A y B, de 15 integrantes cada uno. Cada paciente fue evaluado periodontalmente antes de realizarle el raspado y alisado radicular y optimización de las técnicas de higiene. A los pacientes del grupo A se les indicó tratamiento antibiótico con Azitromicina 500 mg por vía oral cada 24 horas durante 3 días y al grupo B se les indicó un placebo con las mismas características físicas del antibiótico escogido. Los pacientes fueron evaluados a los 3, 4 y 5 meses. La eficacia fue evaluada mediante el test T de Student para comparación de medias, considerando un nivel de significancia para los valores de p<0,05. Resultados: Se encontró que ambos grupos lograron mejorar la profundidad de sondaje y el nivel de inserción clínica sin embargo no presentaron diferencias estadísticamente significativas entre ellos. Conclusión: La azitromicina asociada al RAR no es eficaz como terapia coadyuvante en pacientes con periodontitis crónica.


ABSTRACT: Objective: To evaluate the efficacy of azithromycin associated with the RAR (Associated with scaling and root planing) in patients with chronic periodontitis. Material and Methods: This clinical trial parallel randomized, controlled, triple blind, was developed in the Stomatology Clinic of the Catholic University Los Angeles of Chimbote (Trujillo, Peru), between March and December 2015. The participants were 30 adult patients randomized 2 groups, designated by the letters A and B of 15 members each. Each patient was evaluated periodontally before Give you the scaling and root planing and optimization hygiene techniques. Patients in group A, were prescribed antibiotic treatment with azithromycin 500 mg orally every 24 hours for 3 days. While, group B were prescribed a placebo with the same physical characteristics of the antibiotic chosen. Patients were evaluated at 3, 4 and 5 months. The efficacy was evaluated using the Student t test to compare means, considering a level of significance for the values of p <0.05. Results: It was found that both groups were able to improve probing depth and clinical attachment level however no statistically significant differences between them. Conclusion: Azithromycin associated with the RAR is not effective as adjunctive therapy in patients with chronic periodontitis.

Influence of the Passive Evaporation of adhesive on the conductance of dentin

ABSTRACT: Aim: Determine the influence of time of passive evaporation of the solvent in a universal adhesive on the hydraulic conductance and permeability of dentin in an ex vivo human model. Henceforth, 60 healthy non-occluding third molars, indicated for therapeutic extraction/removal, of informed and consented patients aged between 18 and 30 years, were used here in. First, extracted teeth were incorporated into epoxy resin blocks and then dentin disks (1mm +/- 0.1 mm thick) were prepared. Dentin was acid etched with 35% ortho-phosphoric acid for 15 seconds in order to remove the smear layer and obtain permeable dentin. Samples were then randomized and divided into 5 groups (n=12). A Single Bond Universal adhesive layer with different time of passive evaporation of the solvent was then applied: GC=10 seconds, G1=30 seconds, G2=60 seconds, G3=300 seconds and G4=50 minutes. Finally, the flow rate was measured using a diffusion chamber, a model previously proposed by Pashley et al. Results: The obtained hydraulic conductance averages were as follows: GC=0.00052, G1=0.00018, G2=0.00006, G3=0.00005, G4=0.00005 expressed in uL•cm-2•min-1cm•H2O-1. For comparisons between groups, ANOVA and post hoc Tukey (ρ<0.05) tests were applied, resulting in a statistically significant difference between the GC group and all experimental groups (ρ <0.05). An influence of solvent passive evaporation thereby reducing hydraulic conductance in the experimental groups, was detected.

Relación entre frenillos labiales y periodonto en una población peruana

RESUMEN: Los frenillos aberrantes están incluidos dentro de las deformidades mucogingivales, y estas pueden alterar la función y la estética del periodonto de protección. Este estudio tuvo como objetivo evaluar la inserción de los frenillos labiales y establecer una relación con la pérdida papilar y recesión gingival a nivel de los dientes anterosuperiores y anteroinferiores. Materiales y métodos Estudio transversal que evaluó a 97 pacientes sistémicamente sanos en los cuales se evaluó el tipo de frenillo (TF), la altura del frenillo (AF), el tipo de recesión gingival y la pérdida papilar al nivel de los sectores anteriores maxilar y mandibular. Resultados El TF más prevalente fue el mucoso, con un 54,6% y 93,8% de prevalencia a nivel maxilar y de la mandíbula respectivamente; la AF promedio fue de 8,37mm ± 2,671mm para el frenillo labial maxilar y 8,33mm ± 1,657mm para el frenillo labial mandibular. No se encontraron relaciones entre el TF, la AF con la presencia y severidad de pérdida papilar y recesión gingival al nivel de los incisivos centrales (p>0,05). Conclusión El frenillo más común de la población estudiada fue el frenillo mucoso, no existiendo una relación directa entre su clasificación e inserción con la presencia de una recesión gingival o pérdida papilar.


ABSTRACT: Aberrant frenums are included within the mucogingival deformities, which can alter the function and aesthetic periodontal protection. This study aimed to evaluate the adherence of labial frenulum and establish a relationship with papillary loss and gingival recession of the anterior teeth in maxilla and in the jaw. Materials and methods A cross-sectional study was conducted on 97 systemically healthy patients in whom the type of frenulum (TF), the height of the frenulum (AF), the type of gingival recession, and papillary loss was evaluated in the upper maxillary and mandibular sectors. Results The most prevalent frenum was the mucosal. The prevalence of TF was 54.6% and 93.8% of maxilla and mandible, respectively. The mean AF was 8.37mm ± 2.671mm for the labial frenulum in maxillary, and 8.33mm ± 1.657mm for frenulum. No relationship was found between the TF, AF with the presence and severity of papillary loss and gingival recession (P>.05). Conclusion The most common frenulum of the study population was the mucosal frenulum. No erupción de segundos molares mandibulares permanentes en radiografía panorámica.

