Arc of Bühler: the surgical significance of a rare anatomical variation

Abstract

Purpose

The arc of Bühler, an anastomotic vessel between celiac artery and superior mesenteric artery, is a rare anatomic variation. Various radiologic and surgical procedures can be affected by its existence. We aim to review all available information and identify possible clinical implications.

Methods

A systematic review was conducted in accordance to the Preferred Reporting Items for Systematic reviews and Meta-Analysis (PRISMA) guidelines. The following terms were utilized in various combinations: "Bühler", "arc of Bühler", "visceral aneurysm", "pancreaticoduodenal arcades".

Results

Only 53 cases have been described until today in the literature. The arc of Bühler enhances collateral circulation between celiac artery and superior mesenteric artery alongside with pancreaticoduodenal arcades and dorsal pancreatic artery. Computerized tomography and angiography are the main studies used for its detection and evaluation. Aneurysms of Bühler's arc have been rarely described and seem to share common pathophysiological mechanisms with aneurysms of the pancreaticoduodenal arcades.

Conclusions

Various radiologic and surgical procedures such as embolization or pancreaticoduodenectomy are potentially affected by its existence.

Application of low-cost fused deposition modeling additive manufacturing rapid anatomic models in patients with rhino-cerebral mucormycosis treated with maxillectomy

Abstract

Mucormycosis is a fungal infection mainly of immunocompromised patients. It has many clinical manifestations depending on the system that is affected. Rhinocerebral mucormycosis and pulmonary infection are the most common. In this pathology, the fungi invade the endothelium of the blood vessels with partial or total obliteration, with tissue ischemia and necrosis. The initial symptoms are unspecific making difficult to identify from other infections and need an expert suspicion level. The gold standard is the culture of the fungi; histopathology analysis shows empty hyphae with thin walls. Combined treatment is strongly recommended surgical extended debridement and Amphotericin B is the gold standard of treatment. The craniofacial skeleton (CS) has a very peculiar bone and soft tissue distribution that maintains height, width, and projection. In RCM, aggressive debridement is prognostic for patient survival. Extensive maxillectomies, often with orbital exenteration represent a reconstructive challenge for plastic surgeons. Recently different types of additive manufacture (AM) technology had been proposed in surgical planning. Fused deposition manufacture (FDM) technology could be the most cost-effective for patients. To the best of our knowledge, this is the first report of the use of FDM and reconstruction of patients with RCM. RCM is a very serious life-threatening condition. When treated in time with the adequate surgical approach and a correct antifungal medicine RCM can be a cured. The goals in treating this condition are very clear. A free flap reconstruction is an adequate approach to patients with RCM and FDM RAM can help the surgeon to plan the free flap in site. We present two cases reconstructed with MSAP and ALT-FL respectively with success assisted with FDM RAM.

Level of Evidence: Level V, therapeutic study.

Use of the myocutaneous serratus anterior free flap for reconstruction after salvage glossectomy

Abstract

Purpose

To describe the use of a myocutaneous serratus anterior free flap (SAFF) for tongue reconstruction after salvage subtotal (STG) and total glossectomy (TG).

Methods

In this prospective case series, seven patients underwent salvage STG or TG and reconstruction with a myocutaneous SAFF between 10/2015 and 02/2017. Functional and oncologic outcomes were prospectively evaluated. Donor side morbidity was determined using the Disabilities of the Arm, Shoulder and Hand (DASH) score.

Results

SAFF with mean skin paddles of 6.7 cm × 8.7 cm was used in five STG and two TG patients, respectively. There was a 100% flap survival and a mean DASH score of 10.8 reflected normal arm and shoulder function after surgery. One year after salvage surgery, 1 (14.3%) and 4 (57.1%) patients were tracheostomy and gastrostomy tube dependent. Gastrostomy tube dependence was significantly worse in patients with tumors of the base of tongue compared to other tumor sites (p = 0.030) and in patients who underwent transcervical compared to transoral tumor resection (p = 0.008). Local recurrence rate was 57.1% with a disease-free survival of 17.6 months.

Conclusion

The myocutaneous SAFF represents a safe and reliable flap for tongue reconstruction after salvage glossectomy with satisfying functional outcomes and low donor side morbidity.

Efficacy of Endo-Ice followed by intrapulpal ice application as an adjunct to inferior alveolar nerve block in patients with symptomatic irreversible pulpitis—a randomized controlled trial

Abstract

Objective

The purpose of the present study was to evaluate the effect of Endo-Ice followed by intrapulpal ice application for reducing pain during pulp extirpation in mandibular molars with symptomatic irreversible pulpitis.

