Efficacy of a thermally treated single file compared with rotary systems in endodontic retreatment of curved canals: a micro-CT study
Abstract
Objectives
This study aimed to compare WaveOne Gold with ProTaper and RaCe systems regarding remaining filling material, apical transportation (AT), and working time (WT) after (i) filling removal and (ii) shaping of curved canals.
Methods
Thirty mesiobuccal canals of maxillary molars were prepared and filled. After 30 days, they were randomly assigned into three groups (n = 10), according to the instruments used for filling removal and shaping, respectively: WOG—WaveOne Gold Primary and Medium; PTG—ProTaper Retreatment and ProTaper Next; RCG—D-RaCe and RaCe. Micro-CT analysis assessed the residual filling material and AT. WT was recorded. Data were statistically analyzed (α = .05).
Results
There was no significant difference between groups in the amount of filling material at any evaluated moment (P > .05). All groups presented low AT values. The WT was similar in all groups in filling removal (P > .05), and in shaping step WOG was faster than PTG and RCG (P < .05).
Conclusions
Neither system could completely remove the filling material. The instruments evaluated were safe and the reciprocating system was faster than the rotary systems in shaping the canals.
Clinical relevance
This study provided consistent information on filling material removal capacity of WaveOne Gold. Considering that all tested systems were safe, WaveOne Gold may be an alternative with cost-effectiveness and shorter learning curve for endodontic retreatment.

