Steroid-Dependent Episodic Angioedema with Eosinophilia (Gleich Syndrome) in a Patient with Rheumatoid Arthritis

Episodic angioedema with eosinophilia, or Gleich syndrome, is a rare disorder characterized by periodic episodes of angioedema and leukocytosis with marked eosinophilia. Symptoms resolve spontaneously or are treated with corticosteroids as first-line therapy. Treatment of patients unresponsive to or dependent on corticosteroids is poorly defined. Several rheumatologic diseases, including rheumatoid arthritis (RA), can also present with elevated eosinophilia in blood or tissue. Can the morbidly obese asthmatic child be well controlled? The Breathmobile program

The obese asthmatic is thought to disproportionally experience greater severity, morbidity, increased exacerbations, and reduced inhaled corticosteroid responsiveness. Schilddrüsenkarzinome aus Residuen des Ductus thyreoglossus sind eine Seltenheit und werden meist als Zufallsbefund postoperativ festgestellt. Es gibt keine klaren Richtlinien für das korrekte therapeutische Vorgehen nach erfolgter Resektion, insbesondere bezüglich der Notwendigkeit einer totalen Thyroidektomie und Radioiodtherapie. Vor dem Hintergrund der aktuellen Literatur wird eine Fallserie von fünf Patienten mit Schilddrüsenkarzinomen in einer medianen Halszyste präsentiert. Es erfolgte eine retrospektive Analyse aller Patienten mit einem Schilddrüsenkarzinom in einer medianen Halszyste im Zeitraum zwischen 2002 und 2017.Von 578 Patienten, die aufgrund einer medianen Halszyste operiert wurden, konnte bei fünf dieser Patienten (3 Frauen, 2 Männer), im Alter von 16–73 Jahren (Altersmittelwert 51; Median 56 Jahre) ein Schilddrüsenkarzinom gesichert werden. Klinisch stellten sich alle Patienten mit einer schmerzlosen Schwellung im Bereich des Zungenbeins vor. Die Diagnose wurde in allen Fällen postoperativ gestellt, in einem Fall wurde in der präoperativen Bildgebung bereits ein Malignom vermutet. Histologisch zeigte sich in allen Fällen ein papilläres Schilddrüsenkarzinom. Vier Patienten erhielten im Intervall eine totale Thyroidektomie und Radioiodtherapie, zwei davon zusätzlich eine Neck dissection des medialen und lateralen Kompartiments, der fünfte Patient hat sich einer weiteren Diagnostik und Therapie entzogen. Es wurden in allen vier Fällen keine Karzinome der Schilddrüse oder Lymphknotenmetastasen nachgewiesen.Aufgrund der Seltenheit von Schilddrüsenkarzinomen in medianen Halszysten existiert derzeit kein standardisiertes therapeutisches Vorgehen. Es sollte eine Risikostratifizierung vorgenommen werden, um Patienten, die von einer zusätzlichen Thyroidektomie profitieren könnten, zu identifizieren. Die Prognose ist sehr gut.

Hintergrund Die Therapie der chronischen Sinusitis frontalis stellt sich aufgrund des anatomisch engen Abflusses in die Nasenhaupthöhle häufig als problematisch dar. Neben konservativen Methoden wurden bisher auch viele chirurgische Therapieverfahren, sowohl mit als auch ohne Einsatz von Platzhaltern, jedoch ohne längerfristigen Therapieerfolg erprobt. Ziel dieser Arbeit war die klinische Testung der allgemeinen Praktikabilität und der kurz- sowie langfristige Therapieerfolg des neuartigen bioresorbierbaren Propel mini Stents. Material und Methoden In einer prospektiven Studie an 21 Patienten mit chronischer Rhinosinusitis mit Polyposis nasi wurde die chirurgische Therapie und das Einbringen des Stents dargestellt und die Degradation von insgesamt 31 Stents sowie der Erfolg dieser Therapieform über einen Zeitraum von 6 Monaten endoskopisch kontrolliert und analysiert. Ergebnisse Hinsichtlich chirurgischer Praktikabilität erwies sich der Stent mit Einführungstool als unkompliziertes und leicht bedienbares System. Alle Patienten haben die Stents gut toleriert. Die durchschnittliche, nachweisbare Materialabsorption überdauerte 28 Tage. Das Patientenkollektiv zeigte in 94 % der Fälle nach 3 Monaten und in 87 % der Fälle nach 6 Monaten einen vollständig sondierbares Neo-Ostium des Sinus frontalis. Schlussfolgerung Die Ergebnisse der prospektiven Studie zeigen insgesamt eine gute Akzeptanz bei den Patienten ohne Komplikationen sowie eine vollständige Stentdegradation. Auch die Follow-Up-Untersuchungen ergaben zufriedenstellende Ergebnisse über 6 Monate. Insbesondere unter dem Aspekt der endoskopischen Operation erwies sich das Stent-System als sehr praktikabel.