RESUMEN: La no erupción de segundos molares es relativamente baja, pero ha ido en aumento en los últimos años. Se relaciona con un tratamiento multidisciplinario largo, costoso y complejo, por lo cual es de suma importancia enfocarse en el diagnóstico precoz y tratamiento temprano. El objetivo de este estudio es poder hacer una comparación de medidas radiográficas entre un grupo de pacientes con segundo molar mandibular permanente no erupcionado o sin posibilidades de erupcionar y un grupo control. Método: Se realizó una revisión manual consecutiva de las 16.835 fichas sobre las cuales se aplicaron criterios de inclusión y exclusión. Los datos se obtuvieron de radiografías panorámicas de las cuales se obtuvieron medidas lineales y angulares que fueron comparados entre dos grupos de estudio. Se desarrolló un modelo predictivo mediante análisis discriminante. Resultados: El 1,5% de los pacientes tuvieron segundos molares no erupcionados. Los promedios de las variables relevantes fueron: Ángulo del segundo molar 33,7°; distancia desde distal del primer molar al borde anterior de la rama mandibular 15mm; invasión distal del primer molar por el segundo molar 2,1mm; inclinación mesial de segundo molar con respecto al plano oclusal 128° y al plano mandibular 63°. El puntaje crítico discriminante fue de 36,7. Conclusión: La presencia de segundos molares incluidos es baja. Se encontró diferencias estadísticamente significativas entre casos y controles en todas las variables. El modelo predictivo puede identificar molares sin probabilidad de erupción natural. Es necesario contar con nuevos estudios con muestras poblacionales para establecer cuáles serían los valores de normalidad para poder diagnosticar en forma precoz esta condición.


ABSTRACT: No eruption of seconds is relatively low, has been increasing in recent years. It relates to long, costly and complex multidisciplinary treatment, so it is important to focus on early diagnosis and early treatment. The aim of this study is to make a comparison of radiographic measurements among a group of patients with mandibular second permanent molar eruption and no possibility of a control group. Method: A consecutive manual review of 16,835 sheets on which inclusion and exclusion criteria were applied. The data were obtained from panoramic radiographs of which linear and angular measurements were compared between two study groups were obtained. A predictive model was developed using discriminant analysis. Results: 1.5% of patients had unerupted second molars. The averages of the relevant variables were: Angle of the second molar 33.7 °; distance from distal of the first molar to the anterior border of the ramus 15 mm; invasion distal of the first molar the second molar 2.1mm; second molar mesial inclination relative to the occlusal plane 128 ° and 63 ° mandibular plane. The critical discriminant score was 36.7. Conclusion: Statistically significant differences between cases and controls was found in all variables. The predictive model can identify likely molars with or without natural eruption. It is necessary to have further studies with population samples to establish what would be the normal values to diagnose this condition at an early stage.

Mediación en odontología y Garantías Explícitas en Salud en Chile 2005-2014: las diferencias entre el mundo privado y el público

RESUMEN: Antecedentes: En Chile desde 2005 se implementó la Ley Auge que establece como derecho de los usuarios, beneficiarios tanto del sistema público o privado de salud, cuatro Garantías Explícitas de Salud denominadas GES, entre las que se incluye la garantía de calidad y dentro de ella se establece la mediación como un procedimiento prejudicial, obligatorio, tendiente a evitar la judicialización de los casos de responsabilidad sanitaria. Este procedimiento contempla dos instancias de mediación según el prestador sea público o privado. Objetivo: describir las mediaciones por atenciones odontológicas en ambos subsistemas. Métodos: estudio trasversal descriptivo sobre las mediaciones efectuadas desde la implementación de este procedimiento (año 2005) hasta el año 2014, por prestaciones odontológicas, otorgadas tanto por prestadores públicos como privados. Resultados: En el periodo de estudio, se efectuaron 519 mediaciones en el ámbito odontológico. La mayoría (60,4%) de las reclamantes en mediaciones públicas son mujeres. Las especialidades o prestaciones odontológicas más comprometidas, información no disponible para las mediaciones públicas, son por atenciones de endodoncia (25%), implantología (16%) y prótesis (15%). El 58% de las mediaciones privadas y sólo el 34% de las públicas finalizan con acuerdo total entre las partes, existiendo dificultades para comparar ambos subsistemas, por la amplia variación en la recolección de datos. Entre las mediaciones públicas, un alto porcentaje de los reclamantes (70%) solo solicita disculpas y/o atención de salud y no busca retribuciones económicas. Conclusión: las reclamantes en las mediaciones en odontología son mayoritariamente mujeres, las especialidades odontológicas más comprometidas son endodoncia, implantes y prótesis, y la mayoría solicita sólo disculpas/explicaciones de los hechos y prestaciones asistenciales. Los registros de las mediaciones públicas y privadas contienen información disímil y parcial, por lo que se no se puede concluir sobre sus diferencias.


ABSTRACT: Background: In Chile since 2005 the AUGE Act establishes the rights of beneficiaries of both the public and private health systems four Explicit Health Guarantees called GES, among them quality assurance. The later included mediation implemented and established as a preliminary procedure, mandatory, intended to avoid prosecution of cases of medical malpractice. This procedure involves two instances of mediation depending on public or private providers. Objective: To describe the mediations arising from dental care in Chile. Methods: A descriptive cross-sectional study on mediatons since the implementation of this procedure (2005) until 2014, by dental benefits granted by both public and private providers. Results: During the study period, 519 mediations were performed in the dental field. The majority (60.4%) of complaints in public context are women. Specialties or more committed dental services, data available only for private mediations, are endodontics (25%), implantology (16%) and prosthetics (15%). 58% of private and only 34% of public mediatons end up with full agreement between the parties, with difficulties to compare the two systems due to the wide variation in data collection. Among public averages, a high percentage of claimants (70%) only request explanations/apologize and/or health benefits and do not economic compensation. Conclusion: claimants are mostly women, the most compromised dental specialties are endodontics, implants and prostheses, and most patients request only apologies/explanations about the facts and provided dental healthcare. The public and private mediations are not entirely comparable since data sources are dissimilar and contain partial information, so it is not possible to conclude about their differences.

