A Novel Grading System for Salpingopharyngeal Fold Hypertrophy in Obstructive Sleep Apnoea

Abstract

The present study aimed at reporting the incidence of Salpingopharyngeal Fold (SPF) hypertrophy on Drug Induced Sleep Endoscopy (DISE) in patients with Obstructive Sleep Apnoea (OSA, validate a grading system and analyse the impact of various grades on the clinical presentation while ascertaining its relation with Apnoea-Hypopnoea Index (AHI). A retrospective analysis of 169 patients with polysomnographic confirmation of obstructive sleep apnoea has been done in the study. The DISE video data of 169 patients was evaluated by two ENT surgeons individually and they graded the hypertrophy of the fold as Grade 0 being normal anatomy, Grade 1 being hypertrophy causing partial obstruction and Grade 2 being hypertrophy causing complete obstruction of lateral pharyngeal wall. It was found that the presence of SPF hypertrophy independently adds considerably to the severity of the obstruction, attributing to lateral collapse at the upper retropalatal level and also significantly increases AHI. It is thus advised to consider the grade of SPF hypertrophy while surgically planning the management of patients with OSA.

ConclusionMore than just the seasonal pollen index has to be considered when the SLI of a selected pollen season has to be explained. Cross-reactivity to other pollen types, allergen content and air pollution could play a considerable role. The similar behavior of the SLI in Vienna and a whole region indicate the feasibility of a possible symptom forecast in future and justifies the use of a single pollen monitoring station within a city of the size of Vienna. Distinct roles for Blimp-1 in autoreactive CD4 T cells during priming and effector phase of autoimmune encephalomyelitis

B lymphocyte-induced maturation protein (Blimp-1) is a transcription factor that regulates effector/memory B cells and CD8 T cells. Here we show that Blimp-1 is expressed in both Th1 and Th17 cells in vitro and highly expressed in effector/memory myelin-specific CD4 T cells in experimental autoimmune encephalomyelitis (EAE) mice. The immunized Blimp-1 conditional knockout mice have a significantly delayed disease onset but enhanced disease severity during the effector phase compared to their wild-type littermates, suggesting that Blimp-1 is a unique transcription factor with distinct roles in the regulation of myelin-specific CD4 T cells during priming and effector phase of EAE. The aim of this study was to evaluate the therapeutic efficacy of the oral ingestion of HDM extracts in a murine model of allergic asthma.

Methods

BABL/c mice were sensitized twice by intraperitoneal injection of HDM extracts and Al(OH)3 on day 1 and day 8. Then, the mice received challenge to induce airway inflammation by intratracheal instillation of HDM extracts on days 29–31. The treatment group received immunotherapy with oral HDM extracts ingestion before the challenge. All the mice were sacrificed on day 32 for bronchoalveolar inflammatory cytokines, mediastinal lymph node T cells, lung histology, and serum HDM-specific immunoglobulins analyses.

Results

Upon HDM sensitization and following challenge, a robust Th2 cell response and eosinophilic airway inflammation were observed in mice of the positive control group. The mice treated with HDM extracts ingestion had decreased eosinophilic airway inflammation, suppressed HDM-specific Th2 cell responses in the mediastinal lymph nodes, and attenuated serum HDM-specific IgE levels.

Conclusions

Oral immunotherapy with HDM extracts ingestion was demonstrated to have a partial therapeutic effect in the murine model of allergic asthma. This study may serve as the basis for the further development of oral immunotherapy with HDM extracts in allergic asthma.

Recommendations for the nomenclature of cognitive change associated with anaesthesia and surgery—2018

Abstract

Cognitive change affecting patients after anaesthesia and surgery has been recognised for more than 100 yr. Research into cognitive change after anaesthesia and surgery accelerated in the 1980s when multiple studies utilised detailed neuropsychological testing for assessment of cognitive change after cardiac surgery. This body of work consistently documented decline in cognitive function in elderly patients after anaesthesia and surgery, and cognitive changes have been identified up to 7.5 yr afterwards. Importantly, other studies have identified that the incidence of cognitive change is similar after non-cardiac surgery. Other than the inclusion of non-surgical control groups to calculate postoperative cognitive dysfunction, research into these cognitive changes in the perioperative period has been undertaken in isolation from cognitive studies in the general population. The aim of this work is to develop similar terminology to that used in cognitive classifications of the general population for use in investigations of cognitive changes after anaesthesia and surgery. A multispecialty working group followed a modified Delphi procedure with no prespecified number of rounds comprised of three face-to-face meetings followed by online editing of draft versions.

