A comparison of CD105 and CD31 expression in tumor vessels of hepatocellular carcinoma by tissue microarray and flow cytometry.
In this study, we investigated the endothelial markers CD105 and CD31 in the tumor vessels from 90 patients with hepatocellular carcinoma (HCC) by tissue microarray, in addition to their expression in TECs isolated from fresh tissues resected from 11 patients with HCC by flow cytometry and confocal microscopy. The results revealed that among 90 cases of TMA, all tumor vessels were CD31 positive whereas 39 cases (43.3%) had little or no CD105 expression in tumors and their vessels but not peritumoral tissue spots, and that among these 39, 29 cases (74.4%) were poor-differentiated HCC. Duftstoffe als Allergene
Zusammenfassung
Kontaktallergien durch Duftstoffe sind häufig. Zählt man die Reaktionshäufigkeiten der Indikatoren einer Duftstoffallergie zusammen (Duftstoffmix [DM] I + II, Perubalsam und Terpentinöl), dann erreichen sie im Klinikkollektiv (IVDK-Daten 2016) 17 %, und liegen damit sogar vor der Nickelallergie (etwa 16 %). Bei den „+"-positiven Reaktionen auf den DM I reagieren > 50 % nicht auf eine der Komponenten. Derartige Reaktionen sollten zurückhaltend als allergisch interpretiert werden. Es sei denn, weitere Daten (z. B. positive Anamnese, Gebrauchstest) stützen eine solche Interpretation.
DM-I-Positive reagieren häufig auf Eichenmoos (27,2 %) und Isoeugenol (18,2 %). Es folgen Hydroxycitronellal (10,9 %), Cinnamal (10,6 %), Cinnamyl alcohol (8,4 %), Eugenol (7,3 %), Geraniol (5,5 %) und Amyl Cinnamal (1,5 %). Die Sensibilisierungshäufigkeiten (SH) der einzelnen Duftstoffe sind abhängig von der allergenen Potenz (erfasst durch den lokalen Lymphknotentest; englisch „local lymph node assay", LLNA) und der Nutzungshäufigkeit (Maß der Exposition). Die Relation zwischen SH und Exposition wird mit dem SEQ („sensitization exposure"-Quotient) beschrieben. Der SEQ ist ein Maß für das Sensibilisierungsrisiko. In der Regel korreliert dies einerseits mit der SH, andererseits mit der allergenen Potenz („hazard"/Gefahr). Abweichungen von dieser Regel zeigen ein neu erkanntes Problem an: a) Methyl 2-octynoat spielte in der SH-Statistik keine Rolle, lag aber mit seinem SEQ (= Risiko) nach Baum- und Eichenmoos an dritter Stelle, b) Hydroxyisohexyl 3-cyclohexene carboxaldehyde (Lyral®) hatte trotz schwacher Potenz ein relativ hohes Risiko. Die Determinante des Risikos war in diesem Fall nicht die Potenz (das inhärente „hazard"), sondern zu hohe Einsatzdosen.
Die Allergieüberwachung durch die klinische Epidemiologie sollte in Zukunft verstärkt ergänzt werden durch Daten zur Exposition.

Ein Schritt zur besseren Verfügbarkeit
Die Verfügbarkeit von Testallergenen zur Abklärung von Allergien, die gegen seltene Allergieauslöser gerichtet sind, ist von öffentlichem Interesse. In den letzten Jahren haben Zulassungsinhaber aber viele Zulassungen von Testallergenen für seltene Allergieauslöser zurückgegeben — Ursache: der geringe wirtschaftlich Nutzen. Dem will das Paul-Ehrlich-Institut nun entgegenwirken und reduziert die Gebühren für Amtshandlungen im Zusammenhang mit diesen Arzneimitteln.

Comparison of sublingual immunotherapy and oral immunotherapy in peanut allergy
Abstract
The prevalence of food allergy has been increasing over the past few decades at an alarming rate with peanut allergy affecting about 2 % of children. Both oral immunotherapy (OIT) and sublingual immunotherapy (SLIT) have shown promise as a treatment option for peanut allergy. Immunotherapy induces desensitization and reduces the risk of reaction during accidental ingestion and may also enable those who are successfully desensitized to include the food allergen in their diet. OIT has been very well studied and has been found to be more efficacious than SLIT with an acceptable safety profile. However, SLIT is associated with fewer side effects. Studies indicate that a combination of SLIT and OIT may together induce a significant increase in challenge thresholds with fewer adverse events. More head-to-head clinical trials that directly compare OIT and SLIT as well as SLIT and OIT combination studies are warranted.