Relación entre distancia intercantal y ancho de dientes anterosuperiores en jóvenes chilenos

RESUMEN: Objetivo: La distancia intercantal (DIC) es utilizado para la selección de los dientes anterosuperiores y es la dimensión más estable en el tiempo Sin embargo, es variable entre razas y poblaciones, particularmente en Sudamérica. El objetivo de este estudio es establecer la correlación entre DIC y el ancho de los dientes maxilares anteriores en una población adulta jóven del sur de Chile. Materiales y métodos: Estudio de corte transversal. Participaron 111 estudiantes voluntarios (promedio edad 22.4 años; 65.7% hombres) de la ciudad de Valdivia, sur de Chile. Se midió DIC y el ancho mesiodistal del incisivo central superior, la distancia lineal de los cuatro incisivos superiores y de los seis dientes anterosuperiores en modelos de estudio. Se analizó la diferencia de las mediciones según sexo (test de Student; p<0.05) y la correlación lineal entre DIC y las medidas dentales (r=0.8; p<0.05. STATA v.10.0) Resultados: Todas las mediciones fueron significativamente mayores en hombres que en mujeres (p<0.05). Se observó una correlación estadísticamente significativa entre la DIC y la distancia lineal de los cuatro incisivos superiores (p=0.04) y de los seis dientes anterosuperiores (p=0.03). Se obtuvo una razón de 1:0.94 y 1:1.28 de DIC con la distancia lineal de los cuatro incisivos superiores y los seis dientes anterosuperiores, respectivamente. Conclusión: La DIC presentó una correlación lineal y proporcional con los dientes anteriores, pudiendo estimar el ancho lineal de los dientes anterosuperiores en una población adulta joven del Sur de Chile.


ABSTRACT: Objective: Intercanthal distance (ICD) is a parameter used to select the upper anterior teeth and the most stable measurement overtime. However, it is variable between race and populations, particularly in South America. The aim of this study was to establish the correlation between ICD and the width of maxillary anterior teeth in young adult population from southern of Chile. Materials and Methods: A Cross-sectional study was designed. One Hundred eleven volunteers students (average age 22.4 years; 65.7% men) from Valdivia city (southern of Chile) were included. ICD and mesiodistal width of maxillary central incisor, linear distance of the four upper incisors and linear distance of the six upper front teeth in plaster study models weremeasured. The mean of difference measurement between sex (ttest; p<0.05) and the linear correlation between DIC and the width of the upper anterior were analyzed (r=0.8; p<0.05; STATA v.10.0). Results: All measurement were significantly higher in male than female (p<0.05). A statistically significant correlation between ICD and distance of the four upper incisors (p=0.04) and the six upper front teeth (p=0.03) were observed. A 1:0.94 and 1:1.28 ratios between ICD with the linear distance of the upper incisors and the six maxillary anterior were obtained respectively. Conclusion: ICD presented a linear and proportional correlation with maxillary anterior teeth. With these results it is possible to estimate the linear width of the upper front teeth in young adult population from southern of Chile.

Cirugía y traumatología bucomáxilofacial en la red hospitalaria chilena

RESUMEN: Argumentando la existencia de listas de espera de cirugía bucal y máxilofacial, los Ministerios de Salud y Educación chilenos deciden crear un profesional de menores competencias que el cirujano bucal y máxilofacial existente, cuya formación dure un año y se desempeñe en la atención primaria de salud. Decretan la especialidad de Cirugía Bucal, comprometiendo la calidad y seguridad de la atención, exponiendo a los enfermos a eventos adversos de impredecibles desenlaces, afectando la calidad de vida de éstos o provocando eventualmente un desenlace fatal. El objetivo del presente estudio es establecer el número de cirujanos bucales y máxilofaciales formados en el país, conocer cuantos se desempeñan en hospitales públicos y su distribución en los servicios de salud, así como también evaluar si se cumplen las condiciones de infraestructura necesarias para desarrollar la especialidad según la normativa vigente. Consideramos que éstos son datos fundamentales al momento de modificar políticas públicas. La metodología propuesta es un estudio descriptivo de corte transversal, realizando una encuesta dirigida a los jefes de los servicios odontológicos de la red hospitalaria pública del país, destinada a obtener información del prestador individual e institucional en cada centro de la red. Se concluye que existe una cantidad suficiente de especialistas formados y en vías de formación para atender las necesidades de la población. La evidencia muestra que faltan cargos e infraestructura, por lo que la medida adoptada por el MINSAL respecto a la creación de la especialidad de Cirugía Bucal obedece a un error diagnóstico desde el punto de vista del prestador individual e institucional, originado por la ausencia de un estudio técnico que evalúe la red hospitalaria del país.


ABSTRACT: Arguing the existence of waiting lists in oral and maxillofacial surgery, the chilean Ministries of Health and Education decided to create a professional less skilled than the existing oral and maxilofacial surgeon, whose training lasts one year and works in primary care health. They established the specialty of Oral Surgery, compromising the quality and safety of care, exposing patients to adverse events with unpredictable outcomes, affecting their quality of life or eventually causing a fatal outcome. At the time of this study there is no record of the number of oral and maxillofacial surgeons trained in our country, it is unknown how many are working in public hospitals and their distribution in health services, as well as whether there is the necessary infrastructure to develop the specialty under current regulations. We made a descriptive transversal study, conducting an interview aimed to obtain information from individual and institutional providers to know the reality of the specialty in the public health network. It is concluded that there is a sufficient number of specialists trained and in training to meet the needs of the population. The evidence shows missing possitions and infrastructure, so that the action taken by the Health Ministry regarding the creation of the specialty of Oral Surgery is due to a misdiagnosis from the point of view of an individual and institutional provider, caused by the absence of a technical study to assess the country's hospital network.