Materials and methods

Sixty patients diagnosed with symptomatic irreversible pulpitis participated in the present study. Subjects were randomly allocated to any one of the following groups: control group—inferior alveolar nerve block with lignocaine (2%) adrenaline (1:80000), articaine group—inferior alveolar nerve block with lignocaine (2%) adrenaline (1:80000) + Buccal infiltration with articaine (4%) with adrenaline (1:100000); or cold group—inferior alveolar nerve block with lignocaine (2%) adrenaline (1:80000) + cold application. The outcome assessor measured the level of pain during access opening and pulp extirpation using the visual analogue scale. The anxiety level of the patient was also measured.

Results

During access opening, there was a significant difference in the pain reduction in the articaine group when compared to cold and control group (p value = 0.02). During pulp extirpation, cold group and articaine group showed a significant reduction in pain levels as compared to the control group (p value = 0.001). There was no difference in the pain level during pulp extirpation among the two test arms (articaine and cold) (p value = 0.99). Further, cold significantly reduced the level of anxiety when compared to the articaine or control group (p value = 0.001).

Conclusion

Hence, cold is a simple, supplementary technique in reducing pain during pulp extirpation.

Clinical relevance

Application of cold may help in minimizing the fear of additional injection in managing pain during endodontic treatment.

TMJ arthrosis: does the occlusal relationship really interfere? A comparison between cone beam computed tomography and dried skulls

Abstract

The aim of this study was to investigate the association between condylar bone morphological characteristics with occlusal conditions. Besides the study will compare the tomography images with the real condition in 122 temporomandibular joints from 61 skulls. The occlusal conditions were evaluated by number of teeth missing, measurement of overjet and overbite, in millimeters, and presence or absence of crossbite, openbite and dental rotation. The condylar bone morphological conditions were classified in five types (normal, presence of erosion, presence of osteophytes, flattening and/or deformation). This classification was used in real skulls and in Cone Beam Computed tomography (CBCT) images. The data were submitted to statistical analysis with a level of significance of 0.05. Occlusal variables have no association to morphologic data (p > 0.05). Normal condylar bone was seen in 62 CBCT versus 53 in real skulls while morphological alterations were seen in 60 CBCT versus 67-real condyles. The clinical and tomographic measurements were compared, demonstrating an important difference in the classification demonstrating poor association between detection methods (k − 0.3, p < 0.001). The occlusal conditions appear to have no correlation with the morphological condyle conditions. The CBCT is a reliable diagnostic method, although it may present divergences of findings when compared with clinical raw examination to morphologic condylar conditions.

TMJ arthrosis: does the occlusal relationship really interfere? A comparison between cone beam computed tomography and dried skulls

Abstract

The aim of this study was to investigate the association between condylar bone morphological characteristics with occlusal conditions. Besides the study will compare the tomography images with the real condition in 122 temporomandibular joints from 61 skulls. The occlusal conditions were evaluated by number of teeth missing, measurement of overjet and overbite, in millimeters, and presence or absence of crossbite, openbite and dental rotation. The condylar bone morphological conditions were classified in five types (normal, presence of erosion, presence of osteophytes, flattening and/or deformation). This classification was used in real skulls and in Cone Beam Computed tomography (CBCT) images. The data were submitted to statistical analysis with a level of significance of 0.05. Occlusal variables have no association to morphologic data (p > 0.05). Normal condylar bone was seen in 62 CBCT versus 53 in real skulls while morphological alterations were seen in 60 CBCT versus 67-real condyles. The clinical and tomographic measurements were compared, demonstrating an important difference in the classification demonstrating poor association between detection methods (k − 0.3, p < 0.001). The occlusal conditions appear to have no correlation with the morphological condyle conditions. The CBCT is a reliable diagnostic method, although it may present divergences of findings when compared with clinical raw examination to morphologic condylar conditions.

Immunoglobulin E (IgE) antibodies protect against helminth infections but can also cause life-threatening allergic reactions. Despite their role in human health, the cells that produce these antibodies are rarely observed and remain enigmatic. We isolated single IgE B cells from individuals with food allergies and used single-cell RNA sequencing to elucidate the gene expression and splicing patterns unique to these cells. Brain {beta}-Amyloid and Atrophy in Individuals at Increased Risk of Cognitive Decline [ADULT BRAIN]

BACKGROUND AND PURPOSE:

The relationship between brain β-amyloid and regional atrophy is still incompletely understood in elderly individuals at risk of dementia. Here, we studied the associations between brain β-amyloid load and regional GM and WM volumes in older adults who were clinically evaluated as being at increased risk of cognitive decline based on cardiovascular risk factors.