Duftstoffe als Allergene
Zusammenfassung
Kontaktallergien durch Duftstoffe sind häufig. Zählt man die Reaktionshäufigkeiten der Indikatoren einer Duftstoffallergie zusammen (Duftstoffmix [DM] I + II, Perubalsam und Terpentinöl), dann erreichen sie im Klinikkollektiv (IVDK-Daten 2016) 17 %, und liegen damit sogar vor der Nickelallergie (etwa 16 %). Bei den „+"-positiven Reaktionen auf den DM I reagieren > 50 % nicht auf eine der Komponenten. Derartige Reaktionen sollten zurückhaltend als allergisch interpretiert werden. Es sei denn, weitere Daten (z. B. positive Anamnese, Gebrauchstest) stützen eine solche Interpretation.
DM-I-Positive reagieren häufig auf Eichenmoos (27,2 %) und Isoeugenol (18,2 %). Es folgen Hydroxycitronellal (10,9 %), Cinnamal (10,6 %), Cinnamyl alcohol (8,4 %), Eugenol (7,3 %), Geraniol (5,5 %) und Amyl Cinnamal (1,5 %). Die Sensibilisierungshäufigkeiten (SH) der einzelnen Duftstoffe sind abhängig von der allergenen Potenz (erfasst durch den lokalen Lymphknotentest; englisch „local lymph node assay", LLNA) und der Nutzungshäufigkeit (Maß der Exposition). Die Relation zwischen SH und Exposition wird mit dem SEQ („sensitization exposure"-Quotient) beschrieben. Der SEQ ist ein Maß für das Sensibilisierungsrisiko. In der Regel korreliert dies einerseits mit der SH, andererseits mit der allergenen Potenz („hazard"/Gefahr). Abweichungen von dieser Regel zeigen ein neu erkanntes Problem an: a) Methyl 2-octynoat spielte in der SH-Statistik keine Rolle, lag aber mit seinem SEQ (= Risiko) nach Baum- und Eichenmoos an dritter Stelle, b) Hydroxyisohexyl 3-cyclohexene carboxaldehyde (Lyral®) hatte trotz schwacher Potenz ein relativ hohes Risiko. Die Determinante des Risikos war in diesem Fall nicht die Potenz (das inhärente „hazard"), sondern zu hohe Einsatzdosen.
Die Allergieüberwachung durch die klinische Epidemiologie sollte in Zukunft verstärkt ergänzt werden durch Daten zur Exposition.

Im Jahr 2016 kam es zu einem Tiefpunkt in der Versorgung von Patienten mit allergischer Rhinitis in Deutschland: Durch die Entlassung der nasalen Glukokortikoide Beclomethason, Fluticason und Mometason aus der Verschreibungspflicht waren plötzlich fast keine Medikamente für Rhinitispatienten mehr zulasten der gesetzlichen Krankenversicherungen rezeptierbar. Der Ärzteverband Deutscher Allergologen hat sich von Anfang an mit der Problematik auseinandergesetzt — mit Erfolg.

Acht Jahre ist es her, dass die dritte Auflage des „Weißbuch Allergie in Deutschland" erschienen ist. Derzeit wird das Werk rundum erneuert — mit dem Ziel, die Öffentlichkeit besser über Allergien zu informieren, die aktuellen Probleme in der Allergologie dar- sowie entsprechende Lösungsvorschläge vorzustellen. Voraussichtlich ab November wird die vierte Auflage des Weißbuchs im Springer Medizin Verlag erscheinen.

Der AeDA hat für die deutschen Allergologen in den letzten Jahren die Aufnahme von Diagnostikallergenen für nasale Provokationstestungen in die Sprechstundenbedarfsverordnungen der meisten KV-Regionen erreicht — vor dem Hintergrund, die wirtschaftlichen Zwänge bei der Erbringung allergologischer Leistungen zu verbessern. In Hessen wurde nun diese Vereinbarung bis zum 30. Juni 2019 verlängert.

A game of give and take: category change on the border between adverbial verbal gerunds and augmented absolutes in English
This article discusses one of several reasons for the loss of augmentation in English absolute constructions (ACs): the emergence of the verbal gerund (VG) in -ing. The VG was from the start strongly associated with prepositional contexts (de Smet, 2008) and has been shown to have a history connected to the AC (Fanego, 2004). More specifically, the strong association between VGs and prepositions probably caused a reinterpretation of most prepositionally augmented ACs as VGs, resulting in a functional split (Fonteyn & van de Pol, 2016). The Tip of the Iceberg in Maternally Inherited Diabetes and Deafness.
We report a single case of a patient with MIDD which manifested clinically at 41 years old. In addition to diabetes and deafness, he manifested with seizures, ataxia, myopathy, and hypogonadism. We used established methods for the routine workup of this patient. MIDD is indeed a multisystem condition. A previously undescribed phenotypic feature of MIDD may be hypogonadism. Craniofacial microsomia (CFM) is most often described as a unilateral malformation of derivatives of the first and second branchial arches. The mandible has been classified using several classification systems. However, all are based on two-dimensional imaging. The aim of this study was to mathematically describe the deformed mandible based on principal component analysis (PCA) in a three-dimensional way. This may aid in defining the flaws in existing surgical corrections of the mandible through the identification of the differences in shape compared with a normal mandible in a holistic view with the help of videos.