Rehabilitation using immediate loading in patients with partial resection of the jaw

ABSTRACT: Ameloblastoma is a rare tumor that affects the maxillomandibular region. Surgical resection is often indicated, and oral rehabilitation becomes a challenge. This study aims to report on the mandibular rehabilitation with implant supported prosthesis using immediate loading with subsequent resection. A patient with a confirmed diagnosis of multicystic ameloblastoma in the left jaw underwent a partial resection of the mandible and a reconstruction with a titanium plate. After 2 years of follow-up with clinical examinations and imaging testing and with no signs of recurrence, the patient underwent the technique of immediate load implants, rehabilitating the mandible with an implant supported fixed prosthesis and the maxilla with a conventional complete denture. During the seven years of follow-up with the patient, there was no sign of recurrence of ameloblastoma. The rehabilitation with oral implants under immediate loading demonstrated to be successful, and the cemented cylinder technique used in this study coupled with passivity showed a favorable prognosis for the longevity of implants.

Prótesis y ortodoncia. Rehabilitación de fisurado bilateral. Caso clínico

RESUMEN: El paciente fisurado labiopalatino muestra una falta de fusión de los procesos palatinos presentando una comunicación de la cavidad buconasal, anomalías de forma y/o la agenesia de los dientes cercanos a la fisura y un deficiente crecimiento sagital y transversal maxilar, esto es más complejo en la fisura bilateral, por ser el más severo de los subtipos de fisura. Se dificultan las funciones básicas como comer, hablar y relacionarse con los demás. Su tratamiento requiere del trabajo de un equipo transdisciplinario, coordinado y organizado, conformado por todos aquellos profesionales que colaboren en proporcionar un resultado integral. El objetivo de este artículo es exponer un caso clínico de rehabilitación protésica en conjunto con un tratamiento de ortodoncia en un paciente que presenta labio leporino bilateral y fisura palatina, además de una premaxila móvil, severamente extruida.


ABSTRACT: The cleft lip and palate patient is mainly characterised by the presence of an oronasal communication, malformation or agenesis of the teeth close to the cleft, and deficient sagittal and transverse growth of the maxilla. These patients require various treatments involving a multidisciplinary team, which may include a maxillofacial surgeon, an orthodontist, a speech therapist, a paediatrician, plastic surgeon, a general dentist, a prosthodontist, an ENT specialist, a psychologist, and all those professionals who can help provide functional, aesthetic and psychological improvement. This report describes a case of prosthetic rehabilitation in a patient with cleft lip and palate without alveolar bone in the cleft area, which prevented rehabilitation with an osseointegrated implant, in addition to a mobile and severely extruded pre-maxilla, which required an orthopaedic repositioning.

Membrana de fibrina versus injerto conectivo como tratamiento de recesióngingival

RESUMEN: Este caso clínico de boca dividida a 6 meses tiene como objetivo comparar los resultados obtenidos con una membrana de fibrina rica en plaquetas y leucocitos (L-PRF) y un injerto de tejido conectivo (ITC) en el tratamiento de recesiones gingivales clase1 de Miller en un paciente con biotipo gingival grueso. El resultado muestra que el uso de una membrana de L-PRF provee un parcial cubrimiento de la recesión y un menor malestar subjetivo del paciente cuando se compara con el ITC. Sin embargo, el ITC proporciona un mayor porcentaje de cubrimiento radicular y un mejor resultado estético que la membrana de L-PRF al cabo de 6 meses.


ABSTRACT: The aim of this split-mouth clinical study, covering a 6 month period, is to compare the results obtained from Leucocyte and Platelet Rich Fibrin Membrane (L-PRF) and Connective Tissue Grafting (CTG) in the treatment of class 1 Miller gingival recessions in a patient with thick gingival biotype. The results show that the use of L-PRF membrane provides partial coverage of the recession, as well as lower subjective patient discomfort when compared with CTG. However, CTG provides a higher percentage of root coverage and better aesthetic results than the L-PRF membrane after a period of 6 months.

Biomateriales en levantamiento de seno maxilar para implantes dentales

RESUMEN: Objetivos El objetivo principal es describir las técnicas y condiciones en que se usan los diferentes biomateriales para los procedimientos de aumento del seno maxilar en implantes oseointegrados inmediatos o diferidos. Método Se realizó una búsqueda electrónica de textos completos desde el año 2010 hasta el 15 de agosto del 2014, y que respondieran la pregunta de investigación, utilizando diferentes buscadores y una manual en las revistas científicas de periodoncia. Se determinó el nivel de evidencia, calidad de reporte, sesgos de la literatura analizada y aspectos éticos. Resultados Se seleccionaron 17 estudios, 4 revisiones sistemáticas y 13 ensayos clínicos aleatorizados. Diez estudios comparaban los diferentes biomateriales disponibles entre sí, y no encontraron diferencias estadísticamente significativas en la nueva formación ósea. Un estudio examinaba la opción de no utilizar un biomaterial en el aumento de senos maxilares. Seis estudios indicaban que el uso de biomateriales adicionales no era significativo. Tres estudios concluyeron que no existían diferencias significativas entre uno o 2 tiempos quirúrgicos para realizar los procedimientos. Tres estudios comparaban la técnica quirúrgica de ventana lateral con la técnica indirecta, sin encontrar diferencias estadísticamente significativas, pero sí se asocia a un menor número de complicaciones a la técnica indirecta, especialmente al estar acompañada de la instalación de implantes cortos. Conclusiones No se describen diferencias entre los diferentes biomateriales utilizados en procedimientos de aumento de senos maxilares. El uso de biomateriales adicionales pareciera no ofrecer beneficios medibles al tratamiento. La técnica quirúrgica indirecta puede ser asociada a un menor número de complicaciones. Son necesarios más estudios de ensayos clínicos aleatorizados, con tamaños de muestra mayores, variables controladas y seguimientos a largo plazo para establecer conclusiones.