MATERIALS AND METHODS:

Forty subjects (63–81 years of age) were recruited as part of a larger study, the Finnish Geriatric Intervention Study to Prevent Cognitive Impairment and Disability. Neuroimaging consisted of PET using 11C Pittsburgh compound-B and T1-weighted 3D MR imaging for the measurement of brain β-amyloid and GM and WM volumes, respectively. All subjects underwent clinical, genetic, and neuropsychological evaluations for the assessment of cognitive function and the identification of cardiovascular risk factors.

RESULTS:

Sixteen subjects were visually evaluated as showing cortical β-amyloid (positive for β-amyloid). In the voxel-by-voxel analyses, no significant differences were found in GM and WM volumes between the samples positive and negative for β-amyloid. However, in the sample positive for β-amyloid, increases in 11C Pittsburgh compound-B uptake were associated with reductions in GM volume in the left prefrontal (P = .02) and right temporal lobes (P = .04).

CONCLUSIONS:

Our results show a significant association between increases in brain β-amyloid and reductions in regional GM volume in individuals at increased risk of cognitive decline. This evidence is consistent with a model in which increases in β-amyloid incite neurodegeneration in memory systems before cognitive impairment manifests.

MRI Abnormalities Predominate in the Bottom Part of the Sulcus with Type II Focal Cortical Dysplasia: A Quantitative Study [PEDIATRICS]

BACKGROUND AND PURPOSE:

Type II focal cortical dysplasia is a common histopathological substrate in focal epilepsy. This study explored the spatial distribution of abnormal findings on MR imaging across the sulcus with type II focal cortical dysplasia using quantitative MR imaging postprocessing techniques.

MATERIALS AND METHODS:

The morphometric analysis program and normalized FLAIR signal intensity analysis were applied to retrospectively analyze the MR imaging data of 58 patients with histopathologically confirmed type II focal cortical dysplasia. We divided the dysplastic sulcus into the bottom and nonbottom parts. Then spatial distribution types 1, 2, and 3 were arbitrarily defined as the abnormal findings on MR imaging (z-value > threshold) located in the bottom part, both the bottom and nonbottom parts, and the nonbottom part, respectively. For type 2, the mean z-values and standardized volumes of abnormal findings on MR imaging were compared between the bottom and nonbottom parts.

RESULTS:

Abnormal findings on MR imaging were detected by quantitative techniques in 42 of 58 enrolled patients. Among these 42 patients, 38 and 26 patients showed gray-white matter junction blurring and cortical FLAIR hyperintensity, respectively, which were the 2 most common abnormal MR imaging features. Gray-white matter junction blurring manifested as types 1, 2, and 3 in 24, 13, and 1 patient, respectively, and the corresponding counts for cortical FLAIR hyperintensity were 12, 13, and 1 patient. For the 2 most common abnormal findings on MR imaging spatially manifested as type 2, higher mean z-values and larger corresponding standardized volumes of abnormalities were found in the bottom part.

CONCLUSIONS:

Abnormal findings on MR imaging predominate in the bottom part of the sulcus with type II focal cortical dysplasia, which indicates that this malformation is bottom-of-sulcus-rooted.

Defining the Normal Dorsal Contour of the Corpus Callosum with Time [ADULT BRAIN]

BACKGROUND AND PURPOSE:

Morphological changes of the corpus callosum have been associated with a large number of congenital neurocognitive and psychiatric disorders. Focal defects or notches of the dorsal surface of the corpus callosum have not been well characterized. Our purpose was the following; 1) to characterize the dorsal contour of the corpus callosum during the life span, 2) to characterize the relationship of contour deviations to neighboring vessels, and 3) to determine whether contour deviations are congenital or acquired.

MATERIALS AND METHODS:

We retrospectively reviewed normal sagittal T1-weighted brain MR images. A "notch" was defined as a concavity in the dorsal surface at least 1 mm in depth. The corpus callosum was considered to be "undulating" if there were >2 notches, including an anterior and posterior notch. The presence of a pericallosal artery and its relationship to a notch were assessed.

RESULTS:

We reviewed 1639 MR imaging studies, spanning 0–89 years of age. A total of 1102 notches were identified in 823 studies; 344 (31%) were anterior, 660 (60%) were posterior, and 98 (9%), undulating. There was a positive correlation between the prevalence (P < .001) and depth (P = .028) of an anterior notch and age and a negative correlation between the prevalence of a posterior notch and age (P < .001). There was no difference between patient sex and corpus callosum notching (P = .884). Of the 823 studies with notches, 490 (60%) were associated with a pericallosal artery (P < .001).