A pilot study on adjunctive use of parametric colour-coded digital subtraction angiography in endovascular interventions of haemodialysis access
Abstract
Background
Two-dimensional digital subtraction angiography (DSA) is the gold standard for angiographic evaluation of dysfunctional haemodialysis access. We aim to investigate the utility of parametric colour coded DSA in providing hemodynamic analysis during haemodialysis access interventions.
Methods
We retrospectively studied 20 patients who underwent access intervention and applied parametric colour-coding on selected DSA acquisitions before and after percutaneous transluminal angioplasty (PTA). The difference in time to peak (dTTP) contrast enhancement and time attenuation curve (TAC) of pre- and post-stenotic regions of interest (ROIs) were obtained and compared after treatment.
Results
Improvements were seen in mean percent of stenosis after PTA (p < 0.0001) for all cases. Median dTTP improved from 0.52 (IQR 0.26, 0.8) to 0.25 (IQR 0, 0.26) seconds (p = 0.001). Median 50% contrast washout time improved from 0.77 (IQR 0.39, 1.17) to 0.42 (IQR 0.23, 0.59) seconds (p = 0.031). Conclusions
Adjunctive use of parametric colour-coded DSA may provide potentially useful hemodynamic information during vascular access interventions. Larger prospective studies are needed to validate our findings.

A pilot study on adjunctive use of parametric colour-coded digital subtraction angiography in endovascular interventions of haemodialysis access
Abstract
Background
Two-dimensional digital subtraction angiography (DSA) is the gold standard for angiographic evaluation of dysfunctional haemodialysis access. We aim to investigate the utility of parametric colour coded DSA in providing hemodynamic analysis during haemodialysis access interventions.
Methods
We retrospectively studied 20 patients who underwent access intervention and applied parametric colour-coding on selected DSA acquisitions before and after percutaneous transluminal angioplasty (PTA). The difference in time to peak (dTTP) contrast enhancement and time attenuation curve (TAC) of pre- and post-stenotic regions of interest (ROIs) were obtained and compared after treatment.
Results
Improvements were seen in mean percent of stenosis after PTA (p < 0.0001) for all cases. Median dTTP improved from 0.52 (IQR 0.26, 0.8) to 0.25 (IQR 0, 0.26) seconds (p = 0.001). Generosity and Duration of Medicaid Expansion Waivers and Access to Care

Introduction
Prior to expansion of Medicaid under the Affordable Care Act, some states obtained Section 1115 waivers from the federal government that allowed them to expand eligibility for Medicaid to adult populations that were not covered previously. Expansion waivers in these states differed in their generosity and year of implementation, creating variation in coverage availability and program longevity across states. This study examined the association between generosity and duration of Medicaid expansion waivers and access to preventive services.
Methods
The 2012 Medical Expenditure Panel Survey data were used to estimate adjusted logistic models in 2016, comparing outcomes among low-income non-elderly adults living in generous (Medicaid eligibility threshold ≥138% federal poverty level) and moderate (Medicaid eligibility threshold <138% federal poverty level) waiver states, relative to no-waiver states.
Results
Moderate and generous waivers were associated with statistically significant (p<0.001) increases in probabilities of having a usual source of care and a blood pressure check, relative to states without a waiver to expand. Low-income individuals living in states with longer waiver durations had better access to healthcare services than a similar group living in comparison states.
Conclusions
Not only is Medicaid waiver generosity associated with improving access to healthcare services, but the combination of generosity and longer duration of a waiver also intensifies the association. As states gain flexibility in designing their Medicaid programs, the healthcare benefits associated with both generosity and duration of waivers are important considerations for policy makers.

Introduction
Validity of research linking built environments to health relies on the availability and reliability of data used to measure exposures. As cities transform, it is important to track when and where urban changes occur, to provide detailed information for urban health intervention research. This paper presents an online observation method of the implementation of traffic-calmingfeatures using Google Street View Time Machine. The method is used to validate an existingadministrative database detailing the implementation of curb extensions and speed bumps.
Methods
Online observation of curb extensions and speed bumps was conducted for four boroughsin Montreal, Canada, in autumn 2016, and compared with administrative data documenting traffic-calming measures implemented between 2008 and 2014. All images available through the Time Machine function between 2007 and 2016 for 708 intervention sites were visualized online. Records in the administrative database were compared to real-world Google Street View observations and tested in terms of sensitivity, specificity, and positive predicted value.
Results
Google Street View Time Machine allowed the visualization of a median of seven different dates per street intersection and six dates per street segment. This made it possible to analyze built environment changes within 3,973 distinct time periods at 708 locations. Validation of the administrative data regarding presence of an intervention showed 99% (95% CI=97%, 99%) sensitivity, 58% (95% CI=51%, 64%) specificity, and 77% (95% CI=73%, 81%) positive predictive value.
Conclusions
Google Street View Time Machine allowed past (2007–2016) online documentation of microscale urban interventions—curb extensions and speed bumps. The proposed method offers a new way to document historic changes to the built environment, which will be useful for urban health intervention research.