ABSTRACT: Objectives The main objective is to describe the techniques and conditions in which the various biomaterials are used for sinus augmentation procedures for immediate or delayed osseointegrated implants. Method An electronic search was conducted looking for papers published from 2010 to 15 August 2014 to answer the research question. Several search engines were used as well as a manual on scientific journals of periodontics. The level of evidence, quality reporting, biases of the analysed literature, and ethical aspects were determined. Results A total of 17 studies were selected, of which 4 were systematic reviews, and 13 were randomised trials. Ten different trials compared the available biomaterials with each other, and found no statistically significant differences in new bone formation. One study examined the option of not using biomaterials in the procedures. Six studies indicated that the use of additional biomaterials was not significant. Three studies concluded that there were no significant differences between 1 and 2 surgical times. Three studies compared the surgical technique of lateral window with the indirect technique, with no statistically significant differences, but the indirect technique was associated with fewer complications, especially when accompanied by the installation of short implants. Conclusions No differences between different biomaterials used in augmentation procedures for maxillary sinuses were described. The use of additional biomaterials does not seem to offer measurable benefits to the treatment. The indirect surgical technique may be associated with fewer complications. Further studies of randomised clinical trials, with larger sample sizes, controlled variables, and long term monitoring are needed to draw stronger conclusions.

The treatment of gummy smile: integrative review of literature

ABSTRACT: The use of periodontal surgery to treat the condition of gummy smile is an essentially aesthetic approach and plays an important role in raising the self-esteem of patients. Effective treatment requires identification of the best technique for the correction, and the long-term predictability of the procedure. Objective: The aim of this study was to conduct an integrative review of scientific evidence relating to the periodontal surgery techniques used to treat gummy smile. Method and Materials: According to the inclusion and exclusion criteria A total of five (05) articles from the PubMed, Scopus and Web of Science databases were analyzed. The majority of cases found featured women, and the average age was 26 years. In most cases, the predominant surgical technique was based on the use of gingivectomy with osteotomy, with a follow-up period of around 6 months. Conclusion: This review revealed a serious lack of controlled and randomized clinical studies into the use of periodontal surgery to treat gummy smiles and highlighted a need for longitudinal clinical studies, with greater evidence, about the best type of periodontal surgery for this purpose.

Láser Er:YAG en el tratamiento de la periimplantitis: revisión de la literatura

RESUMEN: La presente revisión bibliográfica describe la efectividad del uso del láser Er:YAG en los últimos 10 años, con el objetivo de esclarecer cuáles han sido sus efectos clínicos, sus efectos microbiológicos, sus efectos físicos y térmicos en la superficie del implante, su efecto en la biocompatibilidad y el tiempo de trabajo requerido. Se efectuó la búsqueda en la base de datos PubMed, seleccionando investigaciones publicadas entre los años 2005 y 2015. Se seleccionaron 14 investigaciones in vitro, un estudio en animales, un reporte de casos, 6 series de casos, 2 estudios clínicos controlados, 4 estudios clínicos controlados aleatorizados y 5 revisiones bibliográficas. La literatura disponible señala que el uso del láser Er:YAG en el tratamiento de la periimplantitis produce una mejoría clínica, expresada en una disminución del sangrado al sondaje, una disminución en la profundidad de sondaje y una ganancia de inserción que se limita a los primeros 6 meses luego del tratamiento. En relación con la descontaminación de la superficie del implante, la mayoría de los estudios reporta una disminución en el número de bacterias adheridas. Para no causar daños físicos y térmicos es necesario utilizar el láser Er:YAG en distintos niveles de energía, de frecuencia y de tiempo de exposición según el tipo de superficie del implante afectado. Respecto a la biocompatibilidad de la superfice existen aún resultados contradictorios. Las investigaciones coinciden en que el tratamiento con láser Er:YAG supone un menor tiempo de trabajo en comparación con terapias convencionales.


ABSTRACT: A literature review is presented on the effectiveness of the use of the erbium-doped yttrium aluminium garnet (Er:YAG) laser over the past 10 years, with the objective of clarifying its clinical, microbiological, physical, and thermal effects on the implant surface, as well as its effect on the biocompatibility and work time required. A search was conducted in PubMed database, selecting studies published between 2005 and 2015. A total of 14 in vitro studies were selected, including one animal study, one case report, six case series, two controlled clinical trials, four randomised controlled trials, and five literature reviews. The literature shows that the use of Er:YAG laser in the treatment of peri-implantitis leads to a clinical improvement, expressed as a decrease in bleeding on probing, a reduction in probing depth, and a clinical attachment gain, that is limited to the first six months after treatment. As regards decontamination of the implant surface, most studies report a decrease in the number of attached bacteria. To avoid physical and thermal damage, the Er:YAG laser must be used at different energy levels, frequency, and exposure times, according to the type of surface of the implant affected. There are still conflicting results as regards the biocompatibility of the surface. Studies agree that treatment with Er:YAG laser leads to less operating time compared to conventional therapies.

Eficacia de la Azitromicina asociada al raspado y alisado radicular en periodontitis crónica

RESUMEN: Objetivo: evaluar la eficacia de la azitromicina asociada al RAR (asociada al raspado y alisado radicular) en pacientes con periodontitis crónica. Material y método: El presente ensayo clínico paralelo aleatorizado, controlado y triple ciego, se desarrolló en la Clínica Estomatológica de la Universidad Católica Los Ángeles de Chimbote (Trujillo, Perú), entre marzo y diciembre de 2015. Participaron 30 pacientes adultos, distribuidos aleatoriamente en 2 grupos, designados con las letras A y B, de 15 integrantes cada uno. Cada paciente fue evaluado periodontalmente antes de realizarle el raspado y alisado radicular y optimización de las técnicas de higiene. A los pacientes del grupo A se les indicó tratamiento antibiótico con Azitromicina 500 mg por vía oral cada 24 horas durante 3 días y al grupo B se les indicó un placebo con las mismas características físicas del antibiótico escogido. Los pacientes fueron evaluados a los 3, 4 y 5 meses. La eficacia fue evaluada mediante el test T de Student para comparación de medias, considerando un nivel de significancia para los valores de p<0,05. Resultados: Se encontró que ambos grupos lograron mejorar la profundidad de sondaje y el nivel de inserción clínica sin embargo no presentaron diferencias estadísticamente significativas entre ellos. Conclusión: La azitromicina asociada al RAR no es eficaz como terapia coadyuvante en pacientes con periodontitis crónica.