The Perioperative Management of Ascending Aortic Dissection
Acute aortic syndromes are a distinct group of pathologies involving the wall of the aorta that present acutely and can be potentially fatal unless treated in a timely fashion. The syndrome is dominated by aortic dissections, which comprise ≥95% of all such presentations. Those involving the ascending aorta are particularly lethal and require specific and early surgical treatment compared to dissections involving other parts of the aorta. The surgical repair of an ascending aortic dissection presents multiple challenges to the anesthesiologist. Thoughtful management throughout the perioperative period is critical for minimizing the significant morbidity and mortality associated with this condition. In this narrative review, we provide an overview of the perioperative management of patients presenting for the surgical repair of an ascending aortic dissection. Preoperative discussion focuses on assessment, hemodynamic management, and risk stratification. The intraoperative section includes an overview of anesthetic management, transesophageal echocardiographic assessment, and coagulopathy, as well as surgical considerations that may influence anesthetic management. Education in Anesthesia: How to Deliver the Best Learning Experience
Provider Education and Vaporizer Labeling Lead to Reduced Anesthetic Agent Purchasing With Cost Savings and Reduced Greenhouse Gas Emissions
Anesthetic agents are known greenhouse gases with hundreds to thousands of times the global warming impact compared with carbon dioxide. We sought to mitigate the negative environmental and financial impacts of our practice in the perioperative setting through multidisciplinary staff engagement and provider education on flow rate reduction and volatile agent choice. These efforts led to a 64% per case reduction in carbon dioxide equivalent emissions (163 kg in Fiscal Year 2012, compared with 58 kg in Fiscal Year 2015), as well as a cost savings estimate of $25,000 per month. In this study high purity grade hemp flour (HF) and hemp protein isolate (HPI) were obtained through a fast and cheap process starting from defatted hemp cakes, a residue of hempseed oil extraction. HPI resulted enriched at nearly 86% protein, mainly constituted by the storage protein edestin (accounting for 70% total protein). In vitro protein digestibility was determined using a static model of gastrointestinal digestion (GID), which included a final step with purified brush border membrane (BBM) enzyme preparations. HF and HPI showed a high degree of digestibility. The survival of potential bioactive and/or allergenic peptide sequences in digests was investigated by peptidomic analysis. Utility of TERT Promoter Mutations for Cutaneous Primary Melanoma Diagnosis
Abstract: Telomerase reverse transcriptase (TERT) promoter mutations are commonly found in malignant melanomas but rare in melanocytic nevi. To assess its potential diagnostic utility for the distinction of melanoma from nevus, we determined the TERT promoter mutation status of 86 primary melanomas, 72 melanocytic nevi, and 40 diagnostically problematic melanocytic proliferations. Of the 86 melanomas, 67 (77.9%) were TERT-positive, defined as harboring a hotspot TERT promoter mutation at positions -124C>T, -124_125CC>TT, -138_139CC>TT, or -146C>T. Of the 72 nevi, only 1 (1.4%) was TERT-positive. Of the 40 diagnostically uncertain melanocytic proliferations, 2 (5.0%) were TERT-positive. TERT positivity as a test for melanoma versus nevus had an accuracy of 87.3% [95% confidence interval (CI), 81.1–92.1], a sensitivity of 77.9% (95% CI, 68.9–85.4), a specificity of 98.6% (95% CI, 95.8–100), a positive predictive value of 98.5% (95% CI, 95.6–100), and a negative predictive value of 78.9% (95% CI, 72.6–85.4). Our results indicate that hotspot TERT promoter mutation status may be a useful ancillary parameter for the diagnosis of melanoma. In particular, the high specificity of these mutations for melanoma indicates the presence of a TERT promoter mutation in a melanocytic neoplasm associated with diagnostic controversy, or uncertainty should increase concern for a melanoma. Facial Phaeohyphomycosis in an Immunocompetent Individual: A Rare Presentation of a Rare Fungus
Abstract: Phaeohyphomycosis is a rare mycotic infection caused by heterogenous groups of dematiaceous fungi involving the skin and subcutaneous tissue. Here, we report a case of cutaneous phaeohyphomycosis presenting as cauliflower-like verrucous lesion over the face in an immunocompetent individual. Histopathology showed suppurative granulomatous inflammation replete with pigmented broad fungal hyphae which is stained with periodic acid–Schiff stain, Grocott's methanamine silver stain, Schmorl's stain, and Masson-Fontana stain. Culture showed black-colored colonies identified as Exophiala spinifera. Pigmented Facial Contact Dermatitis to Benzyl Salicylate: A Comparative Histopathological and Immunohistochemical Study of the Involved Skin and the Positive Patch Test Site
Abstract: Pigmented contact dermatitis (PCD) is a noneczematous variant of allergic contact dermatitis, and benzyl salicylate is one of its causes. This type of PCD shows nonlichenoid interface dermatitis with pigment incontinence. We aimed to characterize the earliest histopathological changes of this reaction. A 51-year-old man presented with persistent facial eruption composed of hyperpigmented and hypopigmented macules due to exposure to benzyl salicylate present in his aftershave. The biopsies obtained from hyperpigmented and hypopigmented macules, and from the positive patch test site to benzyl salicylate, showed a nonlichenoid focal vacuolar interface dermatitis with mononuclear cells in the papillary dermis and around the pilosebaceous units, along with melanophages. A MART-1 immunostain showed intact melanocytes in all 3 biopsies. A Fontana–Masson stain demonstrated intact melanin in the basal cell layer of a facial hyperpigmented macule and the patch test site, but melanin was reduced in the biopsy taken from a hypopigmented facial macule. There were more epidermal and dermal CD1a+ Langerhans cells in the patch test biopsy than in the other 2 biopsies. Most of the mononuclear cells were CD3+. The CD4+ to CD8+ ratio was approximately 1:1 in the facial macules; yet, CD4+ cells outnumbered CD8+ cells in the patch test biopsy. There were a few TIA-1+ cells in all 3 biopsies. In conclusion, the earliest histopathological and immunophenotypical events in PCD due to benzyl salicylate are similar to those of longer-standing lesions, i.e., a nonlichenoid focal interface dermatitis involving the epidermis and pilosebaceous unit, along with dermal melanophages. Atypical Varicella-Zoster Kaposi Varicelliform Eruption in Sézary Syndrome
Abstract: Patients with mycosis fungoides experience considerable morbidity and mortality from secondary bacterial and viral infections. Staphylococcus aureus, β-hemolytic streptococci, herpes simplex virus, and herpes zoster virus remain the most common infectious pathogens in this group of patients. With depressed cellular immunity and diminished skin barrier as the main precipitating risk factors, immunocompromised patients can often present with an atypical presentation of a common dermatologic condition. The case herein discusses a clinically atypical nonvesicular Kaposi varicelliform eruption secondary to a varicella-zoster virus in a patient with Sézary syndrome. Concurrent polypharmacy in these patients is also a risk factor for development of drug hypersensitivity reactions. However, given their immunocompromised status, first and foremost, a careful inspection should be made of every atypical skin eruption in search of an infectious etiology, and afterward, an appropriate treatment should be promptly initiated. Role of herpesviruses in coronary artery disease

Coronary artery disease (CAD) is a huge global burden and is a leading cause of morbidity and mortality across the world. We have done this review to elucidate the pathogenesis of herpesviruses in causing CAD and to study an association between herpesviruses (cytomegalovirus [CMV] and herpes simplex virus [HSV-1 and -2]) and CAD. CMV can cause atherosclerosis directly through the activity of its gene products on endothelial cells, monocytes/macrophages, and smooth muscle cells and indirectly through production of pro-inflammatory cytokines at far off place. HSV infection causes atherosclerosis mainly by causing increased prothrombotic activity on endothelial cells, accumulation of cholesterol esters and triacylglycerols in vascular smooth muscle cells, and upregulating expression of Lectin like oxidized Low density lipoprotein receptor-1 (LOX-1) receptor on macrophages. The association between CMV and CAD is related to its seroprevalence with a positive association in developing countries (higher seroprevalence) and no significant association in developed countries (lower seroprevalence). However, the association between HSV infection and atherosclerosis is not related to its seroprevalence.