Two major classification guidelines [Diagnostic and Statistical Manual for Mental Disorders, fifth edition (DSM-5) and National Institute for Aging and the Alzheimer Association (NIA-AA)] are used outside of anaesthesia and surgery, and may be useful for inclusion of biomarkers in research. For clinical purposes, it is recommended to use the DSM-5 nomenclature. The working group recommends that 'perioperative neurocognitive disorders' be used as an overarching term for cognitive impairment identified in the preoperative or postoperative period. This includes cognitive decline diagnosed before operation (described as neurocognitive disorder); any form of acute event (postoperative delirium) and cognitive decline diagnosed up to 30 days after the procedure (delayed neurocognitive recovery) and up to 12 months (postoperative neurocognitive disorder).

The connection of pollen concentrations and crowd-sourced symptom data: new insights from daily and seasonal symptom load index data from 2013 to 2017 in Vienna

Abstract

Background

Online pollen diaries and mobile applications nowadays allow easy and fast documentation of pollen allergy symptoms. Such crowd-sourced symptom data provides insights into the development and the onset of a pollen allergy. Hitherto studies of the symptom load index (SLI) showed a discrepancy between the SLI and the total pollen amount of a season, but did not analyze the daily data.

Methods

The Patient's Hayfever Diary (PHD) was used as data pool for symptom data. Symptom data of Vienna (Austria) was chosen as a large and local sample size within the study period of 2013 until 2017. The city was divided into three different areas based on equal population densities and different environmental factors. Correlation factors, regression lines, locally weighted smoothing (LOESS) curves and line plots were calculated to examine the data.

Results

Daily SLI and pollen concentration data correlates well and the progress of the SLI within a pollen season is mirrored by the pollen concentrations. The LOESS curves do not deviate much from the regression line and support the linearity of the symptom-pollen correlation on a daily basis. Seasonal SLI data does not follow the same pattern as the respective seasonal pollen indices. Results did not vary in the three areas within Vienna or when compared with the Eastern region of Austria showing no significant spatial variation of the SLI.

Discussion

Results indicate a linear relationship of the SLI and pollen concentrations/seasonal polllen index (SPIn) on a daily basis for both in general and throughout the season, but not on a seasonal basis. These findings clarify the frequent misinterpretation of the SLI as index that is tightly connected to pollen concentrations, but reflects as well the seasonal variation of the burden of pollen allergy sufferers.

Conclusion

More than just the seasonal pollen index has to be considered when the SLI of a selected pollen season has to be explained. Cross-reactivity to other pollen types, allergen content and air pollution could play a considerable role. The similar behavior of the SLI in Vienna and a whole region indicate the feasibility of a possible symptom forecast in future and justifies the use of a single pollen monitoring station within a city of the size of Vienna.

While the progression of acute rhinosinusitis to orbital infections has decreased in the modern antibiotic era, they still occur, particularly in the pediatric population. Infection may spread from the nasal cavity and sinuses via direct extension into the orbit through the thin lamina papyracea or through the valveless veins in the bony orbital wall.1 Congenital dehiscence and trauma also allow for direct extension of the infection to the orbit.1 Most of these orbital infections originate in the ethmoid sinuses and subperiosteal abscesses most commonly occur along the medial orbital wall.

The patients' clinical and personal details, characteristics of the tumours, types of microvascular flap, complications, and outcomes were recorded. The soft tissue flaps used most often were the fasciocutaneous radial forearm free flap (RFFF) (n=86, 45%) and the anterolateral thigh free flap (ALTFF) (n=48, 25%) while the most commonly used osseous flap was the deep circumflex iliac artery flap (DCIA) (n=25, 13%).

They were followed up for 1–9 years to check for the relevant symptoms. We studied 17 female and 20 male patients, mean age 52 (range 19–78) years. The flaps took a maximum of 17minutes to dissect. There was no donor site morbidity, the vascular fat flap was stable in all cases for up to nine years, and none of the patients complained of symptoms of Frey syndrome.