ABSTRACT: Objective: To evaluate the efficacy of azithromycin associated with the RAR (Associated with scaling and root planing) in patients with chronic periodontitis. Material and Methods: This clinical trial parallel randomized, controlled, triple blind, was developed in the Stomatology Clinic of the Catholic University Los Angeles of Chimbote (Trujillo, Peru), between March and December 2015. The participants were 30 adult patients randomized 2 groups, designated by the letters A and B of 15 members each. Each patient was evaluated periodontally before Give you the scaling and root planing and optimization hygiene techniques. Patients in group A, were prescribed antibiotic treatment with azithromycin 500 mg orally every 24 hours for 3 days. While, group B were prescribed a placebo with the same physical characteristics of the antibiotic chosen. Patients were evaluated at 3, 4 and 5 months. The efficacy was evaluated using the Student t test to compare means, considering a level of significance for the values of p <0.05. Results: It was found that both groups were able to improve probing depth and clinical attachment level however no statistically significant differences between them. Conclusion: Azithromycin associated with the RAR is not effective as adjunctive therapy in patients with chronic periodontitis.

Influence of the Passive Evaporation of adhesive on the conductance of dentin

ABSTRACT: Aim: Determine the influence of time of passive evaporation of the solvent in a universal adhesive on the hydraulic conductance and permeability of dentin in an ex vivo human model. Henceforth, 60 healthy non-occluding third molars, indicated for therapeutic extraction/removal, of informed and consented patients aged between 18 and 30 years, were used here in. First, extracted teeth were incorporated into epoxy resin blocks and then dentin disks (1mm +/- 0.1 mm thick) were prepared. Dentin was acid etched with 35% ortho-phosphoric acid for 15 seconds in order to remove the smear layer and obtain permeable dentin. Samples were then randomized and divided into 5 groups (n=12). A Single Bond Universal adhesive layer with different time of passive evaporation of the solvent was then applied: GC=10 seconds, G1=30 seconds, G2=60 seconds, G3=300 seconds and G4=50 minutes. Finally, the flow rate was measured using a diffusion chamber, a model previously proposed by Pashley et al. Results: The obtained hydraulic conductance averages were as follows: GC=0.00052, G1=0.00018, G2=0.00006, G3=0.00005, G4=0.00005 expressed in uL•cm-2•min-1cm•H2O-1. For comparisons between groups, ANOVA and post hoc Tukey (ρ<0.05) tests were applied, resulting in a statistically significant difference between the GC group and all experimental groups (ρ <0.05). Relación entre frenillos labiales y periodonto en una población peruana

RESUMEN: Los frenillos aberrantes están incluidos dentro de las deformidades mucogingivales, y estas pueden alterar la función y la estética del periodonto de protección. Este estudio tuvo como objetivo evaluar la inserción de los frenillos labiales y establecer una relación con la pérdida papilar y recesión gingival a nivel de los dientes anterosuperiores y anteroinferiores. Materiales y métodos Estudio transversal que evaluó a 97 pacientes sistémicamente sanos en los cuales se evaluó el tipo de frenillo (TF), la altura del frenillo (AF), el tipo de recesión gingival y la pérdida papilar al nivel de los sectores anteriores maxilar y mandibular. Resultados El TF más prevalente fue el mucoso, con un 54,6% y 93,8% de prevalencia a nivel maxilar y de la mandíbula respectivamente; la AF promedio fue de 8,37mm ± 2,671mm para el frenillo labial maxilar y 8,33mm ± 1,657mm para el frenillo labial mandibular. No se encontraron relaciones entre el TF, la AF con la presencia y severidad de pérdida papilar y recesión gingival al nivel de los incisivos centrales (p>0,05). Conclusión El frenillo más común de la población estudiada fue el frenillo mucoso, no existiendo una relación directa entre su clasificación e inserción con la presencia de una recesión gingival o pérdida papilar.


ABSTRACT: Aberrant frenums are included within the mucogingival deformities, which can alter the function and aesthetic periodontal protection. This study aimed to evaluate the adherence of labial frenulum and establish a relationship with papillary loss and gingival recession of the anterior teeth in maxilla and in the jaw. Materials and methods A cross-sectional study was conducted on 97 systemically healthy patients in whom the type of frenulum (TF), the height of the frenulum (AF), the type of gingival recession, and papillary loss was evaluated in the upper maxillary and mandibular sectors. Results The most prevalent frenum was the mucosal. The prevalence of TF was 54.6% and 93.8% of maxilla and mandible, respectively. The mean AF was 8.37mm ± 2.671mm for the labial frenulum in maxillary, and 8.33mm ± 1.657mm for frenulum. No relationship was found between the TF, AF with the presence and severity of papillary loss and gingival recession (P>.05). Conclusion The most common frenulum of the study population was the mucosal frenulum. There was no direct relationship between the classification and adherence of the frenulum with the presence of gingival recession or papillary loss.

No erupción de segundos molares mandibulares permanentes en radiografía panorámica.