CONCLUSIONS:

The prevalence and depth of notches in the anterior corpus callosum increase significantly with age; this finding suggests that most notches are acquired. There is a significant positive association between the presence of a corpus callosum notch and adjacent pericallosal arteries, suggesting that this may play a role in notch formation.

Early Diagnosis of Spastic Cerebral Palsy in Infants with Periventricular White Matter Injury Using Diffusion Tensor Imaging [FUNCTIONAL]

BACKGROUND AND PURPOSE:

Periventricular white matter injury is the common cause of spastic cerebral palsy. However, the early diagnosis of spastic cerebral palsy still remains a challenge. Our aim was to investigate whether infants with periventricular white matter injury with bilateral spastic cerebral palsy have unique lesions different from those in infants without cerebral palsy and to evaluate the efficiency of DTI in the early diagnosis of spastic cerebral palsy.

MATERIALS AND METHODS:

Infants with periventricular white matter injury and controls underwent MR imaging at 6–18 months of age. Fractional anisotropy was calculated from DTI. Cerebral palsy was diagnosed by 24–30 months of age. Subjects were divided into 3 groups: infants with periventricular white matter injury with bilateral spastic cerebral palsy, infants with periventricular white matter injury without cerebral palsy, and controls. Tract-Based Spatial Statistics and Automated Fiber Quantification were used to investigate intergroup differences. Receiver operating characteristic curves were used to assess the diagnostic accuracy of spastic cerebral palsy. Correlations between motor function scores and fractional anisotropy were evaluated along white matter tracts.

RESULTS:

There were 20, 19, and 33 subjects in periventricular white matter injury with spastic cerebral palsy, periventricular white matter injury without cerebral palsy, and control groups, respectively. Decreased fractional anisotropy in the corticospinal tract was only observed in infants with periventricular white matter injury with spastic cerebral palsy, whereas decreased fractional anisotropy in the posterior thalamic radiation and genu and splenium of the corpus callosum was seen in both periventricular white matter injury subgroups. Fractional anisotropy in the corticospinal tract at the internal capsule level was effective in differentiating infants with periventricular white matter injury with spastic cerebral palsy from those without cerebral palsy by a threshold of 0.53, and it had strong correlations with motor function scores.

CONCLUSIONS:

Corticospinal tract lesions play a crucial role in motor impairment related to spastic cerebral palsy in infants with periventricular white matter injury. Fractional anisotropy in the corticospinal tract at the internal capsule level could aid in the early diagnosis of spastic cerebral palsy with high diagnostic accuracy.

Defining the Normal Dorsal Contour of the Corpus Callosum with Time [ADULT BRAIN]

BACKGROUND AND PURPOSE:

Morphological changes of the corpus callosum have been associated with a large number of congenital neurocognitive and psychiatric disorders. Focal defects or notches of the dorsal surface of the corpus callosum have not been well characterized. Our purpose was the following; 1) to characterize the dorsal contour of the corpus callosum during the life span, 2) to characterize the relationship of contour deviations to neighboring vessels, and 3) to determine whether contour deviations are congenital or acquired.

MATERIALS AND METHODS:

We retrospectively reviewed normal sagittal T1-weighted brain MR images. A "notch" was defined as a concavity in the dorsal surface at least 1 mm in depth. The corpus callosum was considered to be "undulating" if there were >2 notches, including an anterior and posterior notch. The presence of a pericallosal artery and its relationship to a notch were assessed.

RESULTS:

We reviewed 1639 MR imaging studies, spanning 0–89 years of age. A total of 1102 notches were identified in 823 studies; 344 (31%) were anterior, 660 (60%) were posterior, and 98 (9%), undulating. There was a positive correlation between the prevalence (P < .001) and depth (P = .028) of an anterior notch and age and a negative correlation between the prevalence of a posterior notch and age (P < .001). There was no difference between patient sex and corpus callosum notching (P = .884). Of the 823 studies with notches, 490 (60%) were associated with a pericallosal artery (P < .001).

CONCLUSIONS:

The prevalence and depth of notches in the anterior corpus callosum increase significantly with age; this finding suggests that most notches are acquired. There is a significant positive association between the presence of a corpus callosum notch and adjacent pericallosal arteries, suggesting that this may play a role in notch formation.

Clinical Feasibility of Zero TE Skull MRI in Patients with Head Trauma in Comparison with CT: A Single-Center Study [ADULT BRAIN]

BACKGROUND AND PURPOSE:

Conventional MR imaging techniques cannot produce optimal images of bone structures because bone has little water and a very short T2 life span. The aim of this study was to investigate the clinical feasibility of skull MR imaging using the zero TE sequence in patients with head trauma by assessing its diagnostic image quality and quantitative measurement compared with CT images.