Patients were followed up, and the role of central aortic pressure indices in the prediction of cardiovascular events was analyzed. Results: A total of 623 patients were enrolled. Central pulsatility was found to be most closely associated with severity of CAD by univariate analysis, with age, sex, and smoking status being important predictors of pulsatility. A positive correlation was found between the central and peripheral mean and pulse pressures. No significant association was noted between the central aortic pressures and MACE on short-term follow-up. Conclusions: Of the various central aortic pressure indices studied, central pulsatility was found to be most closely associated with the severity of CAD.

The prediction of left main and/or triple-vessel coronary artery disease by tissue doppler-based longitudinal strain and strain-rate imaging

Background: In the absence of evident regional wall motion abnormality (RWMA) at rest, detection of severe coronary artery disease (CAD) usually requires a stress test. Ventricular strain is a more sensitive parameter of myocardial systolic function and may be abnormal in patients with severe CAD. Thus, this study aimed to evaluate the longitudinal strain and strain-rate imaging for prediction of left main (LM) and triple-vessel disease CAD with normal left ventricular ejection fraction (LVEF) and no RMWA. Materials and Methods: Patients included were of stable CAD, unstable angina, or non-ST segment elevation myocardial infarction with LVEF >50% and without RWMA. A thorough echocardiographic evaluation to assess peak systolic longitudinal strain (PSLS) and PSLS rate (PSLSR) in 16 segments of the left ventricle was done. The visual evaluation of percent diameter stenosis of the angiographic lesions was done according to the American Heart Association classification. Patients were grouped into high-risk group (LM or triple-vessel CAD), low-risk group (CAD other than high risk), and normal group (absence of CAD). Results: A total of 86 patients were studied. Of which, 60 were male and 26 were female. The global PSLS and PSLSR were lower in the high-risk group as compared to the other two groups (P < 0.001). The combined basal plus mid-PSLS and PSLSR were lower in the high-risk group (P < 0.001). Receiver operating characteristic curve analysis showed an optimal cutoff value of −17.3% (sensitivity 82% and specificity 80%) for global PSLS and −15.0% (sensitivity 75% and specificity 73%) for basal plus mid-PSLS for detection of high-risk CAD. Similarly, a cutoff value of −0.74 s−1 (sensitivity 82% and specificity 73.6%) for global PSLSR and −0.66 s−1 (sensitivity 85.7% and specificity 86.7%) for basal plus mid-PSLSR were calculated for the prediction of high-risk CAD. Conclusion: The PSLS and PSLSR were lower in patients having high-risk CAD that included LM and triple-vessel disease, even when resting wall motion and LVEF were normal. The study concluded that PSLS and PSLSR are sensitive and specific noninvasive modalities for predicting the possibility of high-risk CAD in the presence of normal LV systolic function and absence of resting RWMA.

A comparative study of tirofiban plus enoxaparin versus enoxaparin alone along with dual antiplatelet blockade in the management of patients with non-ST elevation acute coronary syndrome