Advancing Semantic Interoperability of Image Annotations: Automated Conversion of Non-standard Image Annotations in a Commercial PACS to the Annotation and Image Markup

Abstract

Sharing radiologic image annotations among multiple institutions is important in many clinical scenarios; however, interoperability is prevented because different vendors' PACS store annotations in non-standardized formats that lack semantic interoperability. Our goal was to develop software to automate the conversion of image annotations in a commercial PACS to the Annotation and Image Markup (AIM) standardized format and demonstrate the utility of this conversion for automated matching of lesion measurements across time points for cancer lesion tracking. We created a software module in Java to parse the DICOM presentation state (DICOM-PS) objects (that contain the image annotations) for imaging studies exported from a commercial PACS (GE Centricity v3.x). Our software identifies line annotations encoded within the DICOM-PS objects and exports the annotations in the AIM format. A separate Python script processes the AIM annotation files to match line measurements (on lesions) across time points by tracking the 3D coordinates of annotated lesions. To validate the interoperability of our approach, we exported annotations from Centricity PACS into ePAD (http://epad.stanford.edu) (Rubin et al., Transl Oncol 7(1):23–35, 2014), a freely available AIM-compliant workstation, and the lesion measurement annotations were correctly linked by ePAD across sequential imaging studies. As quantitative imaging becomes more prevalent in radiology, interoperability of image annotations gains increasing importance. Our work demonstrates that image annotations in a vendor system lacking standard semantics can be automatically converted to a standardized metadata format such as AIM, enabling interoperability and potentially facilitating large-scale analysis of image annotations and the generation of high-quality labels for deep learning initiatives. This effort could be extended for use with other vendors' PACS.

Our work demonstrates that image annotations in a vendor system lacking standard semantics can be automatically converted to a standardized metadata format such as AIM, enabling interoperability and potentially facilitating large-scale analysis of image annotations and the generation of high-quality labels for deep learning initiatives. This effort could be extended for use with other vendors' PACS.

Background

There is a paucity of plasma‐based biomarkers that predict outcome in patients with head and neck squamous cell carcinoma (HNSCC) treated with chemoradiation therapy (CRT). Here, we evaluate the prognostic potential of plasma Melanoma‐Antigen Recognized by T‐cells 1 (MLANA) in this setting.

Methods

Breast Reduction and Mastopexy for Repair of Asymmetry After Breast Conservation Therapy: Lessons Learned

Abstract

Background

Breast conservation therapy (BCT) can cause breast distortion and asymmetry. Repair of this asymmetry by means of breast reduction or mastopexy procedures can be challenging and harbor considerably high rates of complications.

Methods

In this retrospective study, we describe our experience in repairing post-BCT breast asymmetry by performing breast reduction or mastopexy. The surgical protocol we followed consisted of stringent patient selection, thorough surgical planning, basic surgical refinements, and patient education for enhancing the likelihood of achieving a good outcome with minimal surgical complications.

Results

Our search of the departmental database identified 25 patients with breast asymmetry who had undergone breast reduction or mastopexy between 2009 and 2017. Corrective surgery was performed 4 years on average after the completion of radiotherapy, and those patients included eleven who had undergone breast reduction and fourteen who had undergone mastopexy on the radiated side. Two patients (8%) had major complications that required further surgery (major fat necrosis, wound infection, and breast deformation), and five patients (20%) had minor complications (infection, minor fat necrosis, wound dehiscence, and nipple congestion). All complications developed on the radiated breast. There was no correlation between the occurrence of complications and patients' demographics, tumor type, tumor location, and breast tissue resection (p > 0.05).

Conclusion

Only two of our 25 patients had major complications following breast reduction and mastopexy for the repair of asymmetry post-BCT. Following our four-step protocol was instrumental in leading to the successful performance of these procedures.

These data were collected after obtaining informed consent, conducting a structured clinical interview and completing the demographic checklist.

Findings

Data analysis demonstrated that there is a significant improvement in the index of female body image concern after rhinoplasty (p < 0.05). In the indices of sexual self-esteem and sexual body image, the analyses showed no significant difference between the pretest and posttest scores (p > 0.05).

Conclusion

Cosmetic surgery leads to the reduced body image concern of women but has no effect on sexual self-esteem and sexual body image. It seems that the psychological function of patients does not change significantly by rhinoplasty dimensions.