Impact of Data Presentation on Physician Performance Utilizing Artificial Intelligence-Based Computer-Aided Diagnosis and Decision Support Systems

Abstract

Ultrasound (US) is a valuable imaging modality used to detect primary breast malignancy. However, radiologists have a limited ability to distinguish between benign and malignant lesions on US, leading to false-positive and false-negative results, which limit the positive predictive value of lesions sent for biopsy (PPV3) and specificity. A recent study demonstrated that incorporating an AI-based decision support (DS) system into US image analysis could help improve US diagnostic performance. While the DS system is promising, its efficacy in terms of its impact also needs to be measured when integrated into existing clinical workflows. The current study evaluates workflow schemas for DS integration and its impact on diagnostic accuracy. The impact on two different reading methodologies, sequential and independent, was assessed. This study demonstrates significant accuracy differences between the two workflow schemas as measured by area under the receiver operating curve (AUC), as well as inter-operator variability differences as measured by Kendall's tau-b. This evaluation has practical implications on the utilization of such technologies in diagnostic environments as compared to previous studies.

Introducing the craniocervical Y-ligament

Abstract

Purpose

We examined the accessory atlantoaxial ligaments and found them to be a part of a complex ligamentous structure, which we named craniocervical Y-ligament with respect to its shape.

Methods

The ligaments of the upper cervical spine were dissected in ten Thiel embalmed human cadavers. Origin and attachment of the Y-ligament were described and a detailed photo and video-documentation was carried out with the head in the neutral position, flexion, extension and rotation to study the ligament during these movements.

Results

The Y-ligaments were found to be paired and symmetric in all specimens. The shape of the ligament is similar to an Y, its lateral arm connecting the atlas to the axis, its medial arm connecting the occipital bone to the axis, fusing with the two main ligaments, the alar and transverse ligaments. The lateral arm of the Y-ligament was found to be analogous to the accessory atlantoaxial ligament. During cervical flexion, both arms of the Y-ligament became taut while extension made the Y-ligaments relaxed. During rotation both Y-ligaments became taut, moving in the opposite directions in the sagittal plane while following the gliding movements of the lateral masses of the atlas.

Conclusions

The craniocervical Y-ligament is a complex ligamentous structure and has a constant anatomy. Because of its shape and special arrangement, it probably plays a role in limiting both atlantooccipital and atlantoaxial movements. Acknowledgement of this ligamentous structure will help understand upper cervical stability. The present study should serve as a basis for future biomechanical and radiological studies.

Methods

These extractions were divided in such a way that all right-sided extractions were done under 2% lignocaine with 1:80,000 adrenaline; and all left-sided extractions were done under 2% lignocaine with 1:80,000 adrenaline with 7.4% sodium bicarbonate added. All extractions were performed using a consistent intra-alveolar technique by a single operator. Both the patient and the operator were blinded to the contents of the local anesthetic solution. Data records of these patients were procured on the basis of VAS and VRS, requirement of repeated injection, and onset and duration of anesthesia.

Results

Clinical and statistical data confirmed that the addition of sodium bicarbonate in local anesthetics reduces pain and decreased the onset and increased duration of local anesthesia in the palatal site, as compared to injection of local anesthetic without sodium bicarbonate.

Conclusion

Effect of sodium bicarbonate on reducing pain while injecting into the palatal aspect was noted in this study.

We also examined the value of MRI with Tanner-Whitehouse 3 (TW3) method in assessing skeletal maturity. Seventy-nine 12–17-year-old healthy Hong Kong Chinese adolescents were recruited. The left hand and wrist region were scanned by a dedicated skeletal MRI scanner. T1-weighted three-dimensional coronal view images for the left hand and wrist region were acquired. Independent maturity indicators such as subject body height, body weight, bone marrow composition intensity quantified by MRI, and TW3 skeletal age were included for artificial neural network (ANN) analysis. Our results indicated that the skeletal age was generally underestimated using TW3 method, and significant difference (p < 0.05) was noted for skeletal age with chronological age for female category and at later stage of adolescence (15 to 17 years old) in both genders. In our proposed machine learning approach, ages determined by ANN method agreed well with chronological age (p > 0.05).The machine learning approach using ANN method was about 10-fold more accurate than the TW3 method using MRI alone. It offers a more objective and accurate solution for prospective chronological maturity assessment for adolescents.