Introduction: Antithrombotic therapy in patients with non-ST elevation acute coronary syndrome (NSTE-ACS) includes dual antiplatelet therapy (DAPT) along with enoxaparin. However, resistance to antiplatelet action of aspirin and clopidogrel is well known and is about 27% (0-57%) and 30%, respectively. Hence the use of GPIIb/IIIa inhibitor appears to be a reasonable option as they act on the final common pathway of platelet aggregation. Though frequently used in patients undergoing percutaneous intervention (PC1), their use in patients not undergoing such procedure is not popular despite evidence in literature. Materials and Methods: This study was done on 44 patients of NSTE-ACS managed conservatively. The patients were randomized in a 1:1 fashion in two groups. The patients in group 1 received tirofiban(0.4μg/kg/min i.v for 30 min followed by 0.1μg/kg/min for 48 hours) in addition to aspirin (325 mg stat followed by 75 mg P.O. daily), clopidogrel (300 mg stat followed by 75 mg P.O. daily) and enoxaparin (1 mg/kg S.C bid for 5 days). The group 2 patients received DAPT and enoxaparin only, in similar doses. Both the groups received antianginal therapy as appropriate and statins. Results: The study showed the benefits of adding tirofiban to enoxaparin and DAPT irrespective of age, sex, presence or absence of diabetes and dyslipidemia, ECG changes, troponin positivity and TIMI score in patients with NSTE-ACS. There was a significant reduction {Risk reduction (RR) of 45.4% P <0.01} in composite of primary end points refractory ischemia myocardial infarction (MI) and death with Tirofiban. No major or minor bleeding episodes were seen in any patients. Conclusion: Thus, we conclude that tirofiban in addition to DAPT and enoxaparin reduces the risk of refractory ischemia, MI and death in patients with NSTE-ACS without any additional risk of major or minor bleeding. Therefore, we advocate this regimen in patients with NSTE-ACS managed conservatively. Large randomized study is needed to recommend this regimen.

Comparison of left internal mammary harvesting techniques for coronary artery bypass grafting: A prospective study

Introduction: Left internal mammary artery (LIMA) is the most common arterial conduit used for patients undergoing coronary artery bypass grafting (CABG) surgery. Maintaining pleural integrity during harvesting has been documented to have better patient outcomes. Hence, we evaluated postoperative outcomes using two different LIMA harvesting techniques, one with intact pleura and other with pleura opened. Methods: In this prospective study, we recruited a total of 110 patients undergoing CABG. Based on the pleural being intact or opened, the patients were divided in two groups. In Group 1, LIMA was harvested with pleura intact (extra-pleural approach) while in Group 2, LIMA was harvested with pleura being opened (intrapleural / pleurotomy). The primary objective was to evaluate the impact on various post-operative parameters like total ventilatory time, mediastinal drainage, need for Blood transfusions, post operative deterioration of lung function , post operative pain scores and duration hospital stay. Results: Out of 110 patients undergoing CABG, LIMA was harvested by extra-pleural approach in sixty-six while in the rest forty-four pleurotomy was needed. The baseline demographic attributes of patients were similar in both groups. The post operative parameters such as ventilatory time, blood loss, need for blood transfusions, and prolonged hospital stay were found to be significantly higher in the group with intrapleural LIMA harvest technique (P <0.05). Pulmonary complications like pleural effusion, atelectasis were more frequent with in the intrapleural harvest as compared to extra-pleural harvest technique.(P <0.001). Post operative pain scores as evaluated by NRS scale were also significantly higher after intrapleural harvest (P <0.001). Conclusions: Harvesting of LIMA for CABG extrapleurally has significant post-operative benefits and faster recovery compared to pleurotomy and intraplueral approach.

Introduction: Left internal mammary artery (LIMA) is the most common arterial conduit used for patients undergoing coronary artery bypass grafting (CABG) surgery. Maintaining pleural integrity during harvesting has been documented to have better patient outcomes. Hence, we evaluated postoperative outcomes using two different LIMA harvesting techniques, one with intact pleura and other with pleura opened. Methods: In this prospective study, we recruited a total of 110 patients undergoing CABG. Based on the pleural being intact or opened, the patients were divided in two groups. In Group 1, LIMA was harvested with pleura intact (extra-pleural approach) while in Group 2, LIMA was harvested with pleura being opened (intrapleural / pleurotomy). The primary objective was to evaluate the impact on various post-operative parameters like total ventilatory time, mediastinal drainage, need for Blood transfusions, post operative deterioration of lung function , post operative pain scores and duration hospital stay. Results: Out of 110 patients undergoing CABG, LIMA was harvested by extra-pleural approach in sixty-six while in the rest forty-four pleurotomy was needed. The baseline demographic attributes of patients were similar in both groups. The post operative parameters such as ventilatory time, blood loss, need for blood transfusions, and prolonged hospital stay were found to be significantly higher in the group with intrapleural LIMA harvest technique (P <0.05). Pulmonary complications like pleural effusion, atelectasis were more frequent with in the intrapleural harvest as compared to extra-pleural harvest technique.(P <0.001). Post operative pain scores as evaluated by NRS scale were also significantly higher after intrapleural harvest (P <0.001). Conclusions: Harvesting of LIMA for CABG extrapleurally has significant post-operative benefits and faster recovery compared to pleurotomy and intraplueral approach.