RESUMEN: La no erupción de segundos molares es relativamente baja, pero ha ido en aumento en los últimos años. Se relaciona con un tratamiento multidisciplinario largo, costoso y complejo, por lo cual es de suma importancia enfocarse en el diagnóstico precoz y tratamiento temprano. El objetivo de este estudio es poder hacer una comparación de medidas radiográficas entre un grupo de pacientes con segundo molar mandibular permanente no erupcionado o sin posibilidades de erupcionar y un grupo control. Método: Se realizó una revisión manual consecutiva de las 16.835 fichas sobre las cuales se aplicaron criterios de inclusión y exclusión. Los datos se obtuvieron de radiografías panorámicas de las cuales se obtuvieron medidas lineales y angulares que fueron comparados entre dos grupos de estudio. Se desarrolló un modelo predictivo mediante análisis discriminante. Resultados: El 1,5% de los pacientes tuvieron segundos molares no erupcionados. Los promedios de las variables relevantes fueron: Ángulo del segundo molar 33,7°; distancia desde distal del primer molar al borde anterior de la rama mandibular 15mm; invasión distal del primer molar por el segundo molar 2,1mm; inclinación mesial de segundo molar con respecto al plano oclusal 128° y al plano mandibular 63°. El puntaje crítico discriminante fue de 36,7. Conclusión: La presencia de segundos molares incluidos es baja. Se encontró diferencias estadísticamente significativas entre casos y controles en todas las variables. El modelo predictivo puede identificar molares sin probabilidad de erupción natural. Es necesario contar con nuevos estudios con muestras poblacionales para establecer cuáles serían los valores de normalidad para poder diagnosticar en forma precoz esta condición.


ABSTRACT: No eruption of seconds is relatively low, has been increasing in recent years. It relates to long, costly and complex multidisciplinary treatment, so it is important to focus on early diagnosis and early treatment. The aim of this study is to make a comparison of radiographic measurements among a group of patients with mandibular second permanent molar eruption and no possibility of a control group. Method: A consecutive manual review of 16,835 sheets on which inclusion and exclusion criteria were applied. The data were obtained from panoramic radiographs of which linear and angular measurements were compared between two study groups were obtained. A predictive model was developed using discriminant analysis. Results: 1.5% of patients had unerupted second molars. The averages of the relevant variables were: Angle of the second molar 33.7 °; distance from distal of the first molar to the anterior border of the ramus 15 mm; invasion distal of the first molar the second molar 2.1mm; second molar mesial inclination relative to the occlusal plane 128 ° and 63 ° mandibular plane. The critical discriminant score was 36.7. Conclusion: Statistically significant differences between cases and controls was found in all variables. The predictive model can identify likely molars with or without natural eruption. It is necessary to have further studies with population samples to establish what would be the normal values to diagnose this condition at an early stage.

Mediación en odontología y Garantías Explícitas en Salud en Chile 2005-2014: las diferencias entre el mundo privado y el público

RESUMEN: Antecedentes: En Chile desde 2005 se implementó la Ley Auge que establece como derecho de los usuarios, beneficiarios tanto del sistema público o privado de salud, cuatro Garantías Explícitas de Salud denominadas GES, entre las que se incluye la garantía de calidad y dentro de ella se establece la mediación como un procedimiento prejudicial, obligatorio, tendiente a evitar la judicialización de los casos de responsabilidad sanitaria. Este procedimiento contempla dos instancias de mediación según el prestador sea público o privado. Objetivo: describir las mediaciones por atenciones odontológicas en ambos subsistemas. Métodos: estudio trasversal descriptivo sobre las mediaciones efectuadas desde la implementación de este procedimiento (año 2005) hasta el año 2014, por prestaciones odontológicas, otorgadas tanto por prestadores públicos como privados. Resultados: En el periodo de estudio, se efectuaron 519 mediaciones en el ámbito odontológico. La mayoría (60,4%) de las reclamantes en mediaciones públicas son mujeres. Las especialidades o prestaciones odontológicas más comprometidas, información no disponible para las mediaciones públicas, son por atenciones de endodoncia (25%), implantología (16%) y prótesis (15%). El 58% de las mediaciones privadas y sólo el 34% de las públicas finalizan con acuerdo total entre las partes, existiendo dificultades para comparar ambos subsistemas, por la amplia variación en la recolección de datos. Entre las mediaciones públicas, un alto porcentaje de los reclamantes (70%) solo solicita disculpas y/o atención de salud y no busca retribuciones económicas. Conclusión: las reclamantes en las mediaciones en odontología son mayoritariamente mujeres, las especialidades odontológicas más comprometidas son endodoncia, implantes y prótesis, y la mayoría solicita sólo disculpas/explicaciones de los hechos y prestaciones asistenciales. Los registros de las mediaciones públicas y privadas contienen información disímil y parcial, por lo que se no se puede concluir sobre sus diferencias.


ABSTRACT: Background: In Chile since 2005 the AUGE Act establishes the rights of beneficiaries of both the public and private health systems four Explicit Health Guarantees called GES, among them quality assurance. The later included mediation implemented and established as a preliminary procedure, mandatory, intended to avoid prosecution of cases of medical malpractice. This procedure involves two instances of mediation depending on public or private providers. Objective: To describe the mediations arising from dental care in Chile. Methods: A descriptive cross-sectional study on mediatons since the implementation of this procedure (2005) until 2014, by dental benefits granted by both public and private providers. Results: During the study period, 519 mediations were performed in the dental field. The majority (60.4%) of complaints in public context are women. Specialties or more committed dental services, data available only for private mediations, are endodontics (25%), implantology (16%) and prosthetics (15%). 58% of private and only 34% of public mediatons end up with full agreement between the parties, with difficulties to compare the two systems due to the wide variation in data collection. Among public averages, a high percentage of claimants (70%) only request explanations/apologize and/or health benefits and do not economic compensation. Conclusion: claimants are mostly women, the most compromised dental specialties are endodontics, implants and prostheses, and most patients request only apologies/explanations about the facts and provided dental healthcare. The public and private mediations are not entirely comparable since data sources are dissimilar and contain partial information, so it is not possible to conclude about their differences.

