Hearing preservation after Gamma Knife radiosurgery for cerebellopontine angle meningiomas.

CONCLUSIONSGKRS for CPA meningiomas provides excellent hearing preservation in addition to high tumor control rate. Hearing outcome is better with CPA meningiomas than with VSs. Radiotherapy for oligometastatic cancer: a survey among radiation oncologists of Lombardy (AIRO-Lombardy), Italy

Abstract

Aims

To evaluate the use of radiotherapy (RT) for oligometastatic cancer (OMC) among radiation oncologists in Lombardy, Italy.

Methods and study design

A survey with 12 items regarding data of 2016 was sent to all 34 Lombardy RT centers. The survey included six general items and six specific items regarding patient/disease/treatment characteristics.

Results

Thirteen centers answered the survey (38%). All centers responded to general items and 12 centers submitted patient/disease/treatment data. General items The majority of centers (8/13) consider OMC if metastases number is less than 5. The most commonly prescribed dose/fraction is 5–10 Gy (8/13) using schedules of 3–5 fractions (11/13). Patient data items A total of 15.681 patients were treated in 2016 with external beam RT in 12 responding centers, and 1.087 patients were treated for OMC (7%). Primary tumor included lung, prostate, breast, colorectal and other malignancies in 33%, 21%, 12%, 9% and 25% of all OMC patients, respectively. Brain, lymph node, lung, bone, liver and others were the most common treated sites (24%, 24%, 22%, 17%, 8% and 5%, respectively). One and more than one metastasis were treated in 75 and 25% of patients, respectively. The vast majority of patients (95%) were treated with image-guided intensity-modulated RT or stereotactic RT.

Conclusions

Seven percent of all RT patients in Lombardy are treated for OMC. Extreme hypofractionation and high-precision RT are commonly employed. The initiative of multicenter and multidisciplinary collaboration has been undertaken in order to prepare the platform for prospective and/or observational studies in OMC.

Effect of hyaluronic acid on the osseointegration of dental implants

The mechanism of osseointegration is related to many factors, including the quality of the bone, the biocompatibility and surface characteristics of the implant material, the surgical technique, and functional loading. The purpose of this study was to investigate the effects of hyaluronic acid gel on the osseointegration of implants placed in defined areas of the mandible in rabbits. Hyaluronic acid is known to have an osteoinductive effect during regeneration of bony defects, and we thought that it might also have a favourable effect on osseointegration, a specialised mechanism to heal bone. We therefore surveyed surgeons from 15 cleft units who provide orthognathic treatment, to gain an understanding of current practice in the UK and Ireland. A total of 16/21 surgeons from 14/15 units responded. While 14/16 surgeons agreed that these patients are at risk of a deterioration in VPF after maxillary advancement, two disagreed. In 1903, the German pathologist Johann Mönckeberg described calcifications only affecting the tunica media of medium arteries. The entity was named Mönckeberg arteriosclerosis. Other names in the literature are Mönckeberg sclerosis and Mönckeberg medial calcinosis. The entity is strongly associated with chronic kidney disease and diabetes mellitus. Radiographically, medial calcinosis presents as areas of linear calcifications in soft tissue. The linear calcifications are described as "tram track," "pipe stem," "rail track," or "tram line" when viewed longitudinally.

Adenoid cystic carcinoma (ACC) is a rare and relentless malignancy, which accounts for less than 1% of all head and neck carcinomas. The minor salivary glands of the hard palate, as in this case, are most commonly affected. ACC's characteristic perineural invasion and subsequent proximity to vital neural and vascular structures often result in an unresectable lesion at the time of diagnosis. The frequency of recurrence is high. The goal of this report is to present some of the case findings regarding disease progression and treatment sequelae over a 7-year time span.

Therefore, a potent bisphosphonate is used to control bone metastasis, but one of its adverse effects is MRONJ. Previous studies that investigated risk factors associated with MRONJ were done in patients with different hematologic conditions, where the underlining condition might affect the result.

Development of the odontoid process (dens) involves a primary ossification center and a secondary ossification center. The primary center gives rise to the body of the dens. The secondary ossification center, called the chondrum terminale, ossifies and becomes the ossiculum terminale, which normally fuses with the body of the dens and is the tip of the dens. This enigmatic and long described pain disorder is defined by several international organizations.2-4 In 1994, the International Association for the Study of Pain (IASP) defined burning mouth syndrome (BMS) "(also known as glossodynia, glossopyrosis, oral dysesthesia, or stomatodynia) as a chronic oral mucosal pain or discomfort that has no identifiable causative lesions and is not caused by any other condition or disease".2 The IASP recognize the clinical features to include burning, tingling, pricking, or discomfort.

Condylar fractures remain the most controversial topic in maxillofacial trauma. Open reduction and internal fixation (ORIF) using an extra-oral approach has certain benefits over the nonsurgical treatment. Concerns, however, remain about the risk of facial nerve injury, post operative facial nerve weakness and facial scarring when operating in this region, despite the various extra-oral approaches developed for the same. The recently popularized Endoscope assisted open reduction and internal fixation (EAORIF) is claimed to provide better results as it is minimally invasive, provides excellent visibility, eliminates surgical scarring and the risk of facial nerve injury.

Positive influence of simvastatin used as adjuvant agent for cavity lining

Abstract

Objectives

To assess the biological, antimicrobial, and mechanical effects of the treatment of deep dentin with simvastatin (SV) before application of a glass-ionomer cement (GIC).

Materials and methods

Dentin discs were adapted to artificial pulp chambers and SV (2.5 or 1.0 mg/mL) was applied to the occlusal surface, either previously conditioned or not with EDTA (±EDTA). The extracts (culture medium + SV that diffused through dentin) was obtained and then applied to cultured odontoblast-like MDPC-23 cells. Cell viability, alkaline phosphatase (ALP) activity, and mineralization nodule (MN) deposition were evaluated. Untreated discs were used as control. The antibacterial activity of SV (2.5 or 1.0 mg/mL) against Streptococcus mutans and Lactobacillus acidophilus, as well as the bond strength of GIC to dentin in the presence of SV 2.5 mg/mL (±EDTA) were also assessed. The data were analyzed by ANOVA/Tukey tests (α = 5%).

Results

EDTA + SV 2.5 mg/mL significantly enhanced the ALP activity and MN deposition in comparison with the control, without changing in the cell viability (p < 0.05). The association EDTA + SV 2.5 mg/mL + GIC determined the highest ALP and MN values (p < 0.05). SV presented intense antimicrobial activity, and the EDTA dentin conditioning followed by SV application increased bond strength values compared with SV treatment alone (p < 0.05).

Conclusion

SV presents antimicrobial activity and diffuses across conditioned dentin to biostimulate odontoblast-like pulp cells.

Clinical significance

The use of SV as adjuvant agent for indirect pulp capping may biostimulate pulp cells thus preserving vitality and function of the pulp-dentin complex.

Patients with implanted subcutaneous foreign bodies (FB) are a common presentation to Plastic Surgery services. This implantation may be accidental, or as part of self-embedding behaviour. Removal of FB poses multiple risks including the usual risks of an operation, which may include a general anaesthetic and a tourniquet. In addition, there are risks posed by removal of FB in particular. They are often sharp, posing a needlestick risk to the surgeon. They are usually small, making identification potentially difficult, lengthy and occasionally unsuccessful.