Introduction: Antithrombotic therapy in patients with non-ST elevation acute coronary syndrome (NSTE-ACS) includes dual antiplatelet therapy (DAPT) along with enoxaparin. However, resistance to antiplatelet action of aspirin and clopidogrel is well known and is about 27% (0-57%) and 30%, respectively. Hence the use of GPIIb/IIIa inhibitor appears to be a reasonable option as they act on the final common pathway of platelet aggregation. Though frequently used in patients undergoing percutaneous intervention (PC1), their use in patients not undergoing such procedure is not popular despite evidence in literature. Materials and Methods: This study was done on 44 patients of NSTE-ACS managed conservatively. The patients were randomized in a 1:1 fashion in two groups. The patients in group 1 received tirofiban(0.4μg/kg/min i.v for 30 min followed by 0.1μg/kg/min for 48 hours) in addition to aspirin (325 mg stat followed by 75 mg P.O. daily), clopidogrel (300 mg stat followed by 75 mg P.O. daily) and enoxaparin (1 mg/kg S.C bid for 5 days). The group 2 patients received DAPT and enoxaparin only, in similar doses. Both the groups received antianginal therapy as appropriate and statins. Results: The study showed the benefits of adding tirofiban to enoxaparin and DAPT irrespective of age, sex, presence or absence of diabetes and dyslipidemia, ECG changes, troponin positivity and TIMI score in patients with NSTE-ACS. There was a significant reduction {Risk reduction (RR) of 45.4% P <0.01} in composite of primary end points refractory ischemia myocardial infarction (MI) and death with Tirofiban. No major or minor bleeding episodes were seen in any patients. Conclusion: Thus, we conclude that tirofiban in addition to DAPT and enoxaparin reduces the risk of refractory ischemia, MI and death in patients with NSTE-ACS without any additional risk of major or minor bleeding. Therefore, we advocate this regimen in patients with NSTE-ACS managed conservatively. Large randomized study is needed to recommend this regimen.

I decided to raise our students' awareness of their classmates with disabilities. At South Walker (Louisiana) Elementary, I treat students in our significant disabilities classroom, several resource classes and two special-needs pre-K classes. I wanted our typical students to better understand their special education classmates, so I hosted an event called "Understanding Uniqueness." Short and long-term impact of remifentanil on thermal detection and pain thresholds after cardiac surgery: A randomised controlled trial
BACKGROUND The clinical relevance of the suggested hyperalgesic effects of remifentanil is still unclear, especially in the long term. OBJECTIVE The current study evaluated the impact of remifentanil on thermal thresholds 3 days and 12 months after surgery, measured with Quantitative Sensory Testing. DESIGN A prospective, single-blind, randomised controlled trial. SETTING A tertiary care teaching hospital in The Netherlands, from 2014 to 2016. PATIENTS A total of 126 patients aged between 18 and 85 years, undergoing cardiothoracic surgery via sternotomy (coronary artery bypass grafts and/or valve replacement) were included. Exclusion criteria were BMI above 35 kg m−2, history of cardiac surgery, chronic pain conditions, neurological conditions, allergy to opioids or paracetamol, language barrier and pregnancy. INTERVENTIONS Patients were allocated randomly to receive intra-operatively either a continuous remifentanil infusion or intermittent intra-operative fentanyl as needed in addition to standardised anaesthesia with propofol and intermittent intravenous fentanyl at predetermined time points. MAIN OUTCOME MEASURES Warm and cold detection and pain thresholds 3 days and 12 months after surgery. In addition the use of remifentanil, presence of postoperative chronic pain, age, opioid consumption and pre-operative quality of life were tested as a predictor for altered pain sensitivity 12 months after surgery. RESULTS Both warm and cold detection, and pain thresholds, were not significantly different between the remifentanil and fentanyl groups 3 days and 12 months after surgery (P > 0.05). No significant predictors for altered pain sensitivity were identified. CONCLUSION Earlier reports of increased pain sensitivity 1 year after the use of remifentanil could not be confirmed in this randomised study using Quantitative Sensory Testing. This indicates that remifentanil plays a minor role in the development of chronic thoracic pain. Still, the relatively high incidence of chronic thoracic pain and its accompanying impact on quality of life remain challenging problems. According to 2016-2017 data from the American Association of Oral and Maxillofacial Surgeons, there are 102 accredited oral and maxillofacial surgery (OMS) programs, and 1190 residents. This data also shows a total of 432 faculty members nationwide, all of whom are responsible for passing on knowledge, expertise and wisdom to the rising young surgeons of tomorrow. Similar to the days of Hippocrates, the artistry and science of surgery has been passed along through generations by means of a teacher-acolyte relationship.