Relación entre distancia intercantal y ancho de dientes anterosuperiores en jóvenes chilenos

RESUMEN: Objetivo: La distancia intercantal (DIC) es utilizado para la selección de los dientes anterosuperiores y es la dimensión más estable en el tiempo Sin embargo, es variable entre razas y poblaciones, particularmente en Sudamérica. El objetivo de este estudio es establecer la correlación entre DIC y el ancho de los dientes maxilares anteriores en una población adulta jóven del sur de Chile. Materiales y métodos: Estudio de corte transversal. Participaron 111 estudiantes voluntarios (promedio edad 22.4 años; 65.7% hombres) de la ciudad de Valdivia, sur de Chile. Se midió DIC y el ancho mesiodistal del incisivo central superior, la distancia lineal de los cuatro incisivos superiores y de los seis dientes anterosuperiores en modelos de estudio. Se analizó la diferencia de las mediciones según sexo (test de Student; p<0.05) y la correlación lineal entre DIC y las medidas dentales (r=0.8; p<0.05. STATA v.10.0) Resultados: Todas las mediciones fueron significativamente mayores en hombres que en mujeres (p<0.05). Se observó una correlación estadísticamente significativa entre la DIC y la distancia lineal de los cuatro incisivos superiores (p=0.04) y de los seis dientes anterosuperiores (p=0.03). Se obtuvo una razón de 1:0.94 y 1:1.28 de DIC con la distancia lineal de los cuatro incisivos superiores y los seis dientes anterosuperiores, respectivamente. Conclusión: La DIC presentó una correlación lineal y proporcional con los dientes anteriores, pudiendo estimar el ancho lineal de los dientes anterosuperiores en una población adulta joven del Sur de Chile.


ABSTRACT: Objective: Intercanthal distance (ICD) is a parameter used to select the upper anterior teeth and the most stable measurement overtime. However, it is variable between race and populations, particularly in South America. The aim of this study was to establish the correlation between ICD and the width of maxillary anterior teeth in young adult population from southern of Chile. Materials and Methods: A Cross-sectional study was designed. One Hundred eleven volunteers students (average age 22.4 years; 65.7% men) from Valdivia city (southern of Chile) were included. ICD and mesiodistal width of maxillary central incisor, linear distance of the four upper incisors and linear distance of the six upper front teeth in plaster study models weremeasured. The mean of difference measurement between sex (ttest; p<0.05) and the linear correlation between DIC and the width of the upper anterior were analyzed (r=0.8; p<0.05; STATA v.10.0). Results: All measurement were significantly higher in male than female (p<0.05). A statistically significant correlation between ICD and distance of the four upper incisors (p=0.04) and the six upper front teeth (p=0.03) were observed. A 1:0.94 and 1:1.28 ratios between ICD with the linear distance of the upper incisors and the six maxillary anterior were obtained respectively. Conclusion: ICD presented a linear and proportional correlation with maxillary anterior teeth. With these results it is possible to estimate the linear width of the upper front teeth in young adult population from southern of Chile.

Cirugía y traumatología bucomáxilofacial en la red hospitalaria chilena

RESUMEN: Argumentando la existencia de listas de espera de cirugía bucal y máxilofacial, los Ministerios de Salud y Educación chilenos deciden crear un profesional de menores competencias que el cirujano bucal y máxilofacial existente, cuya formación dure un año y se desempeñe en la atención primaria de salud. Decretan la especialidad de Cirugía Bucal, comprometiendo la calidad y seguridad de la atención, exponiendo a los enfermos a eventos adversos de impredecibles desenlaces, afectando la calidad de vida de éstos o provocando eventualmente un desenlace fatal. El objetivo del presente estudio es establecer el número de cirujanos bucales y máxilofaciales formados en el país, conocer cuantos se desempeñan en hospitales públicos y su distribución en los servicios de salud, así como también evaluar si se cumplen las condiciones de infraestructura necesarias para desarrollar la especialidad según la normativa vigente. Consideramos que éstos son datos fundamentales al momento de modificar políticas públicas. La metodología propuesta es un estudio descriptivo de corte transversal, realizando una encuesta dirigida a los jefes de los servicios odontológicos de la red hospitalaria pública del país, destinada a obtener información del prestador individual e institucional en cada centro de la red. Se concluye que existe una cantidad suficiente de especialistas formados y en vías de formación para atender las necesidades de la población. La evidencia muestra que faltan cargos e infraestructura, por lo que la medida adoptada por el MINSAL respecto a la creación de la especialidad de Cirugía Bucal obedece a un error diagnóstico desde el punto de vista del prestador individual e institucional, originado por la ausencia de un estudio técnico que evalúe la red hospitalaria del país.


ABSTRACT: Arguing the existence of waiting lists in oral and maxillofacial surgery, the chilean Ministries of Health and Education decided to create a professional less skilled than the existing oral and maxilofacial surgeon, whose training lasts one year and works in primary care health. They established the specialty of Oral Surgery, compromising the quality and safety of care, exposing patients to adverse events with unpredictable outcomes, affecting their quality of life or eventually causing a fatal outcome. At the time of this study there is no record of the number of oral and maxillofacial surgeons trained in our country, it is unknown how many are working in public hospitals and their distribution in health services, as well as whether there is the necessary infrastructure to develop the specialty under current regulations. We made a descriptive transversal study, conducting an interview aimed to obtain information from individual and institutional providers to know the reality of the specialty in the public health network. It is concluded that there is a sufficient number of specialists trained and in training to meet the needs of the population. The evidence shows missing possitions and infrastructure, so that the action taken by the Health Ministry regarding the creation of the specialty of Oral Surgery is due to a misdiagnosis from the point of view of an individual and institutional provider, caused by the absence of a technical study to assess the country's hospital network.

Describing How School-Based SLPs Determine Eligibility for Children with Speech Sound Disorders.

We report the results of a nation-wide survey of school-based SLPs, which further underscore this variability. Findings provide insight into which and how many factors SLPs report contributing to eligibility decisions, as well as which and how many components of an assessment are mandatory. Our discussion includes a call to advocacy for SLPs, but also a need for increased awareness of this problematic variability for school administrators.