Breast conservation therapy (BCT) can cause breast distortion and asymmetry. Repair of this asymmetry by means of breast reduction or mastopexy procedures can be challenging and harbor considerably high rates of complications.

Methods

In this retrospective study, we describe our experience in repairing post-BCT breast asymmetry by performing breast reduction or mastopexy. The surgical protocol we followed consisted of stringent patient selection, thorough surgical planning, basic surgical refinements, and patient education for enhancing the likelihood of achieving a good outcome with minimal surgical complications.

Results

Our search of the departmental database identified 25 patients with breast asymmetry who had undergone breast reduction or mastopexy between 2009 and 2017. Corrective surgery was performed 4 years on average after the completion of radiotherapy, and those patients included eleven who had undergone breast reduction and fourteen who had undergone mastopexy on the radiated side. Two patients (8%) had major complications that required further surgery (major fat necrosis, wound infection, and breast deformation), and five patients (20%) had minor complications (infection, minor fat necrosis, wound dehiscence, and nipple congestion). All complications developed on the radiated breast. There was no correlation between the occurrence of complications and patients' demographics, tumor type, tumor location, and breast tissue resection (p > 0.05).

Conclusion

Only two of our 25 patients had major complications following breast reduction and mastopexy for the repair of asymmetry post-BCT. Following our four-step protocol was instrumental in leading to the successful performance of these procedures.

Changes in the Indices of Body Image Concern, Sexual Self-Esteem and Sexual Body Image in Females Undergoing Cosmetic Rhinoplasty: A Single-Group Trial

Abstract

Introduction

Psychological factors play a major role in the tendency toward cosmetic surgery. Variety seeking, making changes in one's face and reducing apparent dissatisfaction, improving body image and increasing sexual self-esteem can be considered as some psychological aspects of cosmetic surgery. The present study was conducted with the aim of evaluating the changes in the indices of body image concern, sexual self-esteem and sexual body image in patients undergoing rhinoplasty.

Method

This study was a single-group pretest–posttest quasi-experimental design. The research data were collected from March 2015 to January 2016. For this purpose, of the patients seeking nose surgery who referred to beauty centers in Mashhad, 40 subjects were selected through purposive method sampling and completed a Body Image Concern Inventory, Sexual Self-Esteem Index for Women and Body Exposure during Sexual Activities Questionnaire before and after rhinoplasty. These data were collected after obtaining informed consent, conducting a structured clinical interview and completing the demographic checklist.

Findings

Data analysis demonstrated that there is a significant improvement in the index of female body image concern after rhinoplasty (p < 0.05). In the indices of sexual self-esteem and sexual body image, the analyses showed no significant difference between the pretest and posttest scores (p > 0.05).

Conclusion

Cosmetic surgery leads to the reduced body image concern of women but has no effect on sexual self-esteem and sexual body image. It seems that the psychological function of patients does not change significantly by rhinoplasty dimensions.

Breast Reduction and Mastopexy for Repair of Asymmetry After Breast Conservation Therapy: Lessons Learned

Abstract

Background

Breast conservation therapy (BCT) can cause breast distortion and asymmetry. Repair of this asymmetry by means of breast reduction or mastopexy procedures can be challenging and harbor considerably high rates of complications.

Quality of life and disease severity in dermatitis patients with fragrance allergy ‐ a cross‐sectional European questionnaire study

Background

Fragrance contact allergy is common, but few studies on quality of life (QoL) in affected patients exist. Recently, the disease‐specific Fragrance Quality of Life Index (FQLI) was developed.

Objectives

To characterize QoL in European dermatitis patients with fragrance contact allergy at time of diagnosis.

Methods

Cross‐sectional study, inviting 2011 Danish, British, Spanish and German adult consecutive dermatitis patients to complete a questionnaire on self‐reported disease severity, the Dermatology Quality of Life Index (DLQI), and the FQLI immediately prior to being patch tested.

Results

1758 patients responded and 259 (14.7%, country range 11.7‐18.5%) were fragrance positive. Among these, total (SD) DLQI score was 6.2 (5.6) and total (SD) FQLI score was 60 (25), with both estimates being comparable to fragrance negative patients. Adjusting total FQLI score for age, sex and self‐rated disease severity revealed a higher (i.e. worse) score in fragrance positive patients (3.6‐point increase, 95% CI: 0.30 to 6.8 points, P=0.03). Specifically, contact allergy to oxidized linalool was associated with greater impairment in QoL.

Conclusions

QoL in patients with fragrance contact allergy is impaired at a level comparable to other patch tested dermatitis patients, at time of diagnosis across several European countries.