MATERIALS AND METHODS:

Thirteen enrolled patients with head trauma were assessed using brain CT and skull MR imaging. Image quality was graded on a 5-point Likert scale to compare the 2 modalities. To evaluate quantitative analyses between the 2 imaging modalities, we measured skull thickness and normalized bone tissue signal. Interobserver reliability was assessed using weighted statistics and the intraclass correlation coefficient.

RESULTS:

Both imaging techniques clearly depicted skull fractures in all 13 patients. The mean scores for skull MR imaging and CT were 4.65 ± 0.56 and 4.73 ± 0.45 (P = .157), respectively, with substantial interobserver agreement (P < .05). The 2 imaging modalities showed no difference in skull thickness (P = .092) and had good correlation (r2 = 0.997). The mean value of normalized bone tissue signal among the 3 layers of the skull was relatively consistent (P = .401) with high interobserver agreement (P < .001).

CONCLUSIONS:

Zero TE skull MR imaging has diagnostic image quality comparable with that of CT images. It also provides consistent results on the quantitative measurement of cortical bone with CT images.

Brain {beta}-Amyloid and Atrophy in Individuals at Increased Risk of Cognitive Decline [ADULT BRAIN]

BACKGROUND AND PURPOSE:

The relationship between brain β-amyloid and regional atrophy is still incompletely understood in elderly individuals at risk of dementia. Here, we studied the associations between brain β-amyloid load and regional GM and WM volumes in older adults who were clinically evaluated as being at increased risk of cognitive decline based on cardiovascular risk factors.

MATERIALS AND METHODS:

Forty subjects (63–81 years of age) were recruited as part of a larger study, the Finnish Geriatric Intervention Study to Prevent Cognitive Impairment and Disability. Neuroimaging consisted of PET using 11C Pittsburgh compound-B and T1-weighted 3D MR imaging for the measurement of brain β-amyloid and GM and WM volumes, respectively. All subjects underwent clinical, genetic, and neuropsychological evaluations for the assessment of cognitive function and the identification of cardiovascular risk factors.

RESULTS:

Sixteen subjects were visually evaluated as showing cortical β-amyloid (positive for β-amyloid). In the voxel-by-voxel analyses, no significant differences were found in GM and WM volumes between the samples positive and negative for β-amyloid. However, in the sample positive for β-amyloid, increases in 11C Pittsburgh compound-B uptake were associated with reductions in GM volume in the left prefrontal (P = .02) and right temporal lobes (P = .04).

CONCLUSIONS:

Our results show a significant association between increases in brain β-amyloid and reductions in regional GM volume in individuals at increased risk of cognitive decline. This evidence is consistent with a model in which increases in β-amyloid incite neurodegeneration in memory systems before cognitive impairment manifests.

This study assessed the impact of collaborative working with a headache neurologist on diagnoses of patients attending orofacial pain (OFP) clinic. Patient diagnostic data was collected from adult patients attending an Orofacial Pain Service from January 2013 to January 2017. Characterization of Patients with Ibuprofen Hypersensitivity

Conclusion: In our cohort of patients, ibuprofen hypersensitivity affected females more commonly than males, and presented with mainly cutaneous manifestations. Onset of symptoms was rapid (#x3c; 60 min). Reactions typically ranged in severity from mild to moderate although there was a risk of severe anaphylaxis. There was potential cross-reactivity with other NSAIDs or acetaminophen. The Reliability of MAML2 Gene Rearrangement in Discriminating Between Histologically Similar Glandular Odontogenic Cysts and Intraosseous Mucoepidermoid Carcinomas

MAML2 expression is proven in the majority of mucoepidermoid carcinomas (MEC) arising in salivary glands. MEC can also occur intraosseously (IMEC). Glandular odontogenic cyst (GOC) is an odontogenic cyst with histologic overlap with IMEC. This study will determine the reliability of MAML2 in distinguishing cases of IMEC from GOC.

Diagnostic utility of microsurgical carbon-dioxide laser excision of oral potentially malignant lesions (OPMLs) vs incisional biopsy: a retrospective histopathological review

Complete excision of oral potentially malignant lesions (OPMLs) could result in improved and earlier detection of more severe grades of oral epithelial dysplasia and/or frank malignancy. Transoral micro-surgical carbon dioxide (CO2) laser techniques allow for resection of OPMLs, even those that are extensive. The advantages are improved diagnostic yield, improved viability of the specimen for pathologic evaluation reduced post-surgical morbidity and easier post-operative clinical surveillance.
